The site consists of a convenience store which stores petroleum products in underground storage tanks (USTs) for retail sale. Three USTs are located at the site. Recently, a release was identified during equipment upgrade activities. An excerpt from the report prepared by Progress is below: Progress Environmental Group, Inc. (Progress) is pleased to submit this Site Check Report for the Taylorsville Quality Plus in Taylorsville, Alexander County, North Carolina. The objective of the assessment was to collect and test a groundwater sample in the vicinity of the three underground storage tanks (USTs) spill buckets where evidence of leaks was detected during a previous inspection. Utilizing a track-mounted drill rig, 3D Environmental Investigations, LLC installed a Type II monitoring well (MW-1) immediately adjacent to the UST pit on January 11, 2021. Progress supervised the installation of the monitoring well. Evidence of the water table was observed in the soil samples and drill cuttings at an approximate depth of 30 feet below land surface (bls). 3D Environmental Investigations, LLC installed a two-inch monitoring well to an approximate depth of 40 feet bls with 15 feet of 0.01 slotted Schedule 40 PVC screen using the solid stem auger drilling method. Progress allowed the monitoring well adequate time to equilibrate prior to purging and sampling. A groundwater sample was collected using a peristaltic pump and dedicated, disposable tubing and transferred to laboratory containers. The groundwater samples were decanted into laboratory-supplied bottles and placed into a cooler packed with ice. The cooler was delivered under chain-of-custody to Pace Analytical Services, LLC (Pace) in Kernersville, North Carolina for analysis for volatile organic compounds (VOCs) using Standard Method 6200B, volatile petroleum hydrocarbons (VPH) using the Massachusetts Department of Environmental Protection (MADEP) Method, for ethylene dibromide (EDB) using EPA Method 504.1, and for lead using the Standard Method. A summary of the groundwater sample analytical results is included in Table B-4. Laboratory analysis of the groundwater sample collected from monitoring well MW-1 detected tert amyl alcohol (1,240 ug/L), tert butyl alcohol (1,040 ug/L), benzene (143 ug/L), methyl tert- butyl ether (20.9 ug/L), naphthalene (16.7 ug/L), C5-C8 Aliphatics (1,270 ug/L), and C9-10 Aromatics (407 ug/L) at concentrations exceeding their respective North Carolina 2L Groundwater Quality Standards (NC2LGWQS). None of the targeted compounds were detected in the groundwater sample from monitoring well MW-1 exceeds their respective Gross Contamination Levels (GCLs). Limited Site Assessment On May 10, 2022, 3D Environmental Investigations mobilized a track-mounted drill rig to install a soil boring in the vicinity of the previously installed monitoring well MW-1. Progress collected four soil samples for laboratory analysis from a depth of approximately ten feet below land surface (bls), 15 feet bls, 20 feet bls, and from a depth of 23 feet bls during the advancement of the soil boring using the continuous sampling method. Please note, GeoprobeTM refusal was encountered at a depth of 23 feet bls. The soil samples were analyzed for volatile organic compounds (VOCs) using EPA Method 8260 and/or for volatile petroleum hydrocarbons (VPH) using the Massachusetts Department of Environmental Protection (MADEP) Method. Laboratory analysis of the soil samples collected from monitoring well SB-1 @ 10’ detected benzene (8.0 mg/Kg), naphthalene (26.0 mg/Kg) 1,2,4-trimethylbenzene (26.7 mg/Kg), 1,3,5-trimethylbenzene (22.2 mg/Kg), and total xylenes (12.8 mg/Kg) at concentrations that exceed their respective Soil-to-Groundwater Maximum Soil Contaminant Concentrations (MSCCs). Please note, the concentration detected of naphthalene exceeds its Residential Cleanup Level. Laboratory analysis of the soil samples collected from monitoring well SB-1 @ 15’ detected C9-C10 Aromatics (143 mg/Kg) at a concentration that exceeds its Soil-to-Groundwater MSCC. Laboratory analysis of the soil sample collected from monitoring well SB-1 @ 20’ did not detect targeted compounds at concentrations exceeding the laboratory detection limits. Laboratory analysis of the soil samples collected from monitoring well SB-1 @ 23’ detected naphthalene (95.3 mg/Kg) 1,2,4-trimethylbenzene (235 mg/Kg), and total xylenes (41.0 mg/Kg) at concentrations that exceed their respective Soil-to-Groundwater MSCCs. Please note, the concentration detected of naphthalene exceeds its Industrial/Commercial Cleanup Level and the concentration detected of 1,2,4- trimethylbenzene exceeds its Residential Cleanup Level. Progress also collected a groundwater sample from the monitoring well as part of the Phase II Limited Site Assessment activities. The groundwater sample was collected using a disposable bailer and new length of cord and transferred to laboratory containers. The groundwater sample collected from monitoring well MW- 1 was analyzed for VOCs using EPA Method 6200B, for VPH using the MADEP Method, for ethylene dibromide (EDB) using EPA Method 504.1, and for lead using Method 200.7. Laboratory analysis of the groundwater sample collected from monitoring well MW-1 detected tert amyl alcohol (467 µg/L), tert butyl alcohol (800 µg/L), benzene (48.6 µg/L), naphthalene (63.3 µg/L), C5-C8 Aliphatics (751 µg/L), and C9-C10 Aromatics (496 µg/L) at concentrations exceeding their respective North Carolina 2L Groundwater Quality Standards (NC2LGWQS). None of the compounds were detected at concentrations exceeding the applicable Gross Contamination Levels (GCLs). 