1.0 PROJECT INFORMATION
The subject site is located at 187 Deaverview Road in Asheville, North Carolina (Figure 1). MB Haynes Corporation (MB Haynes) operates a multi-discipline construction services company on the property. The facility includes a vehicle fueling operation with one above ground diesel and one above ground gasoline storage tank with a single dispenser pump for each fuel type. The property slopes from Deaverview Road to the north and terminates at a tributary to Smith Mill Creek. The fueling area is located near the top of the slope approximately 150 feet from Deaverview Road (Figure 2).
We understand that on April 30, 2007, a discharge of gasoline into a tributary of Smith Mill Creek was discovered on the subject property. The local fire department conducted an emergency response action by
deploying oil absorbent booms at the point of discharge and further downstream. MB Haynes personnel investigated the incident by excavating a small trench near the discharge point. MB Haynes confirmed the presence of gasoline in the trench’s shallow groundwater. MB Haynes deployed absorbent materials in the
trench as part of an emergency remedial action. Further investigation by MB Haynes revealed that a flexible hose coupling at their gasoline dispenser pump had failed resulting in a release of gasoline. The hose was replaced by a contractor working for MB Haynes and the leak was terminated. On May 1, 2007, MB Haynes contacted the Asheville Regional Office of the North Carolina Department of Environment and Natural Resources (NCDENR) [now the NCDEQ] to report the incident. The NCDENR assigned the Department of Waste Management (DWM) as the regulatory authority for the incident. The DWM conducted a site visit on May 2, 2007 and subsequently issued a Notice of Violation (NOV) letter on May 4, 2007. The NOV assigned an incident number (88075) and required MB Haynes to perform assessment and remedial actions.
The aforementioned NOV requires the performance of a Comprehensive Site Assessment (CSA) followed by a Corrective Action Plan (CAP). MB Haynes retained BLE to perform the CSA and to implement interim remedial actions.
A soil assessment in the release area was conducted in 2007 and 2008. The limits of contaminated soil were defined and a mobile source remediation unit (soil vapor extraction [SVE] system) was deployed in
2008 as an interim remedial action as approved by the DWM. The SVE system concluded operations in 2020 per DWM approval. Seventeen groundwater monitoring wells were installed by BLE from 2007 through 2009 to establish the horizontal limits of the free-product and dissolved phase gasoline. The wells were designated MW-27 through TMW-43. The limits of the gasoline contaminants were defined. Please note that a groundwater monitoring well designated MW-26 was already present on the MB Haynes property and was installed as part of the neighboring Andrex groundwater incident. Groundwater monitoring well MW-26 has been designated as the upgradient well for the MB Haynes groundwater incident. Additionally, BLE conducted surface water sampling in Smith Mill Creek Tributary and in Smith Mill Creek which are the receiving waters for the MB Haynes groundwater discharge. The surface water impacts were also substantially
defined by this assessment. Free-phase (free-product) gasoline was present near the release area (gas pump) and near Smith Mill Creek Tributary. BLE initiated product removal in 2008 utilizing a passive downhole skimmer in several affected wells. Product recovery from the skimmer was performed by MB Haynes. Aggressive fluid vapor recovery (AFVR) was also performed in affected wells by a contractor retained by MB Haynes in 2007 and 2008.

