SUBJECT: Request for Health Risk Evaluation of Water Supply Tracking ID: #12913, Harold Smith Grocery Enclosed please find the analytical results for a water sample collected on February 28, 2017 from the McMillan water supply well, identified as WSW-2765, located at 2765 Union Ridge Road, Burlington, Alamance County. The McMillan residence is currently using a point of entry treatment system. Please perform a health risk evaluation and send the evaluation with your recommendation and a copy of this memo to me. I will then forward the health risk evaluation and recommendation to any well users.
