In accordance with the requirements of a correspondence from the North Carolina Department of Environmental Quality (NCDEQ), contained herein is a Limited Site Assessment for the release which occurred at the above referenced facility. These activities have been conducted following the release of petroleum which occurred in the vicinity of one 550 gallon heating oil underground storage tank (UST). LIMITED SITE ASSESSMENT REPORT Former Wachsmann Property 3980 Old Beatty Ford Road China Grove, North Carolina 1.0. - SITE HISTORY AND SOURCE CHARACTERIZATION
Cassia Wachsmann formerly owned a property located at 3980 Old Beatty Ford Road in China Grove, NC. This property contains one permanent structure which is used as a residence. Figure 1 illustrates the location of this facility on the China Grove Quadrangle U.S.G.S. Topographic Map. The site formerly contained one 550 gallon heating oil UST (identified as Tank #1) which was used for heating the residence and one 550 gallon diesel UST (identified as Tank #2) which was believed to be used for fueling farm equipment. Figure 2 illustrates the site layout and the location of the former USTs. Information regarding the ownership of the non-regulated USTs which were formerly located at this facility is contained in Table 1. A release from both 550 gallon tanks was confirmed upon receipt of analytical results for soil samples collected during an inspection of the USTs by a separate consultant for potential buyers of the property, prior to the removal activities. A soil sample collected from the vicinity of the heating oil UST was reported at a level of 11,992 milligrams per kilogram (mg/kg) by Diesel Range Organics (DRO). A soil sample collected from the vicinity of the diesel UST was reported at a level of 13,097 mg/kg. An Initial Abatement Action Report for this facility dated November 16, 2023 has been submitted to the NCDEQ. The initial abatement activities were successful at removing all of the contaminated soil in the vicinity of the former diesel UST (Tank #2), but petroleum impacted soil remains in the vicinity of the former heating oil UST (Tank #1).

3.0 - RECEPTOR INFORMATION
3.1 Water Supply Wells
A supply well survey has been conducted within a radius of 1,000 feet from the release
area. During this reconnaissance, twenty-seven water supply wells which are in use for
consumption were found to be located within this radius. Figure 3 illustrates the locations
of the supply wells on a receptor well map, and Table 2 summarizes the well owners’ names
and addresses and all known well construction information.
3.2 Public Water Supplies
There are no public water supplies available to any properties within a radius of 1,000 feet from 3980 Old Beatty Ford Road.
3.3 Surface Water
The partial U.S.G.S. map included as Figure 1 indicates that surface waters in the vicinity
of the release area generally drain towards Lake Fisher located approximately 9,700 feet
to the southwest. Lake Fisher drains into Cold Water Creek, which flows into Irish Buffalo
Creek approximately 69,000 feet to the South. Irish Buffalo Creek is a tributary of the
Rocky River, which lies within the Yadkin Pee-Dee River Drainage Basin.

3.4 Wellhead Protection Areas
No wellhead protection areas are known to exist within the area of this release. 3.5 Deep Aquifers in the Coastal Plain Physiographic Region
This release is not located in the coastal plain.
3.6 Subsurface Structures
The house located on the impacted property has a basement. Subsurface utilities are present in the form of the water line from the supply well and the septic system. Figure 4 illustrates the locations of all known subsurface utilities.
3.7 Land Use
The possibility of human exposure to soil contamination at the Former Wachsmann
Property is minimal. The impacted soil which remains in place is present at a depth of
more than 11 feet below surface and is covered with clean backfill above this depth. The
facility lies within a primarily residential area.
3.8 Property Owners and Occupants
Figure 5 illustrates the surrounding properties, and Table 3 contains information regarding the adjacent property owners. This information was obtained from the Rowan County Tax Department’s records.
4.0 - SITE GEOLOGY AND HYDROGEOLOGY
4.1 Site Geology
The site is situated in the Piedmont Region of the North Carolina Slate Belt. According to the Geological Map of North Carolina local bedrock geology of the region consists of Late Proterozoic to Cambrian aged metamorphosed granitic bedrock. Competent bedrock was not encountered at a depth of 34 feet below land surface during the installation of the groundwater monitoring well.
4.2 Soils Investigation
The soils at the project site consist of clay with varying amounts of silt. A soil boring log
for the boring advanced for monitor well installation at the site is contained as Appendix
A. One soil sample was collected by a separate consultant during an inspection conducted
prior to the tank removal activities which indicated Total Petroleum Hydrocarbon (TPH)
contamination at a level of 11,992 mg/kg by DRO beneath the former heating oil UST.

