The truck was initially inspected on 5/30/2024. During this inspection the town name shown on the
truck did not match the information on the permit application (Four Oaks town name on application).
Photos verifying corrections made to the town name lettering were received on 6/3/2024.
1) All lettering indicating firm name, phone number, town name and septage management firm permit
number was permanently attached to each side of the truck, were plainly visible and met height
requirements.
2) The septage holding tank was constructed of metal and appeared to be structurally sound.
3) The tank was affixed to the truck from with permanent fixtures.
4) The septage drain valve appeared to be in working order. No septage leaks were observed.
5) All port covers and caps where in place and appeared to be in proper working order.
Photos above taken on 5/30/2024 truck lettering shows Smithfield as town name.
