These activities have been conducted following the release of petroleum which occurred in the vicinity of one 550 gallon heating oil underground storage tank (UST) and one 550 gallon diesel UST. The soil remediation consisted of the excavation and disposal of approximately 24 cubic yards of contaminated soil and confirmatory sampling of in-situ soils. All activities were conducted in accordance with North Carolina Department of Environmental Quality (NCDEQ) guidelines and the requirements of 15A NCAC 2N. Site History 1. Introduction Cassia Wachsmann formerly owned a property located at 3980 Old Beatty Ford Road in China Grove, NC. This property contains one permanent structure which is used as a residence. Figure 1 illustrates the location of this facility on the China Grove Quadrangle U.S.G.S. Topographic Map. The site formerly contained one 550 gallon heating oil UST (identified as Tank #1) which was used for heating the residence and one 550 gallon diesel UST (identified as Tank #2) which was believed to be used for fueling farm equipment. Figure 2 illustrates the site layout and the location of the former USTs. Information regarding the ownership of the non-regulated USTs which were formerly located at this facility is contained in Table 1. A release from both 550 gallon tanks was confirmed upon receipt of analytical results for soil samples collected during an inspection of the USTs by a separate consultant for potential buyers of the property, prior to the removal activities. A soil sample collected from the vicinity of the heating oil UST was reported at a level of 11,992 milligrams per kilogram (mg/kg) by Diesel Range Organics (DRO). A soil sample collected from the vicinity of the diesel UST was reported at a level of 13,097 mg/kg. Following discovery of the tank release, Paragon Environmental Consultants, Inc. was contracted by HS&E, Inc. to perform confirmatory soil sampling and laboratory analyses after soil remediation activities in the areas of the contaminated 550 gallon heating oil UST and the contaminated 550 gallon diesel UST. Contained herein is a summary of activities which were completed at the project site. 2. Scope of Services This Initial Abatement Action Report contains documentation concerning the following activities which have been conducted at this facility: o Excavation of contaminated soils and backfilling of the excavation performed by D.H. Griffin Wrecking Company, Inc. of Greensboro, NC o Transportation of contaminated soil by D.H. Griffin Wrecking Company, Inc. and disposal of the excavated material at Carolina Composting of Wadesboro, NC o Collection of soil samples from the limits of the excavations performed by Paragon Environmental Consultants, Inc. o Laboratory analyses of soil samples conducted by Waypoint Analytical, LLC of Charlotte, NC Page - 2 C. Site Investigation 1. Excavation and Soil Sampling On November 8, 2023 personnel from D.H. Griffin Wrecking Company, Inc. (D.H. Griffin) and Paragon Environmental Consultants, Inc. (Paragon) removed the 550 gallon heating oil tank and the 550 gallon diesel tank. On November 8, 2023 personnel from D.H. Griffin and Paragon attempted to remove the petroleum impacted soil identified during the UST closure activities. Appendix A contains a tank disposal manifest for the USTs which were formerly located at the subject property. The impacted soils in the excavation areas were excavated until field analyses indicated that all petroleum contaminated soil had been removed, the foundation of the house structure was encountered (in the heating oil remedial excavation), or the vertical limit of the excavation equipment was reached. In order to verify site conditions in the vicinity of the heating oil tank remedial excavation, one sample was collected from the pit bottom at a depth of 13 feet below land surface and one sample was collected from each of the walls at depths of 11 feet below grade. In order to verify site conditions in the vicinity of the diesel tank remedial excavation, one sample was collected from the pit bottom at a depth of 13 feet below land surface and one sample was collected from each of the walls at depths of 11 feet below grade. Appendix B contains a geologic log of excavation for the soil remediation activities at this facility. The soil samples collected from the remedial excavations at 3980 Old Beatty Ford Road were analyzed according to Gasoline Range Organics (GRO) and Diesel Range Organics (DRO) for TPH. The in-situ soil samples from the bottoms and the walls of the excavations were collected with the excavator bucket and were immediately placed into laboratory supplied glassware and placed on ice for transportation to the analytical laboratory utilizing EPA approved chain-of-custody procedures. According to the laboratory analytical results for the soil samples collected from the final limits of the heating oil UST remedial excavation, soil sample PB-1 was reported at a concentration of 45,800 mg/kg by DRO, which exceeds the