Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any

ADDITIONAL COMMENTS:
01 – The purpose of this site visit was to complete a Compliance Inspection. 02 – The facility has locking gate and fencing for security. 03 – The landfill side slopes appear to meet the 3:1 requirement. 04 – The landfill has good vegetative cover and is maintained in good condition.
05 – The roads are of all-weather construction and maintained in good condition. Small scrubs were observed growing at the base of the landfill where the road is constructed. Care should be taken when uprooting scrubs close to the base. Some areas of the landfill access road were wet with tire tracks. Efforts should be taken to seed and repair, and to minimize any unnecessary vehicle traffic. 06 - One of the Edge of Waste Markers observed had fallen over and needs to be secured in an upright position. 07 - Groundwater monitoring results from December 2023 show Benzene and 1,4 Dioxane elevated above the 2 L
groundwater standards in well #5, and well #8. NC DEQ SWS Permitting Unit, Benjamin Kelly is in receipt of this data. The COC form attached with the lab results did not indicate whether the samples were packed in ice when the lab received them. Please confirm the COC and receiving lab acknowledge whether ice was used. The temperature on receipt was
noted as 3.1 Deg Celsius.

Landfill road and landfill access road. Some small scrubs at base. Vegetative cover was in good condition.
Edge of waste marker(s) were present as required. One of the EOW markers observed had fallen over. Access road and Edge of Waste Marker adjacent to pond.
Some areas of the landfill access road were wet with tire tracks along the base of the landfill.

Groundwater Monitoring Wells #5 and #6 were observed locked, labelled, and in good condition.
