You are hereby advised that, pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $5,000 per day may be
assessed for each violation of the Solid Waste Statute or Regulations. If the violation(s) noted here continue, you may be
subject to enforcement actions including penalties, injunction from operation of a solid waste management facility cr a solid
waste collection service and any such further relief as may be necessary to achieve compliance with the North (Carolina
Solid Waste Management Act and Rules. _

OTHER CI4TS /SUGGESTIONS:
1) Need to provide new locks for GW monitoring wells
2) Reverse grade on drainage Swale on eastern side of the facility needs to be corrected.
3) Need to apply mulch stored on the back of the facility to bare spots on the landfill and other areas cif the
facility.
4) Overall the facility looks good, has been mailed twice this year.
5) Need to clean trash from old LF area.
6) Need to repair check dam to main sedimentation basin.
