NOTICE OF VIOLATION (S): None
You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be assessed for each
violation of the Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be subject to enforcement actions
including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such
further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

STATUS OF PAST NOTED VIOLATIONS:
The following are violations noted on the July 24, 2003 audit.
Permit condition #11 a. Unresolved
15A NCAC 13B .0505 (10)(e) Unresolved
15A NCAC 13B .0505 (11)(a) Unresolved
15A NCAC 13B .0505 (11)(b) Unresolved
The above Violations have not been resolved as of this date. A response to my letter of August 11, 2003 has not
been received. The contingency plan was reviewed today with Mr. Smith and Mr. Syck. The plan will need to
be updated to address concerns that we have on handling and identifying wastes accepted at the facility.

OTHER COMMENTS /SUGGESTIONS:
1. Progress has been made on the LCID unit. An extension of 30 days will be granted to complete the required
cover. Attention should be given to the perimeter slopes to ensure that adequate cover is provided and that the
slopes do not exceed a 3:1 slope.
2. The C& D unit has not been cover as required in the July 24th Notice of Violation. An extension of 14 days, from
receipt of this notice, will be granted to gain compliance.
3. I am in receipt of the August 12, 2003 letter to Jim Barber requesting information on extending this permit.
