Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 2 ADDITIONAL COMMENTS TRUCK LICENSE PLATE # VIN # TANK CAPACITY (gal)  TZ‐5957 3HAMMMML5EL067006 2,000  The truck was initially inspected on 5/30/2024. During this inspection the town name shown on the  truck did not match the information on the permit application (Four Oaks town name on application).   Photos verifying corrections made to the town name lettering were received on 6/3/2024.  1) All lettering indicating firm name, phone number, town name and septage management firm permit  number was permanently attached to each side of the truck, were plainly visible and met height  requirements.    2) The septage holding tank was constructed of metal and appeared to be structurally sound.  3) The tank was affixed to the truck from with permanent fixtures.  4) The septage drain valve appeared to be in working order. No septage leaks were observed.  5) All port covers and caps where in place and appeared to be in proper working order.      Photos above taken on 5/30/2024 truck lettering shows Smithfield as town name.    Photos above showing Four Oaks as town name.    Please contact me if you have any questions or concerns regarding this inspection report.        ____________________________________________ Phone: (919) 707-8285 Jeffrey Bullard, Environmental Program Consultant Division of Waste Management, NCDEQ Regional Representative Sent on: 6/4/2024 Email X Hand delivery US Mail Certified No. [ _]
