LOS – PTO Issued, September 23, 2022, and expires September 17, 2064. Area 1 and Area 2, phase 1. PURPOSE OF SITE VISIT: Comprehensive Facility Compliance Inspection STATUS OF PAST NOTED VIOLATIONS: None OBSERVED VIOLATIONS: None FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 8 ADDITIONAL COMMENTS: 1. LOS - PTO / PTC was issued on September 23, 2022, and expires on September 17, 2064. 2. This PTO covers Area 1 and Area 2, phase 1. 3. This permit approves the construction of Area 2, Phases 2 through 4. The initial, substantial, construction authorized by this Permit Approval to Construct must commence within 18 months from the issuance date of the permit issued September 23, 2022. If substantial construction does not begin by this date, then the permit to construct shall expire. No construction of Area 2 has commenced as of the date of this inspection. and the permit approval to construct issued September 23, 2022, is beyond the 18-month window provided to start construction at Area 2. Facility staff indicated that the consultant at Smith Gardner has been contacted to resubmit an updated construction plan, that will reflect Area 2, phases 1-3, and not phase 4 at this time. 4. Edge of waste markers are present. Ensure that all EOW markers are clearly visible. 5. This facility is permitted to receive Construction and Demolition (C&D) debris, Inert debris, Land-clearing waste, and Regulated asbestos-containing material. The day of this inspection a truck had unloaded waste not consistent with C&D that included an old box stereo cabinet, couch, mailbox, and other miscellaneous household items mixed with construction and demolition debris at the working face. Staff at the facility indicated they were able to identify who had dumped the load to get them to return to the landfill. Also, during this inspection, a truck with trash bags full of plastic bottles and other household trash mixed with some construction and demolition debris showed up at the working face and was turned away to the convenience site. It is recommended that a strong presence and screening process continue at both the scale house and the working face to prevent items that are not allowed in the C & D Landfill from being dumped there. 6. This facility is permitted to receive solid waste generated within Wilson County consistent with the resolution passed by the Wilson County Board of Commissioners on June 1, 2020. 7. The facility is approved to accept waste at a rate of maximum 50,000 tons per year. 8. Incoming load receipts at the scale house were reviewed and records showed several loads had been rejected with explanations on why the loads were rejected. 9. Inert Debris Staging area was observed and it appeared operations were in keeping with the permit requirements. 10. Glass Fines are permitted to be used for road material within the landfill footprint. 11. Operational conditions – Scrap Tires collection was observed, and it appeared operations were in keeping with the permit requirements. 12. The Treatment & Processing Wood Grinding area was observed. 13. The facility operation includes a 4.5-acre Wood Processing Area, and a small Type 1 composting operation. The operation is approved to accept land clearing debris, yard waste, and untreated and unpainted wood waste/pallets. Temperature data for compost was available but not reviewed during this inspection. Maximum storage at any given time is 6,000 cubic yards. The maximum stored allowed appears to have been reached. Larger storage amounts will require a permit application for a Large Type 1 facility. Wilson County did apply for a Large Type 1 facility permit, and the permit is pending a final issuance once an appropriate soil is identified and used to amend the areas within the proposed composting windrow footprint, verification by the soil scientist will be needed The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 3 of 8 with a letter addressing that the correct soil was used, and the amount placed meets the rule requirements. A letter from the soil scientist can be provided prior to the on-site pre-operational meeting or provided at the meeting. 14. 9809-LCID-2024, the new LCID Permit to construct and operate was issued April 16, 2024, andexpires April 16, 2029. The LCID Total Gross permitted capacity is 245,500 Cubic yards within the permitted waste footprint and approximately 5.1 acres in size was observed. The LCID area hastree debris that was stored prior to the issuance of the permit. The tree debris should be removed.Please grind and/or sell/remove old tree debris from the facility as soon as possible/prior tooperation of the new LCID. Please contact the Permitting Unit for guidance on methods that maybe available to you to manage this debris and contact me, the Environmental Senior Specialist, for the required pre-operative site visit. 15. Several Groundwater monitoring wells checked and were labelled, tagged, and locked as required.16. Erosion and Sedimentation Control plan (revised October 2021) was available but not reviewed.17.The working face at the CDLF was <1/2 acre.18.Cover material was adequate and readily available on the working face of this C&DLF. Some windblown waste was observed. Remember that ALL WINDBLOWN WASTE MUST BE PICKED BY THEEND OF EACH WORKING DAY AND RETURNED TO THE WORKING FACE.19.Screening records were available, 2023 and 2024 records were reviewed and up to date.20. Area 1 C&DLF has been Closed. Vegetation is present and is being maintained.21.This Facility has Certified Landfill Operations Specialists: Paige Aycock, exp. 10/19/25, MichelleEdwards, exp. 8/17/25, Amber Fedette, exp. 8/17/25, Kenny Matthews, exp. 10/19/25 and Terry McClintock, exp. 5/3/27, and Robert Patterson, exp. 4/11/26.22.Facility has 2 Manager of Landfill Operations, Andrew Davis, exp. 8/6/26 and J. Mark Denton, exp.9/13/25.23.Windrow’s (mulch and active compost) were observed, however amounts of compost, and compost temperature records were not reviewed during this inspection. Wilson County has a pending Type 1Compost Permit, and a separate pre-operative inspection will be scheduled for the new compostpermit.24.Old LCID N0292 – The old LCID landfill is scheduled for closure by December 2024. Ensure that theLCIDLF is closed in accordance with 15A NCAC 13B .0567, 25.Overall, this facility is being managed well.26.Digital pictures were taken during this inspection with some attached to this report. Some windblown trash was observed. Good efforts to clean up the trash the day of the inspection were also observed. Good vegetation and road maintenance was observed in the closed landfill areas.
