(ATC) has completed a Groundwater Monitoring Report for the former Avent’s Grocery site. This report documents a limited groundwater and water supply well sampling event conducted at the site on January 30, 2024. The monitoring activities are being performed based on a January 16, 2024, Task Authorization (TA-02) by the Trust Fund Branch of the Underground Storage Tank (UST) Section, Division of Waste Management, North Carolina Department of Environmental Quality (NCDEQ). The work is being performed as part of the State Lead Contract N70522-B between NCDEQ and ATC. The site consists of two properties (Avent’s Grocery – Incident #12793 and Giddensville Game Room - Incident #3550 [closed]) associated with separate releases. Both properties are located at the intersection of Goshen Church Road and Giddensville Road in Giddensville, Sampson County, North Carolina. The Avent’s property is developed with one single-story building occupied by El Lucero (a restaurant). Eight monitoring wells (MW-1, MW-2, MW-3R, MW-4R, MW-5, MW-6R, MW-10, and MW-11) are associated with the site. The Giddensville Game Room site is developed with a vacant one-story building and the property is used for storing vehicles. Four monitoring wells (GMW-1R through GMW-4R) are located on the property. Twenty water supply wells are also located within 1,000 feet of the site. Municipal water is not available to the site or the site vicinity. On January 30, 2024, groundwater samples were collected from monitoring wells MW-1, MW-10, MW-11, and GMW-4R and water supply well WSW-6. Laboratory analytical results from the groundwater sampling event indicate petroleum constituents in monitoring wells MW-1, MW-10, and GMW-4R above the Title 15A NCAC 02L .0202 Groundwater Quality Standards (2L Standards). No compounds were detected above 2L Standards in MW-11. No compounds were identified at concentrations exceeding the corresponding Gross Contamination Levels (GCLs). Compounds for the wells are generally stable, decreasing, or have no discernable trend, with the exception of ethylbenzene, total xylenes, and naphthalene in well GWM-4R, which indicate an increasing trend. As a result, it appears natural attenuation may not be occurring at the site. During the sampling event, a groundwater sample was collected from water supply well WSW-6. ATC was unable to collect groundwater samples from WSW-2A or WSW-19. The onsite building associated with WSW-19 was vacant and the well did not have power. During the event, the owner of WSW-2 stated that the well pump was inactive and could not be sampled; however, public water is not available in the area. Laboratory analytical results indicated concentrations of acetone and ethyl alcohol at estimated concentrations in water supply well WSW-6. These compounds have been historically detected in WSW-6. Additionally, acetone and ethyl alcohol were also detected in the trip blank sample at similar concentrations and therefore may be laboratory artifacts. If funding allows, ATC recommends installing one vertical extent well next to MW-1 to delineate the vertical extent of the groundwater plume and assessing remedial alternatives for the site. In the interim, ATC recommends semi-annual water supply well sampling and annual sampling of select groundwater monitoring wells. Avent’s Grocery April 5, 2024 Page 4 of 9 ATC Project No. STL1279302 NCDEQ State Lead Program 3.0 SITE HISTORY AND CHARACTERIZATION 3.1 SITE CHARACTERIZATION The site consists of two properties located across the street from each other (Avent’s Grocery – Incident #12793 and Giddensville Game Room - Incident #3550 [closed]). Both properties are located at the intersection of Goshen Church Road and Giddensville Road in Giddensville, Sampson County, North Carolina. The Avent’s Grocery property is developed with a single-story building occupied by El Lucero (a restaurant). The former Avent’s Grocery UST system consisted of two 1,000-gallon gasoline USTs and one 550-gallon gasoline UST. The UST system was taken out of operation in December of 1993 when the USTs were removed from the site. Eight monitoring wells (MW-1, MW-2, MW-3R, MW-4R, MW-5, MW-6R, MW-10, and MW-11) are associated with the site. The former Giddensville Game Room UST system consisted of three 1,000- gallon gasoline USTs as well as one 500-gallon UST and one 750-gallon orphan UST of unknown contents. The Giddensville Game Room UST system was closed by removal in 1992. The Giddensville Game Room site is developed with a vacant one-story building and the property is used to store vehicles. The properties are divided by Giddensville Road to the east of the former Avent’s Grocery property and west of the former Giddensville Game Room property. The properties are each bounded by Goshen Church Road to the north, and residential properties to the north and south. A volunteer fire department is located west of former Avent’s Grocery. Surrounding properties are characterized by residential, agricultural, and commercial use. Twenty water supply wells are located within 1,000 feet of the site, seventeen of which are used for potable purposes, one which is used for irrigation purposes, and two which are inactive. One water supply well (WSW- 19) is located on the former Avent’s Grocery site and is used for drinking purposes. One water supply well (WSW-6) is located on the former Giddensville Game Room property and is used for drinking purposes. Municipal water is not available to the site or site vicinity. Water supply well ownership information is included in Table 1. A site topographic map is included as Figure 1 and a site map depicting monitoring well locations and other pertinent site features is included as Figure 2. A Potential Receptor Map showing water supply well locations is included as Figure 3. Site photographs are included as Appendix A. 3.2 SITE HISTORY 3.2.1 Avent’s Grocery Property Investigative activities at the Avent’s Grocery site were first conducted by Associated Environmental Consultants