3. Other Site Activities Progress has not conducted additional activities at the subject site. C RECEPTOR INFORMATION Comprehensive Site Assessment October 18, 2023 Taylorsville Quality Plus Site Progress Project No. QUA420N.GW 1115 NC Highway 90 Taylorsville, Alexander County, North Carolina 5 1. Water-Supply Wells Progress previously mailed 70 water-supply well questionnaires to the property owners within a 1,000- foot radius of the site. 17 of the questionnaires have been completed and returned at the time of this report. Progress also contacted the Energy United Water Corporation to confirm that the properties within 1,000 feet are connected to the public water system. Progress also completed a visual reconnaissance within 1,500 feet of the site. Progress identified evidence of one active water-supply well used for drinking water within 1,000 feet of the site. One additional water-supply well that is not currently providing drinking water was identified slightly greater than 1,000 feet of the site. The active water-supply well was identified at 166 Sunny Lane, located approximately 485 feet north-northeast from the source area. The water-supply well provides drinking water to the following addresses: 166 Sunny Lane; 177 Sunny Lane; 196 Sunny Lane; 200 Sunny Lane; and, 210 Sunny Lane. Please note, according to the Energy United Water Corporation, water service is not provided to following occupied properties within 1,000 feet of the source area: 74 Bandit Street, 76 Bandit Street, 76 Justice Court, and 980 NC 90 Highway. The property owner of 74 Bandit Street indicated the property does not have a water-supply well and drinking water is provided by “Havacup Quality Water Dispensers”. It is possible that the remaining properties are served drinking water via a private water-supply well. Progress understands that public water is available in the vicinity of the site and that the properties along Sunny Lane that are currently using a water-supply well for drinking have access to the public water system. 2. Public Water Supplies Are public water supplies available within 1,500 feet of the source are of the release? YES If yes, where is the location of the nearest public water line and the source(s) of the public water supply? Public water is available along NC Highway 90 and along secondary roads in the vicinity of the site. 3. Surface Water According to the USGS topographic map, an unnamed pond which feeds an unnamed tributary to Glade Creek is located approximately 1,240 feet to the southwest of the source area. 4. Wellhead Protection Areas The source area is not located within an established wellhead protection area according to the North Carolina Well Head Protection Areas Map (https://www.arcgis.com/home/webmap/viewer.html?webmap=7492e0fe11874ccca699679d03bc0751). 5. Deep Aquifers in the Coastal Plain Physiographic Region The subject site is located in the Piedmont Physiographic Province. 6. Subsurface Structures Overhead electrical and phone utility lines are located at the subject site. Subsurface water and sewer lines are located at the site. Subsurface utilities (power and communication lines) were identified in the location of the diesel fuel UST system. Property Owners and Occupants Quality Oil Co Limited Part currently owns the subject property. Adjacent properties are developed as residential or commercial properties. The names and addresses of the adjacent property owners are provided in Table B-6 and referenced on Figure 13. SITE GEOLOGY AND HYDROGEOLOGY According to the Geologic Map of North Carolina (1985), the subject property is situated in the Piedmont Physiographic Province of North Carolina. Recharge to the water table is primarily by precipitation infiltrating the upper soils and percolating downward, under the influence of gravity, to the groundwater table. Typically, the water table is not a level surface, but a subdued reflection of the land surface. However, the depth to the water table is not constant, and depends on many factors, which include the amount of rainfall, the permeability of the soil, and the amount of groundwater being pumped in the area. Based on our review of the topographic map and on-site observation, the site is located on a ridge. Groundwater generally flows in directions sub-parallel to the ground surface slopes and under the influence of gravity toward points of discharge such as creeks, swamps, drainage swales, or pumped groundwater wells. Based on topographic inference, groundwater beneath the site most likely flows to the south. An unnamed pond which feeds an unnamed