1.0 Executive Summary An apparent release of an unknown quantity of petroleum hydrocarbons occurred at the site prior to October 1997. The location of the release is unknown although previous reports suggest
a surface release proximal to the former loading rack.
• On October 21, 1997 DENR issues Notice of Regulatory Requirement (NORR) requiring site assessment related to a reported diesel fuel surface spill.
• Thompson Geological and Environmental Services (TGES) submitted a Comprehensive
Site Assessment to DENR on June 29, 1998.
• TGES submitted a CSA Addendum on October 25, 1998.
• Karen E. Heater, P.E. submitted a Corrective Action Plan (CAP) on April 6, 1999. • Duncklee & Dunham Environmental Consultants supervised the removal of 1,526 tons of
petroleum impacted soil and applied ORC (oxygen release compound) to the base of the
excavation in May 2003.
• Haggett Engineering Associates, Inc. (HEA) submitted a CAP Addendum on July 15, 2014.
• DENR approved the CAP Addendum on July 25, 2014.
• Startup of the Soil Vapor Extraction System was completed on January 12, 2015.
Four (4) monitoring wells were sampled on January 31, 2023. The Groundwater samples from monitoring wells MW-7D and MW-14 and MW-14D were
reported to contain petroleum hydrocarbon constituents above the 2L standard.
The concentration of Benzene in monitoring well MW-7D was reported at 431 ug/l which is a decrease from 518 ug/l. Concentrations in MW-14 and MW-14D were 1.60 ug/l and 385 ug/l respectively.
Groundwater elevations were slightly more than 3-feet higher in MW-3D and MW-7D than those reported during the October 4, 2022 gauging. However, the elevations reported in MW-14 and MW-14D were slightly lower.
The SVE system has operated few hours since October 2022 due to operational issues.
HEA recommends that the site return to semi-annual sampling beginning in June 2023. Sampling should include monitoring wells MW-3D, MW-7D, MW-9, MW-9D, MW-14 and MW-14D. In addition, sampling of the water supply wells should continue.

2.0 Introduction
2.1 Site Location
Home Concrete and Supply, LLC (formerly Steele Feed and Seed) is located at 11575 Graham
Road, in Mt. Ulla, North Carolina (figures 1 & 2). The property was previously used by Home Oil Company for the bulk storage of heating oil and motor fuels. Figure 3 illustrates the current layout of the site. A bulk storage facility was operated at the site from the early 1970’s until the mid 1990’s. The bulk storage facility consisted of five aboveground storage tanks (AST’s), a
loading rack and gasoline dispenser island.
2.2 Purpose of Groundwater Monitoring Report
This report is intended to provide a summary of the remediation completed and to recommend
changes that may be necessary to enhance the remedial effort. This is the eighth active
remediation monitoring report for this site. 3.0 Discussion of Sampling Results
3.1 Summary of Analytical Results
The groundwater in the monitoring wells has been periodically sampled to determine the concentration of hydrocarbon constituents present. Tables 2, 3 and 4 are provided to summarize the groundwater data collected during sampling events. Table 2 provides a summary of the analytical data from the most recent sampling event. Table 3 provides a
chronologic listing of the groundwater elevation data and table 4 provides a chronologic listing of the groundwater chemistry. Table 5 provides a chronologic listing of the soil vapor extraction (SVE) system emissions. Four (4) monitoring wells were sampled on January 31, 2023.
The Groundwater samples from monitoring wells MW-7D, MW-14 and MW-14D were reported to contain petroleum hydrocarbon constituents above the 2L standard. The concentrations of toluene were reported in the air emission sample. Emissions have
increased slightly since the October 4, 2022 sampling. The SVE system has operated 3,137.9 hours over the past year. 4.0 Environmental History

4.2 Release Information
An apparent release of an unknown quantity of petroleum hydrocarbons occurred at the site
proximal to the former AST System prior to September 1997. Information from previous reports
indicates that there may have been a surface spill of diesel fuel proximal to the loading rack. Sampling results indicate the presence of petroleum hydrocarbon constituents typical of gasoline and diesel fuel in the soil and groundwater. 5.0 Site Remediation
5.1 Summary of Remedial Activities to Date
Information from previous reports indicates that following a surface release of diesel fuel prior
to September 1997, soils were excavated and removed from the site. An AFVR (aggressive fluid vapor recovery) event was completed on MW-7R proximal to the former loading rack on July 29, 1998. In May 2003, 1,526 tons of petroleum impacted soils were excavated from the site. Following this excavation, ORC® (Oxygen Release Compound) was applied as a slurry to
the base of the excavation. In July 2003 ORC was injected into the sub-surface under a permit issued by the UIC Program. The current SVE (soil vapor extraction) system began operation on January 12, 2015. Since the initial air emission sampling on January 12, 2015, the daily emissions have decreased by more than 99%. We believe this reduction is an indication that the concentrations of the petroleum hydrocarbon constituents in the sub-surface soils have
been reduced. However, the concentration in the off-gas is influenced by many factors including

