NOTICE OF VIOLATION: None
You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be assessed for each
violation of the Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be subject to enforcement actions
including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such
further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

STATUS OF PAST NOTED VIOLATIONS:
None
OTHER COMMENTS /SUGGESTIONS:
1. I am in receipt of the August 12, 2003 letter to Jim Barber requesting information on extending this permit. Efforts
to extend this permit need to continue.
2. The Construction and Demolition waste is being disposed of into a small area and is being managed well.
3. The provisions for ground cover need to be completed within 30 days after grading is completed on the C&D waste
and the LCID unit. Previously disturbed areas need to be properly seeded and mulched within 15 days of
grading activities to minimize onsite erosion.
4. The LCID unit is being covered as required.
5. The wind blown is being managed well.
6. The yard waste is being ground up and removed from the facility.
7. The contingency plan, methane monitoring records, certifications, waste screenings records and a copy of the
financial assurance were reviewed and were in order.
8. Personnel training records were reviewed and are as follows: Randy Smith issued July 30, 2003
David Syck issued December 15, 2003
Linwood Slaughter issued July 30, 2003
9. A copy of the permit and operational plan needs to be at facility available for review.
10. The facility is being managed well. Good Job!
