Box 1360 Robbins, NC 27325 Attention: Mr. Steven Majors Reference: Report of Limited Groundwater Assessment Eskimo Joes/Incident #20918/UST #WS-5813 1705 Cotton Grove Road, Lexington, Davidson County, North Carolina S&ME Project No. 1584-02-045F Dear Mr. Majors: S&ME, Inc. (S&ME) submits this Letter Report for the above-referenced site in accordance with S&ME’s Proposal No. 22380059A, dated April 26, 2023. This work was performed at the request of the North Carolina Department of Environmental Quality (NCDEQ) as pre-approved under Task Authorization No. 20918-17 and signed by the Trust Fund Section of the Central Office on April 26, 2023. The scope of services included the following tasks. Measure depth to water and collect groundwater samples from monitoring wells MW-1 and MW-2 for analyses by Method 6200B and by Method 504.1. Submit a letter report. Tasks were completed as proposed. Current Site Assessment Information Current Liquid Levels On August 18, 2023, S&ME measured the depth to groundwater from the top of casing in monitoring wells MW-1 and MW-2 using an electronic water level indicator. The depth-to-water measurements ranged from 17.99 feet below top of casing (ft-BTOC) in monitoring well MW-1 to 17.75 ft-BTOC in monitoring well MW-2. Compared to the previous sampling event performed during February 2023, groundwater occurrence in monitoring wells MW-1 and MW-2 decreased approximately 3.5 feet in elevation. The depth to groundwater measurements and relative groundwater elevations are provided in Table 1. Groundwater Monitoring Event On August 18, 2023, after measuring the depth-to-water, S&ME personnel purged monitoring wells MW-1 and MW-2, using disposable polyethylene bailers of approximately three well volumes. Field parameters pH, specific conductivity and temperature were measured during the well purging process and recorded on the groundwater sampling forms, which are included in Attachment I. After completing the well purging, S&ME personnel collected groundwater samples from monitoring wells MW-1 and MW-2. After arriving on site, I opened both wells to allow liquid levels to equilibrate before taking depth to water measurements. After letting the wells sit for approximately 10-15 minutes, I began measuring DTW. After taking DTW measurements, I began purging and sampling MW-1 and MW-2. I collected both samples after purging each well of at least 3 volumes. Each collected sample was placed in a laboratory supplied container. After sampling MW-1 and MW-2, I closed both wells. I stored the samples in a cooler on ice as they were collected. The samples were delivered to Pace Laboratories in Kernersville, NC. ND - Not Detected at or above adjusted reporting limit. TNTC - Too Numerous To Count J - Estimated concentration above the adjusted method detection limit and below the adjusted reporting limit. MDL - Adjusted Method Detection Limit. PQL - Practical Quantitation Limit. RL - Reporting Limit - The lowest concentration value that meets project requirements for quantitative data with known precision and bias for a specific analyte in a specific matrix. S - Surrogate 1,2-Diphenylhydrazine decomposes to and cannot be separated from Azobenzene using Method 8270. The result for each analyte is a combined concentration. Consistent with EPA guidelines, unrounded data are displayed and have been used to calculate % recovery and RPD values. LCS(D) - Laboratory Control Sample (Duplicate) MS(D) - Matrix Spike (Duplicate) DUP - Sample Duplicate RPD - Relative Percent Difference NC - Not Calculable. SG - Silica Gel - Clean-Up U - Indicates the compound was analyzed for, but not detected. Acid preservation may not be appropriate for 2 Chloroethylvinyl ether. A separate vial preserved to a pH of 4-5 is recommended in SW846 Chapter 4 for the analysis of Acrolein and Acrylonitrile by EPA Method 8260. N-Nitrosodiphenylamine decomposes and cannot be separated from Diphenylamine using Method 8270. The result reported for each analyte is a combined concentration. Reported results are not rounded until the final step prior to reporting. Therefore, calculated parameters that are typically reported as "Total" may vary slightly from the sum of the reported component parameters. Pace Analytical is TNI accredited. Contact your Pace PM for the current list of accredited analytes. TNI - The NELAC Institute. ANALYTE QUALIFIERS The precision between the sample and sample duplicate exceeded laboratory control limits.D6 Matrix spike recovery exceeded QC limits. Batch accepted based on laboratory control sample (LCS) recovery.M1 Surrogate recovery not evaluated against control limits due to sample dilution.S4 REPORT OF LABORATORY ANALYSIS This report shall not be reproduced, except in full, without the written consent of Pace Analytical Services, LLC.Date: 08/24/2023 01:27 PM Pace Analytical Services, LLC 9800 Kincey Ave. Suite 100 Huntersville, NC 28078 (704)875-9092 Page 26 of 30 #=CR# QUALITY CONTROL DATA CROSS REFERENCE TABLE Pace Project No.: Project: 92683440 Eskimo Joes Lab ID Sample ID QC Batch Method QC Batch Analytical Method Analytical Batch 92683440001 795052 795203MW-1 EPA 504.1 EPA 504.1 92683440002 795052 795203MW-2 EPA 504.1 EPA 504.1 92683440001 795384MW-1 SM 6200B 92683440002 795384MW-2 SM 6200B 92683440003 795157Trip Blank SM 6200B REPORT OF LABORATORY ANALYSIS This report shall not be reproduced, except in full, without the written consent of Pace Analytical Services, LLC.Date: 08/24/2023 01:27 PM Pace Analytical Services, LLC 9800 Kincey Ave. Suite 100 Huntersville, NC 28078 (704)875-9092 Page 27 of 30 Page 28 of 30 Page 29 of 30 Page 30 of 30 Attachment III- Receptor Survey Update Documentation Updated November 5, 2020 Owner's Name Approximate Well Use Parcel Number Source Well # and Well Address Owners Mailing Address Well Depth Well Casing Distance From (Lexington, N.C.)(feet)Depth (feet)Source (feet) 0600200000010B Owner interview (2020)W14 Bruce Lopp P.O.