tributary of Glade Creek is located approximately 1,240 feet to the southwest of the source area (Figure 1). Determination of Groundwater Flow Direction The depth to the groundwater surface in each of the monitoring wells was measured prior to well purging using a decontaminated electric sounder. The top of casings of each of the monitoring wells were surveyed by Progress using a relative benchmark (top of casing for monitoring well MW-1) that was arbitrarily given an elevation of 100 feet. The elevations of the monitoring wells and the groundwater surface are both relative to that benchmark. The relative groundwater elevations are shown on Table B-7. Groundwater elevation data were collected during the CSA activities. Relative groundwater elevations are depicted on Figure 3. Based on information collected during the CSA field activities, the groundwater flow direction beneath the site appears to be generally to the south towards monitoring well MW-2. As presented in detail in Section I, the hydraulic conductivity was calculated as 0.161 ft/day and the groundwater flow velocity was calculated as 0.024 ft/day or 8.7 feet/year. D SOIL SAMPLING RESULTS Soil samples were not collected for laboratory analysis during the Comprehensive Site Assessment. Progress Environmental performed the installation of additional horizontal delineation monitoring wells, including the addition of a vertical delineation monitoring well. According to the Phase I Limited Site Assessment prepared by Progress Environmental, soil samples were collected proximal to monitoring well (MW-1) to document the absence or presence of residual soil contamination. The subsequent laboratory analyses indicated residual soil contamination in the vicinity of the monitoring well location. Additionally, naphthalene was detected at a concentration that exceeded it Residential Cleanup Level in the soil sample collected from a depth of 10 feet bls and naphthalene was detected at a concentration that exceeds its Industrial/Commercial Cleanup Level and 1,2,4- trimethylbenzene was detected at a concentration that exceeds its Residential Cleanup Level in the soil sample collected from a depth of 23 feet bls. E MONITORING WELL INSTALLATION AND GROUNDWATER SAMPLING RESULTS Five additional Type II monitoring wells (MW-2 through MW-6,, Figure 2) were installed during the CSA activities using a truck-mounted drill rig with hollow-stem augers on August 28, through August 30, 2023 by Geologic Exploration. One Type III telescoping monitoring well (DW-1) was installed using the hollow stem auger and air-rotary drilling methods on August 28, and August 29, 2023 in an effort to vertically delineate the extent of impacted groundwater. Well construction information is provided in Table B-7. The Non-Residential Well Construction Records, which include boring logs, are included in Appendix A. Progress was on site on August 31, 2023 to collect groundwater samples from monitoring wells MW-1 through MW-6 and DW-1. The groundwater samples were decanted into laboratory-supplied bottles and placed into a cooler packed with ice. The cooler was delivered under chain-of-custody to Pace Analytical Services, LLC (PACE) in Huntersville, North Carolina for testing. The groundwater samples from the new and existing monitoring wells were analyzed for VOCs by Standard Method 6200B including oxygenates; volatile petroleum hydrocarbons (VPH) by the MADEP Method, for ethylene dibromide using EPA Method 504.1, and for lead by the Standard Method. A summary of the groundwater sample analytical results is included in Table B-4. The groundwater samples were collected using the methods described in Appendix B. Laboratory analysis of the groundwater samples collected from monitoring wells MW-6 and DW-1 did not detect targeted compounds above their NC2LGWQS. Laboratory analysis of the groundwater samples collected from monitoring wells MW-1, MW-2, MW-3, and MW-5 exhibited concentrations of lead exceeding the NC2LGWQS. Laboratory analysis of the groundwater samples collected from monitoring wells MW-1 and MW-4 detected targeted petroleum compounds above their respective NC2LGWQS. None of the targeted petroleum hydrocarbons exceed their applicable Gross Contamination Level (GCL). The laboratory analytical report for the groundwater samples is provided in Appendix C. Prior to sampling, the wells were developed until the development water ran relatively clear, and the well was purged of an additional three casing volumes of water prior to sampling. The depth to the groundwater surface in each of the wells was measured prior to well purging using a decontaminated electric sounder. The wells were previously surveyed by Progress using a relative benchmark that was arbitrarily given an elevation of 100 feet (top of casing of monitoring well MW-1). The elevations of the monitoring wells and the groundwater surface are both relative to that benchmark. The relative groundwater elevations are shown in Table B-7. The relative groundwater elevations are depicted on Figure 3. Based on information collected during the CSA field activities, the