the depth to groundwater. As the groundwater surface rises there is a corresponding reduction in the soil that is exposed to remediation by the vapor extraction system. Based on the results of the emission sampling, we estimate the removal of approximately 304.74 kg of petroleum
hydrocarbon constituents by the soil vapor extraction system. In addition to the removal of petroleum hydrocarbons constituents in the air stream, the SVE system also introduces oxygen into the subsurface with promotes bio-remediation by naturally occurring microbes in the soil. The air sample collected on January 31, 2022 reported a slight increase in concentrations.
Results of the recent groundwater sampling indicate that the concentrations of petroleum hydrocarbon constituents in the groundwater have increased slightly. Based on the analytical results from the January 31, 2023 sampling, monitoring wells MW-7D, MW-14 and MW-14D contain benzene at a concentration exceeding the 2L standards. No other wells were reported to contain constituents exceeding the 2L standards.
5.2 Remediation System Status
The SVE system began operation on January 12, 2015 and is currently being repaired. Run time for the system was reported to be 3,138.2 hours during the January 31, 2023 site visit.
The previous run time was recorded to be 3,137.9 on October 4, 2022. Total run time for the system is approximately 44,215 hours.
5.3 Remediation System Sampling Data
Emissions from the SVE system were sampled following startup and have been sampled periodically since that time. Table 5 presents a Chronologic Listing of the SVE Emissions.
5.4 Future Remedial Activities
The Corrective Action Plan (CAP) approved on July 25, 2014 included the injection of Boss 200® into the aquifer to address groundwater contamination. Based on the results of the October 4, 2022 groundwater sampling we believe that the operation and sampling of the SVE should continue to the extent practical. Although the emissions are minimal, we believe the circulation of air through the unsaturated soils does enhance the biodegradation of the
hydrocarbon constituents. The results do not support the need for additional groundwater remediation. Additional sampling of select groundwater monitoring wells and potable water wells should be completed in June 2023.
5.5 Discharge Permits
No discharge permits are required for this system.

6.0 Conclusion The area proximal to the site is currently not served by a public water system. Therefore, homes and businesses located proximal to the site depend on groundwater for their water supply.
Groundwater samples collected on January 31, 2023 indicate the presence of petroleum hydrocarbon constituents in levels exceeding the 15A NCAC 2L Class GA groundwater standard. The analytical results indicate an increase in concentrations in three monitoring wells at the site. Concentrations at the site appear to vary as the groundwater levels rise and fall.
Recent elevations indicate an anomaly near MW-3D and MW-7D in relation to changes
observed in MW-14D. An increase of slightly more than 3-feet was observed in MW-3D and MW-7D while the elevation in MW-14D was nearly unchanged. The SVES is currently undergoing repair and adjustment to the control panel. Sampling of the
SVES discharge should be completed whenever monitoring wells are sampled in order to
assess the sub-surface soil conditions. Based on the groundwater data from the sample event, HEA recommends the continued operation of the SVE system including semi-annual sampling of the air emissions. The SVE
system operation should be adjusted to maximize the flow in the most impacted areas (see
Figure 5 for valve settings). HEA also recommends that the SVE system be cycled to maximize the efficiency. This will reduce power consumption now that the emission concentrations have remained low. The following operation regime is recommended to take advantage of ambient air temperature as we enter the warmer months:
• 7 AM to 8 AM
• 12 PM to 1 PM
• 5 PM to 6 PM
We do not believe it will be necessary to proceed with additional groundwater remediation based
on the current concentrations of petroleum hydrocarbon constituents in the groundwater.
There are a significant number of monitoring wells related to this incident. Many of these have not been sampled in quite a few years due to consistent sampling events with no detection. HEA recommends that the following wells be properly abandoned to reduce the risk to the
aquifer(s) and cost for maintenance:

CASE NARRATIVE
All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form
or properly qualified within the sample results. By my digital signature below, I affirm to the best of my
knowledge, all problems/anomalies observed by the laboratory as having the potential to affect the
quality of the data have been identified by the laboratory, and no information or data have been
knowingly withheld that would affect the quality of the data.

GLOSSARY OF TERMS
Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.
Abbreviations and Definitions
MDL Method Detection Limit.
ND Not detected at the Reporting Limit (or MDL where applicable).
RDL Reported Detection Limit.
Rec.Recovery.
RPD Relative Percent Difference.
SDG Sample Delivery Group.
(S)Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media.
U Not detected at the Reporting Limit (or MDL where applicable).
Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL”
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non-conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
Qualifier Description
C3 The reported concentration is an estimate. The continuing calibration standard associated with this data responded low.
Method sensitivity check is acceptable.
J1 Surrogate recovery limits have been exceeded; values are outside upper control limits.
J3 The associated batch QC was outside the established quality control range for precision.
J4 The associated batch QC was outside the established quality control range for accuracy.

