EXECUTIVE SUMMARY
Rushco Food Store #5 is located at 601 Jake Alexander Boulevard West in Salisbury, Rowan County, North
Carolina. The site is currently owned and operated by Clark Stores, LLC as a petroleum retail facility and
convenience store. The ground water assessment was done as a follow up to incident number 16588.
Currently, one compartmentalized 30,000-gallon gasoline UST, subdivided into a 10,000-gallon gasoline
compartment and a 20,000-gallon gasoline compartment is in use at the site. The adjoining properties
are a mixture of multi-family residential, single family residential, and commercial properties. According
to the information determined during receptor survey activities, no water supply wells were identified
within 1,500 feet of the site. Public water is available to the site and surrounding properties within a
1,500-foot radius of the site. The site is not in a Wellhead Protection Area. An unnamed tributary to Grants
Creek lies within 500 feet of the source area.
On August 24, 2023, four monitoring wells, one telescoping well, and one surface water body were
gauged, purged and/or sampled. Free product was not observed at the site. The concentrations of MTBE,
and TBA reported in the ground water samples collected from MW-1, MW-4 and/or TW-1 exceeded the
MACs. An estimated concentration of MTBE was detected in the surface water sample (SW-1) but did not
exceed the MACs. All of the reported contaminant concentrations were below the 10 times 2B surface
water standards and the GCLs.
Based on this information, the incident should be closed with a Notice of Residual Petroleum and No
Further Action.

1.0 SITE HISTORY AND CHARACTERIZATION
GRI presents the results of the August 2023 ground water sampling event at the Rushco Food Store #5
site; located at 601 Jake Alexander Boulevard West in Salisbury, Rowan County, North Carolina. A
topographic map showing the general site location and the surrounding area is included as Figure 1. These
activities were conducted in accordance with GRI Proposal No. 23-458, which was submitted to the
NCDEQ on July 20, 2023, and was approved as Task Authorization No. 16588-1 on July 25, 2023. State
correspondence has been included as Appendix A.
The site is currently owned and operated by Clark Stores, LLC as a petroleum retail facility and convenience
store. Currently, one compartmentalized 30,000-gallon gasoline UST, subdivided into a 10,000-gallon
gasoline compartment and a 20,000-gallon gasoline compartment is in use at the site. UST
owner/operator information and UST system information are included as Tables 1 and 2, respectively.
The Registered Tank Report is shown below:
The site and surrounding area is a mixture of multi-family residential, single-family residential, and
commercial properties. The site contains one building and is primarily capped with concrete and asphalt.
The overall topography at the site is inclined with a down gradient from south to north. Four Type II
monitoring wells (MW-1 through MW-4), and one telescoping monitoring well (TW-1) were installed at
the site during previous ground water assessment activities. A Site Map showing the locations of the
monitoring wells and the structures on-site is included as Figure 2.
On August 24, 2023, GRI personnel conducted an updated receptor survey at the site. No Water Supply
Wells were found within 1,500 feet of the site. The site is not in a Wellhead Protection Area. An unnamed
tributary to Grants Creek is located approximately 450 feet northeast of the subject site. Receptor
information is included in Table 3. Adjacent property owner information is included in Table 4. A Site
Vicinity Map showing the adjacent properties is included as Figure 3.
No additional releases exist at the site.
One incident (Incident No. 36974) has been reported on an adjacent property. Copies of the NCDEQ
Incident Report are included as Appendix B. A map showing the locations of additional incidents in the
vicinity of the site is shown below:
No CAP has been planned or implemented for the site.
2.0 PRESENTATION OF CURRENT SITE ASSESSMENT INFORMATION/ COMPARISON TO HISTORICAL
ASSESSMENT INFORMATION
Soil assessment was not conducted during the current scope of work. Historical soil quality information
is included as Appendix C.
36974 Site
On August 24, 2023, four Type II monitoring wells (MW-1 through MW-4), one telescoping well (TW-1),
and one surface water body (SW-1) were gauged, purged and/or sampled in accordance with the NCDEQ
guidelines. Free product was not observed at the site. Depths to ground water in the Type II monitoring
wells ranged from 7.76 feet to 21.05 feet. Ground water elevations in the Type II monitoring wells ranged
from 78.36 feet to 92.89 feet. Ground water flow was generally to the north with a horizontal gradient of
approximately 0.05 feet per foot. The vertical gradient between wells MW-1 and TW-1 was approximately
0.42 feet per foot in the downward direction. Well construction and gauging information is included in
Table 5. A Water Table Surface Map based on the Type II monitoring wells is included as Figure 4. A
summary of the current ground water gauging data is shown below:

