Dear Mr. Hursey:
The Groundwater Monitoring Report received by the UST Section, Division of Waste Management,
Winston-Salem Regional Office on June 9, 2023 has been reviewed. The risk posed by the discharge or
release at the subject site is classified by the Department of Environmental Quality as HIGH, as stipulated
under Title 15A NCAC 2L .0406. The land use at the site is classified as residential. Therefore, our office
requests that groundwater monitoring be conducted on a semi-annual schedule. In addition, as part of the
next monitoring event, our office requests that the water supply wells located at 6494 and 6496 Burlington
Road be sampled and analyzed for petroleum constituents. The next monitoring event should be conducted
in October 2023.
If you have any questions regarding the actions that must be taken or the rules mentioned in this letter,
please contact Ashley Dinkins at (336) 641-5557 or at the Guilford County Environmental Health address