CASE NARRATIVE
All sample aliquots were received at the correct temperature, in the proper containers, with the
appropriate preservatives, and within method specified holding times, unless qualified or notated within
the report. Where applicable, all MDL (LOD) and RDL (LOQ) values reported for environmental samples
have been corrected for the dilution factor used in the analysis. All Method and Batch Quality Control
are within established criteria except where addressed in this case narrative, a non-conformance form
or properly qualified within the sample results. By my digital signature below, I affirm to the best of my
knowledge, all problems/anomalies observed by the laboratory as having the potential to affect the
quality of the data have been identified by the laboratory, and no information or data have been
knowingly withheld that would affect the quality of the data.

GLOSSARY OF TERMS
Guide to Reading and Understanding Your Laboratory Report
The information below is designed to better explain the various terms used in your report of analytical results from the Laboratory. This is not
intended as a comprehensive explanation, and if you have additional questions please contact your project representative.
Results Disclaimer - Information that may be provided by the customer, and contained within this report, include Permit Limits, Project Name,
Sample ID, Sample Matrix, Sample Preservation, Field Blanks, Field Spikes, Field Duplicates, On-Site Data, Sampling Collection Dates/Times, and
Sampling Location. Results relate to the accuracy of this information provided, and as the samples are received.
Abbreviations and Definitions
MDL Method Detection Limit.
ND Not detected at the Reporting Limit (or MDL where applicable).
RDL Reported Detection Limit.
Rec.Recovery.
RPD Relative Percent Difference.
SDG Sample Delivery Group.
(S)Surrogate (Surrogate Standard) - Analytes added to every blank, sample, Laboratory Control Sample/Duplicate and Matrix Spike/Duplicate; used to evaluate analytical efficiency by measuring recovery. Surrogates are not expected to be detected in all environmental media.
U Not detected at the Reporting Limit (or MDL where applicable).
Analyte The name of the particular compound or analysis performed. Some Analyses and Methods will have multiple analytes
reported.
Dilution
If the sample matrix contains an interfering material, the sample preparation volume or weight values differ from the
standard, or if concentrations of analytes in the sample are higher than the highest limit of concentration that the
laboratory can accurately report, the sample may be diluted for analysis. If a value different than 1 is used in this field, the
result reported has already been corrected for this factor.
Limits
These are the target % recovery ranges or % difference value that the laboratory has historically determined as normal
for the method and analyte being reported. Successful QC Sample analysis will target all analytes recovered or
duplicated within these ranges.
Qualifier
This column provides a letter and/or number designation that corresponds to additional information concerning the result
reported. If a Qualifier is present, a definition per Qualifier is provided within the Glossary and Definitions page and
potentially a discussion of possible implications of the Qualifier in the Case Narrative if applicable.
Result
The actual analytical final result (corrected for any sample specific characteristics) reported for your sample. If there was
no measurable result returned for a specific analyte, the result in this column may state “ND” (Not Detected) or “BDL”
(Below Detectable Levels). The information in the results column should always be accompanied by either an MDL
(Method Detection Limit) or RDL (Reporting Detection Limit) that defines the lowest value that the laboratory could detect
or report for this analyte.
Uncertainty
(Radiochemistry)Confidence level of 2 sigma.
Case Narrative (Cn)
A brief discussion about the included sample results, including a discussion of any non-conformances to protocol
observed either at sample receipt by the laboratory from the field or during the analytical process. If present, there will
be a section in the Case Narrative to discuss the meaning of any data qualifiers used in the report.
Quality Control
Summary (Qc)
This section of the report includes the results of the laboratory quality control analyses required by procedure or
analytical methods to assist in evaluating the validity of the results reported for your samples. These analyses are not
being performed on your samples typically, but on laboratory generated material.
Sample Chain of
Custody (Sc)
This is the document created in the field when your samples were initially collected. This is used to verify the time and
date of collection, the person collecting the samples, and the analyses that the laboratory is requested to perform. This
chain of custody also documents all persons (excluding commercial shippers) that have had control or possession of the
samples from the time of collection until delivery to the laboratory for analysis.
Sample Results (Sr)
This section of your report will provide the results of all testing performed on your samples. These results are provided
by sample ID and are separated by the analyses performed on each sample. The header line of each analysis section for
each sample will provide the name and method number for the analysis reported.
Sample Summary (Ss)This section of the Analytical Report defines the specific analyses performed for each sample ID, including the dates and
times of preparation and/or analysis.
Qualifier Description