A free-product interceptor trench was designed by BLE and installed by MB Haynes in February 2009 near Smith Mill Creek Tributary. Product recovery from the trench was performed by MB Haynes from November 2009 until December 2020 when the DWM approved termination of remedial actions. Several groundwater and surface water sampling events have been conducted as part of the CSA. As required by the DWM we conducted one site-wide groundwater and surface water sampling event in January 2010 to establish baseline water quality conditions. The results of the 2010 event have been
submitted to the DWM. The DWM required that MB Haynes perform subsequent annual sampling events to monitor groundwater quality and to evaluate effectiveness of the ongoing remedial measures. Annual sampling events have been conducted each winter or early spring from 2010 through September 2022.
On December 10, 2020, Ms. Caroline LaFond of the DWM communicated with MB Haynes and BLE concerning a request for site closure. Ms. LaFond outlined an approach for the site to quality for a NORP.
The approach included termination of remedial actions (SVE and Product Recovery in December 2020) and the performance of four (4) semi-annual water quality sampling events to be conducted in 2021 and 2022. The schedule and sampling plan was documented in a letter dated December 16, 2020 from BLE to the DWM on behalf of MB Haynes. As of January 2021, all remedial actions have been terminated. Sampling was performed in accordance with the DWM approved 2020-2022 Sampling Matrix. BLE performed the fourth of the four required semi-annual sampling events during September 2022. The results indicated that the detected concentration of benzene in MW-28 exceeded the DEQ Gross Contamination Levels for Groundwater (GCL). The report of the September 2022 sampling event was recently issued.
We contacted Ms. LaFond to discuss the findings on November 30, 2022. Ms. LaFond stated that the DWM cannot complete an NORP with GCL exceedances. The DMW suggested that MB Haynes perform a single day 8-hour duration AFVR event in MW-28 to attempt to remove residual product and potentially lower the
benzene concentration to below the GCL. The DWM also specified a post-AFVR sampling frequency for 2023 to demonstrate compliance for the NORP (Table 1).
This report report describes the work performed and presents the results obtained for the single AFVR event and three (3) of the four (4) post-AFVR sampling events in MW-28.

2.0 SCOPE OF SERVICES
2.1 Aggressive Fluid Vapor Recovery (AFVR) On January 16, 2023, BLE mobilized a multiphase extraction unit (MEU), mobile holding tanks, and crew to the site. The MEU and mobile tanks was set up by our crew on an off-site private property north of MB Haynes on the opposite side of the creek. BLE provided the services of an environmental technician to coordinate MEU setup with MB Haynes and to perform the AFVR event.
On January 17, 2023, a vacuum was applied to MW-28 for 8-hours. Off-gas treatment was be provided by BLE for the test period. Monitoring of the system off-gas and liquids recovery was be performed by the crew in accordance with standard practice.
2.2 Groundwater Sampling A groundwater sample was collected from MW-28 in accordance with the approved matrix in February,
May, and July 2023 (Table 1). The groundwater sample for each event was submitted to an environmental laboratory for analysis of volatile organic compounds (VOCs) with IPE and MTBE by EPA Method 8260D.
3.0 RESULTS
3.1 AFVR Results The January 17, 2023 AFVR event recovered 125 gallons of liquids from MW-28 which were disposed at VLS Recovery Services in Mauldin, SC on January 18, 2023. The AFVR field log and disposal manifest is included in Appendix A. 3.2 Groundwater Results The laboratory analytical results from each sampling event are included in the appendices as follows: February
22, 2023 (Appendix B); May 19, 2023 (Appendix C); and July 19, 2023 (Appendix D). The results from September 2022 and from all three 2023 sampling events are summarized on Table 2.
The results indicate that the detected concentration of benzene (and all other VOCs) do not exceed the GCL in MW-28.

4.0 CONCLUSIONS AND FUTURE PLANS
We conclude that the January 2023 AFVR event resulted in reduced concentrations of benzene in MW-28 from that detected September 2022. We also conclude that the detected benzene concentrations for three (3) of the four (4) required post-AFVR sampling events do not exceed the GCL and demonstrate compliance for the future NORP. Sampling and analysis for the fourth of the four (4) required post-AFVR sampling events in MW-28 is
currently underway. If the results indicate that the benzene concentration in MW-28 remains below the GCL a site-wide sampling event will be performed in accordance with the DWM approved plan (Table 1).
A report of findings from the above will be submitted to the DWM as required.
5.0 QUALIFICATION OF REPORT
The activities and evaluative approaches used in this assessment are consistent with those normally employed in environmental assessment projects of this type. Our evaluation of the site conditions was based on the unique project information and data obtained. This report has been prepared on behalf of and exclusively for the use of MB Haynes Corporation, Asheville, North Carolina. This report and the findings contained herein are not intended to be relied upon by any other party without BLE’s prior written consent.