groundwater flow direction appears to be generally to the south beneath the site. Well construction information is provided in Table B-7. The Non-Residential Well Construction Records, which include boring logs, are included in Appendix A. Maps depicting the plume of impacted groundwater and benzene, Total BTEX, MTBE, and naphthalene concentrations are included on Figures 6 through 9, respectively. Water-Supply Well Sampling Progress was not requested to sample each accessible water-supply well within the vicinity of the source area. F FREE PRODUCT INVESTIGATION/RECOVERY Progress did not detect free product in the site monitoring wells. G HYDROGEOLOGIC INVESTIGATION Progress conducted single-well groundwater recovery tests (slug tests) in monitoring wells MW-4 and MW- 6 to develop hydraulic conductivity estimates for the surficial aquifer. Each recovery test involved quickly removing water from the well by bailing and measuring the rate of groundwater recovery using a water level sounder. The recovery test data measures the productive capacity of each well and allows estimation of the hydraulic conductivity and transmissivity of the surrounding aquifer. Progress conducted the recovery tests on August 31, 2023. The recovery tests were terminated when the water-level reached at least 90 percent of the initial static water level measured prior to bailing or pumping or when sufficient time had elapsed. Estimating Aquifer Parameters The results of the recovery tests were used to calculate an average hydraulic conductivity for the surficial aquifer of approximately 0.161 ft/day or 58.7 ft/year. The Bower and Rice Slug Test Method was utilized to develop hydraulic conductivity estimates. The recovery test data and calculations are presented in Appendix D. The average linear groundwater flow velocity was calculated based on the measured water table gradient (0.037 ft/ft, gradient presented from potentiometric map in the CSA), an average hydraulic conductivity of 0.161 ft/day, and an estimated value of 0.25 for effective porosity. The average linear groundwater flow velocity (v) was calculated according to the following equation: v = (K dh/dl)/ne , where K = Hydraulic Conductivity dh/dl = Hydraulic gradient ne = effective porosity Based upon this calculation, the average linear velocity of groundwater is expected to be approximately 0.024ft/day or 8.7 feet/year. It is important to note that the assumed value for effective porosity is an estimate based on predominant soil types encountered during construction of monitoring wells at the site. It should also be noted that the calculated velocity is an approximate average for the surficial aquifer in the vicinity of the former UST system. Based on interpretation of the topographic maps and on-site observations, surface runoff at the site would be expected to flow overland following topography to the southeast along a stormwater feature located within the NC Highway 90 right-of-way. Based on the surface topography inferred from topographic maps, Comprehensive Site Assessment October 18, 2023 Taylorsville Quality Plus Site Progress Project No. QUA420N.GW 1115 NC Highway 90 Taylorsville, Alexander County, North Carolina 9 properties generally to the north would be considered upgradient of the site. Based on the groundwater flow direction calculations, groundwater beneath the site appears to flow generally to the south. Estimated Contaminant Mass Based on review of the groundwater data, Progress estimates that there is an approximately 11,000 square foot groundwater contamination plume emanating from the source area exhibiting non-zero Total BTEX compounds concentrations. Based on an estimated 25-foot saturation thickness, an assumed porosity of 25%, and average BTEX concentration in the two monitoring wells exhibiting exceedances of Total BTEX compounds of 198.4 µg/L, Progress estimates that there is 0.38 Kg of residual contaminant mass in the groundwater beneath the site. H GROUNDWATER MODELING RESULTS Groundwater modeling was not completed since a surface water feature is not located within 500 feet of the site. I DISCUSSION 1. The site meets the criteria for a High Risk site based on the presence of one active water- supply well being identified within 1,000 feet of the source area during the vehicular reconnaissance of accessible properties within 1,000 feet of the site or based on the response from Energy United Water Corporation. The property containing the active water-supply well is located at 166 Sunny Lane. Please note, the water-supply well located at this property serves the following addresses 166 Sunny Lane; 177 Sunny Lane; 196 Sunny Lane; 200 Sunny Lane; and, 210 Sunny Lane. Please note, according to the Energy United Water Corporation, water service is not provided to the following occupied properties within 1,000 feet of the source area: 74 Bandit Street, 76 Bandit Street, 76 Justice Court, and 980 NC 90 Highway. The property owner of 74 Bandit Street indicated the property does not have a water-supply well and drinking water is provided by “Havacup Quality Water Dispensers”. It