Dear Mr. Holt,
The NC Division of Waste Management received your notice of cease operation to close the
Septage Detention & Treatment Facility (permit #SDTF 31-08) located on Hwy 117 North in Duplin
County. The effective cease operation date was June 2, 2023 and a final inspection was performed.
As of this date, permit #SDTF 31-08 has been closed out. If in the future you wish to obtain a
permit for a septage facility or land application site, please contact this office to initiate the
permitting process. Use of a septage facility or land application site that is not permitted may result
in administrative penalties up to $15,000 per violation in accordance with NC General Statute 130A-
22.

1 — At time of inspection sign was missing but has been replaced
2 — Tank was empty and accessible for inspection
3 — Tank appears to be structurally sound
4 — Septage transfers appear to be done in a safe and sanitary manner
5 — No vector problems or objectionable odors noted
6 — Setback appear compliant
7 — Close out form was submitted on 06-05-2023. Recommend closure of tank from this inspection
