Figure 1 illustrates the location of this facility on a partial Statesville West Quadrangle U.S.G.S. Topographic Map. One 8,000 gallon gasoline underground storage tank (UST) and two 6,000 gallon gasoline USTs were assessed during this site check. One 8,000 gallon diesel UST, one 1,000 gallon kerosene UST, and one 1,000 gallon off-road diesel UST are also currently operated at this facility. On May 23, 2023 field activities necessary to comply with the requirements of a site check were performed at this facility to discover the release point(s) of the existing UST system. As directed by the Mooresville Regional Office, only the gasoline portion of the tank system was assessed during the site check activities. The site layout and the existing UST locations are illustrated on Figure 2. 1.2 Scope of Services This Site Check Report contains documentation concerning the following activities which were conducted at this facility: o Fifteen Geoprobe soil samples obtained around the USTs, piping, and dispensers spill buckets and the installation of one Type II monitor well by AmeriProbe, LLC of Concord, NC o Supervision of probing/drilling activities and field sampling of in-situ soils and groundwater by Paragon o Laboratory analyses of soil samples conducted by Waypoint Analytical, LLC of Charlotte, NC 2.0 Soil Sampling and Analyses 2.1 Soil Borings and Soil Sampling On May 23, 2023, Paragon and AmeriProbe, LLC performed assessment sampling in the area of the existing tank system. AmeriProbe conducted six pushprobe samples around the existing tanks which were labeled as GeoProbe #1 (GP-1) through GeoProbe #6 (GP-6) at depths of 15 feet below land surface. AmeriProbe also conducted pushprobe samples adjacent to each of the product piping trenches and each dispenser, labeled as GeoProbe #7 (GP-7) through GeoProbe #15 (GP-15), at depths of 5 feet below land surface. The samples were conducted using a Geoprobe hydraulic pushprobe sampling rig. Page - 2 The soil samples which were selected for laboratory analyses were placed into laboratory supplied glassware. The soil samples were labeled with sample location, analyses to be performed, time, date, and the sampler's name. They were then immediately placed in a cooler and chilled with ice to approximately 4oC in preparation for transportation to the analytical laboratory, utilizing EPA approved chain-of-custody procedures. The locations of each of the soil samples are illustrated on Figure 2. A soil boring log for the activities conducted at the site is contained as Appendix A. Following collection of the samples, the open boreholes were backfilled with the sample cuttings and bentonite and then the holes were patched with concrete at surface grade. 2.2 Laboratory Analyses of Soil Samples The soil samples collected from the areas of the gasoline USTs were submitted to Waypoint Analytical, LLC for analyses according to Gasoline Range Organics (GRO). GRO tests for Total Petroleum Hydrocarbons (TPH) from low boiling-point fuels such as gasoline, aviation fuel, and gasohol. The current action level for GRO contaminants is 50 milligrams per kilogram (mg/kg). According to the laboratory analytical results, soil sample GeoProbe #6 (GP-6) was reported with a concentration of 3,360 mg/kg by GRO. This soil sample was collected from the northeast end of the premium gasoline UST. All of the remaining soil samples collected from the vicinity of the gasoline USTs, product piping, and dispensers were below the current action levels by GRO. Table 1 summarizes the soil analytical results of the samples collected by Paragon on May 23, 2023. Appendix B contains a copy of the laboratory analytical report and the chain of custody record for the soil samples collected by Paragon on May 23, 2023. 2.3 Monitor Well Installation One North Carolina Type II groundwater monitoring well has been installed at the site. Figure 2 illustrates the site layout and the location of the monitor well, labeled as MW-1. The monitoring well was constructed of 2-inch Schedule 40 PVC pipe with 15 feet of 0.010 inch slotted screen. Based on the assumption that the contaminants being addressed were primarily hydrocarbon constituents with specific gravities of less than 1.0, the groundwater monitoring well was installed so that the screened intervals intersected the shallow groundwater table at the time of installation. Table 2 summarizes the monitoring well information and groundwater elevation as measured on May 24, 2023. Appendix C contains a copy of the monitor well construction record. 2.4 Groundwater Analyses Following installation, the monitoring well was developed and sampled in accordance with Paragon’s Standard Operating Procedures which are contained as Appendix D. The groundwater samples were submitted to Waypoint Analytical for laboratory analyses according to EPA Method 6200B plus MTBE, IPE, EDB, Ethanol, and 5 Oxygenates as well as for Lead. The samples collected from this monitor well was also analyzed by EPA Method 504.1 for EDB and MADEP methods for VPH. Page - 3 According to the analytical results for monitor well MW-1, Benzene, tert-Butyl Alcohol (TBA), MTBE, and EDB were detected at concentrations in excess of the 2L Standards. Benzene was reported at a concentration of 2.21 ug/L, which slightly exceeds the 2L Standard of 1 ug/L. TBA, which has a 2L Standard of 10 ug/L, was reported at a concentration of 107 ug/L. MTBE was listed at a level of 134 ug/L, which is over the 2L Standard of 20 ug/L. EDB was shown at a concentration of 0.239 ug/L, in excess of the 2L Standard of 0.02 ug/L. Xylenes and two carbon fraction classes by VPH were also reported at detectable levels but were below the 2L Standards. MW-1 was also below the 2L Standard for Lead. Table 3 summarizes the groundwater analytical results, and Appendix E contains a copy of the laboratory analytical report and the chain-of-custody record for the groundwater sample. 3.0 Conclusions 3.1 General Summary Based on a review of all information gathered during this Site Check, Paragon makes the following conclusions: o One soil sample collected from the vicinity of the premium gasoline UST was detected at a concentration over the current action level for GRO. The remaining fourteen soil samples were all below the current action levels according to GRO. o The groundwater sample collected from MW-1 reported Benzene, TBA, MTBE, and EDB at concentrations in excess of the 2L Standards. 3.2 Recommendations Based on the findings from our investigation which indicate TPH concentrations in excess of the current action levels and groundwater levels in excess of the 2L Standards, Paragon recommends that the NC DEQ – UST Section – Corrective Action Branch make a determination on any further actions which will be required at Yogi Foods. 3.3 Limitations This report has been prepared for the exclusive use of Bhavani Statesville, LLC for the specific application to the referenced site located in Iredell County, North Carolina. The assessment was conducted based on the scope of work and level of effort desired by the client. Our findings have been developed in accordance with generally accepted standards in the practice of Site Check Reports in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. The data presented in this report are indicative of conditions that existed at the precise locations sampled and at the time the samples were collected. Additionally, the data obtained from the samples would be interpreted as meaningful with respect to the parameters indicated in the laboratory reports. No additional information can be logically inferred from this data. I. SOIL SAMPLE PROCEDURES 1. Collect all samples using disposable Latex gloves. Gloves are not to be re- used. 2. Place samples into laboratory supplied glassware following requirements for specific analysis. 3. Label samples with sample ID, date, time, and job number. Immediately place samples on ice or in refrigerator to be cooled to approximately 4 degrees Celsius. 4. Store all samples on ice or refrigerate until samples are delivered to the laboratory. 5. Complete a chain of custody record for samples to be submitted to laboratory. Sign and date the chain of custody when samples are relinquished in accordance with EPA chain of custody protocol. II. GROUNDWATER SAMPLING 1. Use new disposable bailer and new nylon string to develop well and collect sample. Handle bailer and string with Latex gloves. 2. Develop well by removing 3 well volumes of water. Dispose of water in accordance with NCDEQ guidelines. 3.