is possible that the remaining properties are served drinking water via a private water-supply well. 2. Laboratory analysis of the soil samples collected from the soil boring associated with monitoring well MW-1 detected targeted compounds at concentrations exceeding the Soil-to- Groundwater MSCCs in each of the soil samples except for the soil sample collected from a depth of 20 feet bls. Additionally, naphthalene was detected at a concentration that exceeded it Residential Cleanup Level in the soil sample collected from a depth of 10 feet bls and naphthalene was detected at a concentration that exceeds its Industrial/Commercial Cleanup Level and 1,2,4-trimethylbenzene was detected at a concentration that exceeds its Residential Cleanup Level in the soil sample collected from a depth of 23 feet bls. 3. Laboratory analysis of the groundwater samples collected from monitoring wells MW-6, and DW-1 did not detect targeted compounds above their respective NC2LGWQS. Laboratory analysis of the groundwater samples collected from monitoring wells MW-1, MW-2, MW-3, MW-4, and MW-5 detected lead at concentrations their respective NC2LGWQS. Laboratory analysis of the groundwater samples collected from monitoring wells MW-1 and MW-4 detected targeted petroleum hydrocarbons at concentrations exceeding their respective NC2LGWQS. Please note, none of the targeted petroleum compounds were detected at concentrations that exceeded their respective GCL. Free product was not detected in any of the monitoring wells sampled as part of this assessment. 5. Water-supply wells located proximal to the site were not sampled as part of this assessment. J CONCLUSIONS AND RECOMMENDATIONS Based on the findings of this CSA, Progress makes the following conclusions and recommendations: • Groundwater flow beneath the site appears to be flowing to the south; • Soil impacted above the Soil-to-Groundwater MSCCs, Residential Soil Cleanup Levels, and/or Industrial Cleanup Levels appears to be located proximal to the south side of the UST system. The collection of additional soil samples from areas adjacent to the UST system was not conducted as part of this assessment; • Targeted petroleum compounds were detected in excess of their respectiveNC2LGWQS in the groundwater monitoring wells MW-1 and MW-4. None of the compounds detected in the groundwater samples exceed the applicable GCL; • Free product has not been measured in any of the monitoring wells installed as part of this assessment; • The release related to the USTs appears to have been adequately defined; • Because public water is available in the vicinity of the site, groundwater remediation and/or the connection of the remaining properties still utilizing groundwater obtained from water-supply wells will be required in an effort to receive regulatory closure of the site; • Prior to a real estate transaction occurring, a Notice of Residual Petroleum (NORP) must be filed with the Alexander County Register of Deeds. Progress recommends that the current property owner be notified of the findings of this assessment and that a copy of this report be submitted to the NCDEQ; • Prepare and submit a Corrective Action Plan to address the continued dissolved phase petroleum groundwater contamination. Based on the absence of receptors immediately downgradient of the source area, it is anticipated that the proposed corrective action will include long term natural attenuation. . Progress will also perform an inquiry with the property owners of the identified water-supply wells to determine whether the connection of the remaining water-supply wells is feasible. • Provide a copy of this report to the property owner. EXHIBIT B Procedures for Collecting Groundwater Samples • Each well will be purged of three standing well volumes prior to sample collection to remove stagnant water from the well and well bore in an effort to collect samples that are representative of the water quality in the formation surrounding each well. • Chemical preservatives, if applicable, will be added to sample bottles by the laboratory. • Sample bottles will be labeled prior to sample collection. • Sample bottles will be filled directly from the Teflon or disposable bailer. Volatile organic samples will be collected first. • The pH, temperature, and specific conductance of the sample will be measured and recorded. These measurements will be taken from a sample deposited in a separate container. Visual characteristics of the sample, including insoluble materials, will be recorded. • Caps will be secured on bottles, bottle will be placed in plastic bags and the bags will be sealed. • Documentation, including Chain-of-Custody Record and laboratory analytical request form, will be completed for all samples. • Samples will be packed in coolers with "bubble wrap" and ice packs for shipment. • Chain-of-Custody Record and analytical request form will be placed inside cooler sealed with security tape. • Samples will be shipped via overnight express to the analytical laboratory within 24 hours following collection. • Laboratory will be advised of each sample shipment.