On November 8, 2023, following the excavation of approximately 11.5 cubic yards of contaminated soils, Paragon collected samples from in-situ soils surrounding the former UST location. The soil samples were submitted to Waypoint Analytical, LLC for analyses by Gasoline Range Organics (GRO) and Diesel Range Organics (DRO) for TPH. According to the laboratory analytical results for the soil samples collected from the final limits of the heating oil UST remedial excavation, soil sample PB-1 was reported at a concentration of 45,800 mg/kg by DRO, which exceeds the DRO action level of 100 mg/kg. Soil sample EW-1 was listed at a level of 8,680 mg/kg by DRO, which is in excess of the DRO action level of 100 mg/kg. Soil sample NW-1 was shown at a concentration of 7,600 mg/kg by DRO, which is over the DRO action level of 100 mg/kg. NW-1 was reported at a concentration of 74.9 mg/kg by GRO, which exceeds the GRO action level of 50 mg/kg. The remaining two in-situ samples from the heating oil remedial excavation were below the NCDEQ action levels for TPH by GRO and DRO. Two additional “Risk-Based” samples, labeled as MW1-13 and MW1-23, were collected for laboratory analyses from depths of 13 feet and 23 feet below surface grade during the installation of the monitor well. These samples were analyzed by EPA Methods 8260 and 8270 as well as by MADEP methods for Volatile Petroleum Hydrocarbons (VPH) and Extractable Petroleum Hydrocarbons (EPH). According to the laboratory analytical results, 1-Methylnaphthalene, 2-Methylnaphthalene, C9-C18 Aliphatics, and C9-C22 Aromatics were reported at concentrations in excess of the Residential Standards in soil sample MW1-13. The laboratory analytical results for soil sample MW1-23 reported C9-C22 Aromatics at a concentration above the Residential Standards. Figure 6 illustrates the locations of the “Risk-Based” soil samples obtained at 3980 Old Beatty Ford Road, and Table 4 summarizes the soil analytical results for the subject property. A copy of the laboratory analytical report and the chain-of-custody record for the “Risk-Based” soil sample collected from the monitor well boring is contained as Appendix B. 5.0 - SAMPLING
5.1 Monitor Well Installation
One North Carolina Type II groundwater monitoring well has been installed at the site. Figure 7 illustrates the site layout and the location of the monitor well, labeled as MW-1. The monitoring well was constructed of 2-inch Schedule 40 PVC pipe with 20 feet of 0.010 inch slotted screen. Based on the assumption that the contaminants being addressed were primarily hydrocarbon constituents with specific gravities of less than 1.0, the groundwater monitoring well was installed so that the screened interval intersected the shallow groundwater table at the time of installation. Table 5 summarizes the monitoring well information and groundwater elevation as measured on November 20, 2023. 5.2 Groundwater Analyses
Following installation, the monitoring well was developed and sampled in accordance with Paragon’s Standard Operating Procedures which are contained as Appendix B. The groundwater sample was submitted to Waypoint Analytical, LLC for laboratory analyses according to EPA Methods 602 and 625 plus 10 largest Tentatively Identified Compounds (TICs). The groundwater sample from the monitor well was also analyzed by MADEP methods for VPH and EPH.

The analytical results for monitor well MW-1 reported Naphthalene, 1-Methylnaphthalene, 2-Methylnaphthalene, and C9-C22 Aromatics at concentrations in excess of the 2L Standards. The groundwater results were below the Gross Contaminant Levels (GCLs) for all compounds by EPA Methods 602 and 625. Table 6 summarizes the groundwater analytical results, and Appendix C contains a copy of the laboratory analytical report and the chain-of-custody record for the groundwater sample.
6.0 - CONCLUSIONS AND RECOMMENDATIONS
6.1 General Summary
Limited Site Assessment activities at the Former Wachsmann Property have been completed. From a review of all information gathered during this project, Paragon Environmental Consultants, Inc. makes the following conclusions:
o A petroleum release of unknown quantity has occurred at this site. All accessible
contaminated soils were removed by D.H. Griffin Wrecking Company, Inc. on
November 8, 2023.
o One shallow groundwater monitoring well was constructed at the site during this
investigation. Free product was not observed in monitor well MW-1.
o The analytical results for a groundwater sample from the monitor well indicated
Naphthalene, 1-Methylnaphthalene, 2-Methylnaphthalene, and C9-C22 Aromatics
in excess of the 2L Standards, but below the GCLs.
6.2 Recommendations
Based upon a review of all information gathered during this project, Paragon makes the following recommendations:
o Since no further soil excavation is possible and the groundwater is above the 2L Standards, abandoning the single supply well within 150 feet of the release should be considered. This would make the site Low Risk and make the site eligible for a notice of No Further Action after deed recordation and public notification.

6.3 Limitations
This report has been prepared for the exclusive use of HS&E, Inc. for the specific application to the referenced site located in Rowan County, North Carolina. The evaluation was conducted based on the scope of work and level of effort desired by the client and with resources adequate only for the scope of work. Our findings have been developed in accordance with generally accepted standards for Limited Site Assessments in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data presented in this report are indicative of conditions at the precise locations sampled and the time the sample was collected. Additionally, the data obtained from the samples would be interpreted as meaningful with respect to the parameters in the laboratory reports. No additional information can be logically inferred from this data.

PARAGON ENVIRONMENTAL CONSULTANTS, INC.
I. SOIL SAMPLE PROCEDURES
1. Collect all samples using disposable Latex gloves. Gloves are not to be re- used. 2. Place samples into laboratory supplied glassware following requirements for specific analysis. 3. Label samples with sample ID, date, time, and job number. Immediately place samples on ice or in refrigerator to be cooled to approximately 4 degrees Celsius. 4. Store all samples on ice or refrigerate until samples are delivered to the laboratory. 5. Complete a chain of custody record for samples to be submitted to laboratory. Sign and date the chain of custody when samples are relinquished in accordance with EPA chain of custody protocol. II. GROUNDWATER SAMPLING 1. Use new disposable bailer and new nylon string to develop well and collect sample. Handle bailer and string with Latex gloves. 2. Develop well by removing 3 well volumes of water. Dispose of water in accordance with NCDEQ guidelines. 3. Following well development obtain samples in laboratory supplied glassware following requirements for specific analysis. 4. Handle, store, and transport samples in same manner as for soil samples. See items I.3, I.4, and I.5 above. III. EQUIPMENT CONTAMINATION 1. Decontaminate augers, split spoons, and other sampling equipment by the following procedure: A. Soap and tap water wash B. Tap water rinse C. Distilled deionized water rinse D. Isopropyl alcohol rinse E. Distilled water rinse 2. Use new disposable sampling equipment whenever practical.