DRO action level of 100 mg/kg. Soil sample EW-1 was listed at a level of 8,680 mg/kg by DRO, which is in excess of the DRO action level of 100 mg/kg. Soil sample NW-1 was shown at a concentration of 7,600 mg/kg by DRO, which is over the DRO action level of 100 mg/kg. NW-1 was reported at a concentration of 74.9 mg/kg by GRO, which exceeds the GRO action level of 50 mg/kg. The remaining two in-situ samples from the heating oil remedial excavation were below the NCDEQ action levels for TPH by GRO and DRO. According to the laboratory analytical results for the soil samples collected from the final limits of the diesel UST over-excavation, the five in-situ samples were below the laboratory detection limits for TPH by GRO and DRO. Based on the soil sample results from the limits of the diesel UST remedial excavation, no petroleum contamination above the NCDEQ action levels remains in place in the vicinity of the former diesel UST. Figure 3 illustrates the areas of soil remediation and the locations of the in-situ soil samples collected at this facility. The analytical results for the TPH soil analyses are summarized in Table 2. A copy of the laboratory analytical report and the chain-of-custody record for the samples from this subject site is included in Appendix C. All samples were obtained in accordance with Paragon’s Standard Operating Procedures which are included as Appendix D. Page - 3 2. Soil Disposal Approximately 11.5 cubic yards (16.07 tons) of heating oil impacted soils were removed from the heating oil UST remedial excavation. Approximately 12.5 cubic yards (17.59 tons) of diesel impacted soils were removed from the diesel UST remedial excavation. A total of approximately 24 cubic yards (33.66 tons) of petroleum impacted soils were removed during site remediation activities on November 8, 2023. This material was transported by D.H. Griffin to Carolina Composting’s facility in Wadesboro, NC for treatment and disposal. Copies of the soil disposal manifests are contained in Appendix E. 3. Conclusions and Recommendations 3. A Conclusions and Recommendations The soil remediation activities at the Former Wachsmann Property have been completed. From a review of all information gathered during this remediation project, Paragon Environmental Consultants, Inc. makes the following conclusions: o Laboratory results for sampling of in-situ soils following the over-excavation activities in the vicinity of the diesel UST showed all five soil samples were below the NCDEQ action levels for TPH by GRO and DRO. o Approximately 16.07 tons of heating oil impacted soils were removed from beneath the former heating oil UST. Approximately 17.59 tons of diesel impacted soils were removed from beneath the former diesel UST. This material was transported by D.H. Griffin to Carolina Composting in Wadesboro, NC for disposal. 3. B. Recommendations Based upon a review of all information gathered during this Initial Abatement project, Paragon makes the following recommendations: o A Limited Site Assessment (LSA) should be conducted at this facility to detail site specific information and allow the site to be given a priority ranking and a land use classification by the NCDEQ. o A copy of this report should be forwarded to the following address: Mooresville Regional Office - UST Section 610 E. Limitations This report has been prepared for the exclusive use of HS&E, Inc. for the specific application to the referenced site located in Rowan County, North Carolina. The assessment was conducted based on the scope of work and level of effort desired by the client. Our findings have been developed in accordance with generally accepted standards in the practice of Initial Abatement Action Reports in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. Additionally, the data obtained from the samples would be interpreted as meaningful with respect to the parameters indicated in the laboratory reports. No additional information can be logically inferred from this data. Collect all samples using disposable Latex gloves. Gloves are not to be re- used. 2. Place samples into laboratory supplied glassware following requirements for specific analysis. 3. Label samples with sample ID, date, time, and job number. Immediately place samples on ice or in refrigerator to be cooled to approximately 4 degrees Celsius. 4. Store all samples on ice or refrigerate until samples are delivered to the laboratory. 5. Complete a chain of custody record for samples to be submitted to laboratory. Sign and date the chain of custody when samples are relinquished in accordance with EPA chain of custody protocol. II. GROUNDWATER SAMPLING 1. Use new disposable bailer and new nylon string to develop well and collect sample. Handle bailer and string with Latex gloves. 2. Develop well by removing 3 well volumes of water. Dispose of water in accordance with NCDEQ guidelines. 3. Following well development obtain samples in laboratory supplied glassware following requirements for specific analysis. 4.