Ground water samples collected from the monitoring wells were submitted for laboratory analyses using
EPA Method 6200B with oxygenates. The concentrations of MTBE, and TBA reported in the ground water
samples collected from MW-1, MW-4 and/or TW-1 exceeded the MACs. An estimated concentration of
MTBE was detected in SW-1 but did not exceed the MAC. All of the reported contaminant concentrations
were below the 10 times 2B surface water standards and the GCLs. A summary of the analytical results
for ground water samples collected on August 24, 2023. is included in Table 6. A Ground Water Quality
Map based on the August 2023 data is included as Figure 5. A complete report of laboratory analyses of
the ground water samples collected from the monitoring wells during the August 2023 sampling event is
included as Appendix D. A summary of the current ground water analytical data is shown below:

The site is located in the Piedmont Physiographic Province of North Carolina. The seasonal high ground
water table is approximately seven feet BGS.
3.0 FREE PRODUCT REMOVAL
To the knowledge of GRI, no free product has been observed at the site.
4.0 NATURAL ATTENUATION / NATURAL SOURCE ZONE DEPLETION STATUS
The site is in the active assessment phase. No natural attenuation monitoring plan is in place at the site.
5.0 CONCLUSIONS
• On August 24, 2023, GRI personnel conducted an updated receptor survey at the site. No Water
Supply Wells were found within 1,500 feet of the site. The site is not in a Wellhead Protection
Area. An unnamed tributary to Grants Creek is located approximately 450 feet northeast of the
subject site.

6.0 LIMITATIONS
This report has been prepared for the exclusive use of Rusher Oil Company, Inc. and the NCDEQ for specific
application to the referenced site in Rowan County, North Carolina. The assessment was conducted based
on the scope of work and level of effort desired by the NCDEQ and with resources adequate only for that
scope of work. Our findings have been developed in accordance with generally accepted standards of
environmental practices in the State of North Carolina, available information, and our professional
judgment. No other warranty is expressed or implied.
The data that are presented in this report are indicative of conditions that existed at the precise locations
sampled and at the time the samples were collected. In addition, the data obtained from samples would
be interpreted as being meaningful with respect to parameters indicated in the laboratory report. No
additional information can logically be inferred from these data.

Dear Mr. Rusher:
The report received on August 30, 2004, has been reviewed by the UST Section, Division of Waste
Management, Mooresville Regional Office. The risk posed by the discharge or release at the subject site is
classified by the Department of Environmental Quality as intermediate, as stipulated under Title 15A
NCAC 02L .0406. The land use at the site is classified as industrial/commercial. Title 15A NCAC 02L
.0407(a) requires you to notify the Department of any changes that might affect the risk or land use
classifications that have been assigned.
The review indicates that corrective action is necessary to remediate environmental contamination. Title
15A NCAC 02L .0407(d) [Intermediate Risk] requires you to comply with the assessment and cleanup
requirements of Title 15A NCAC 02N .0706, Title15A NCAC 02L .0106(c), Title 15A NCAC 02L .0110,
and Title 15A NCAC 02L .0111. A Groundwater Monitoring Report prepared in accordance with
these requirements and a new Receptor Survey completed by October 17, 2023, would suffice in
moving forward with an NRP if the surface water body can be eliminated as per UST MRO staff.
For additional reference, please refer to the most recent version of the Guidelines for Site Checks, Tank
Closures, and Initial Response and Abatement. Report requirements can be found in the most recent version
of the Comprehensive Appendices for Correction Guidelines. Reports must contain the information
specified in the report template presented in Appendix A of the Guidelines. The report template is attached
for your information.
Failure to comply in the manner and time specified may result in the assessment of civil penalties
and/or the use of other enforcement mechanisms.

The Department of Environmental Quality requires that all work not determined to be an emergency
response or associated with risk assessment (i.e., the Limited Site Assessment Report, per Title 15A NCAC
02L .0405) must be preapproved if State Trust Fund reimbursement is anticipated. To comply with this
requirement, a completed Preapproval/Claim Authorization Form, encompassing the required remedial
activities, must be received in this office within 14 days of the date of this letter. Upon completion of the
preapproved activities, you should submit your claim promptly. Reimbursement funds are budgeted based
on completed preapprovals, but delays in reimbursement or even denial due to exceeding the statute of
limitations on claiming eligible costs can result where claims are not submitted immediately following work
completion.
Because a release or discharge has been confirmed, a Licensed Geologist or a Professional Engineer,
certified by the State of North Carolina, is required to prepare and certify all reports submitted to the
Department in accordance with 15A NCAC 02L .0103(e) and 02L .0111(e) if required.
