CES PROJECT NO.EM235912S3 - CMK183 TRUCK NO.877017 Pursuant to North Carolina state regulations,petroleum related releases in excess of 25 gallons are considered reportable.This release was estimated to be approximately 40 gallons;therefore,regulatory notification was required. On November 9,2023,at the time of the incident,on-site fire department personnel responding to the scene deployed granular absorbents to contain the release until crews arrived.The remaining diesel fuel from the saddle tanks was pumped into one (1)55-gallon drum for disposal.At approximately 11:39 AM CST,a representative from US Foods retained Cura Emergency Services,L.C.(CES)to manage the environmental remediation of the site on their behalf.Based on the available information,the CES incident manager dispatched a crew from First Call Environmental, LLC (FCE) to assess and remediate the site as necessary. Dec 19, 2023 Please accept this document as the final report detailing the emergency response and remedial actions taken for the diesel fuel (generic) release that occurred on 11/9/2023, at the above referenced location. On November 9,2023,at approximately 12:51 PM CST,a crew from FCE arrived on-site.Following a site assessment,FCE personnel noted approximately 40 gallons of diesel fuel released to the asphalt and soil REMEDIAL ACTIONS: On November 9,2023,at approximately 12:11 PM CST,the CES incident manager verbally reported the release to the North Carolina DENR Region 7 on behalf of US Foods.Incident number 20231109/5 was assigned to the release. To Whom it May Concern: Mooresville, NC, 28115- Cura Emergency Services, L.C. © 1998Dec 19, 2023 shoulder.Crews deployed additional granular absorbents to the impacted area and worked them in with hand tools.Hand tools were utilized to surface scrape the impacted soil to a depth of four (4)inches until no visual or olfactory evidence of material remained.FCE personnel took possession of the previously generated 55-gallon drum for disposal purposes.All diesel fuel impacted materials and spent absorbents were collected and containerized into two (2)additional 55-gallon drums for transport and disposal.FCE personnel noted no further actions required on-site and crews demobilized. WASTE DISPOSAL: On December 8,2023,two (2)55-gallon drums containing 250 pounds of Non DOT/Non RCRA Regulated Material diesel impacted soil +sorbents were transported to,and disposed of at,Terra Nova in Thomasville, NC (see attached disposal documents). On December 8,2023,one (1)55-gallon drum containing 40 gallons of Non DOT/Non RCRA Regulated Material diesel was transported to,and disposed of at,Terra Nova in Thomasville,NC (see attached disposal documents). CONCLUSION AND RECOMMENDATIONS: Respectfully, Following the corrective actions,no visual or olfactory evidence of diesel fuel could be observed at the site. Based on these results,it appears the corrective actions were successful,and no further action should be required. CES recommends this incident be closed. Cura Emergency Services, L.C.
