STATUS OF PERMIT:
Permit to Operate issued March 20, 2020.
Permit to Operate will expire March 20, 2025. The Permit Renewal paperwork should be submitted to the Section
Permitting Engineer by September 20, 2024.
PURPOSE OF SITE VISIT:

STATUS OF PAST NOTICE OF VIOLATIONS:
None.
OBSERVED VIOLATIONS:
None
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any
such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1. The facility has been shut down since mid -March 2020, due to COVID-19. Mr. Hare stated that there may be a
demonstration of the facility in the upcoming weeks. Please keep me informed as to when the demonstration is so
that I may be able to attend.
2. The facility is restricted to collecting and storing less than 13,000 scrap tires at any time in accordance with the
Financial Responsibility requirements of 15A NCAC 13B .1111. There are currently 3 bays containing tires, an
amount less than the 13,000-scrap tire maximum.
3. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact
information as well as materials accepted at the facility.
4. The access around the facility is stable and no soil erosion was noted.
5. The sediment basins appeared to be functioning properly at the time of this inspection. The woody vegetation has
been removed from the sediment basin since the las inspection.
6. A street sweeper is maintained on -site and used to keep the operations area clean.
7. Digital pictures were taken during the inspection.