and Engineers, Inc., in December 1993 during closure of the UST system. The UST system consisted of two 1,000-gallon gasoline USTs and one 550-gallon gasoline UST. During the UST system removal, approximately 100 tons of impacted soil was removed from the site. Soil samples collected following excavation activities indicated total petroleum hydrocarbons (TPH) exceeded applicable Action Levels. Initial Site Characterization activities were conducted in June 1994. During the activities, monitoring wells MW-1, MW-2, and MW-3 were installed and sampled. Groundwater samples indicated exceedances of 2L Standards. A Comprehensive Site Assessment (CSA) was performed in 1994. During the CSA, monitoring wells MW- 4 through MW-10 were installed. A groundwater sampling event indicated exceedances of groundwater above 2L Standard. Groundwater and water supply well sampling events were performed periodically at the site from June 1994 through May 2013. In February 2015, ATC conducted a groundwater and water supply well sampling event. Results from the groundwater monitoring event indicate volatile organic compound (VOC) impact, including petroleum constituents, in monitoring wells MW-1 and MW-5 above 2L Standards. No compounds were detected above GCLs. ATC collected samples from water supply wells WSW-6, WSW-16, WSW-19, and WSW-22. Laboratory analytical results indicated concentrations of acetone below the 2L Standards in water supply Avent’s Grocery April 5, 2024 Page 5 of 9 ATC Project No. STL1279302 NCDEQ State Lead Program wells WSW-6, WSW-19, and WSW-22. No compounds were detected in the sample collected from WSW- 16. ATC submitted a Groundwater Monitoring Report on April 15, 2015, documenting a February 18, 2015, groundwater monitoring event. ATC recommended performing a geophysical survey at the site in an attempt to locate monitoring wells that could not be found during the February 2015 sampling event. In addition, ATC recommended an aggressive fluid vapor recovery event on monitoring wells MW-1 and MW-5 in order to reduce contaminant concentrations at the site. In April 2016, ATC conducted a groundwater and water supply well sampling event. Results from the groundwater monitoring event indicated VOC impact, including petroleum constituents, in monitoring wells MW-1, MW-2, and MW-10 above the 2L Standards. No compounds were detected above GCLs. ATC collected groundwater samples from water supply wells WSW-11, WSW-16, WSW-19, WSW-21, WSW-22, and WSW-23. No petroleum constituents were detected in any of the water supply well samples above the laboratory detection limits. On August 18, 2016, The John R. McAdams Company (McAdams) conducted a ground penetrating radar (GPR) survey to attempt to locate monitoring wells MW-3 through MW-9. Several possible well locations were inspected; however, only monitoring wells MW-5 and MW-7 were located. The results of the GPR survey were documented in a December 27, 2016, letter from McAdams. ATC submitted a Groundwater Monitoring Report on January 4, 2018, documenting an October 2017 groundwater monitoring event. Results from the event indicated petroleum constituents in monitoring wells MW-1, MW-2, MW-5, MW-7, and MW-10 above the 2L Standards. ATC collected samples from water supply wells WSW-11, WSW-19, and WSW-22. No petroleum constituents were detected in any of the water supply well samples above the laboratory detection limits. On April 2, 2018, ATC installed ORS sleeves in well MW-1, MW-2, and MW-5. ATC submitted a Groundwater Monitoring Report on December 6, 2018, documenting an October 2018 groundwater monitoring event. ATC collected samples from monitoring wells MW-1, MW-2, and MW-10 (MW-5 and MW-7 were not located). Results from the groundwater monitoring event indicated petroleum constituents in all sampled wells above the 2L Standards. No compounds exceeded the GCLs. The ORS sleeves installed in April 2018 were effective in reducing contaminant concentrations in well MW-2; however, the sleeves did not appear effective in MW-1. ATC also collected samples from water supply wells WSW-11 and WSW-19. No petroleum constituents were detected in any of the water supply well samples above the laboratory detection limits. On January 14, 2019, ATC supervised the installation of monitoring wells MW-3R, MW-4R, and MW-6R to replace previously destroyed wells. A subsequent limited sampling event indicated petroleum constituents in monitoring wells MW-4R, MW-5, and MW-6R above the 2L Standards. A Groundwater Monitoring Report was submitted on March 25, 2019, to document the activities. On August 6, 2019, ATC installed an additional offsite monitoring well (MW-11) in order to delineate the southern extent of the constituent plume. No constituents were detected in MW-11 exceeding their respective 2L Standards. A Groundwater Sampling Letter Report was submitted on October 15, 2019. On February 27, 2020, ATC conducted a groundwater sampling event. Results from the event indicated petroleum constituents in monitoring wells MW-1, MW-4R, MW-5, MW-10, and GMW-4R above the 2L Standards. A Groundwater Monitoring Report was submitted on April 27, 2020. On March 1, 2021, ATC conducted a groundwater sampling event. Results from the event indicated petroleum constituents in monitoring wells MW-1 and GMW-4R above the 2L Standards. A Groundwater Monitoring Report was submitted on April 13, 2021. On May 10, 2022, ATC sampled water supply wells WSW-2A and WSW-19. Based on the results, naphthalene was detected in WSW-2 at a concentration of 1.04 micrograms per liter (µg/L), below the 2L Avent’s Grocery April 5, 2024 Page 6 of 9 ATC Project No. STL1279302 NCDEQ State Lead Program Standard. Naphthalene was also detected in the laboratory blank associated with the sample, so the naphthalene detected in the water supply well sample may not be indicative of site conditions. A Groundwater Sampling Letter Report was submitted on June 1, 2022. On August 31, 2023, ATC submitted a Groundwater Sampling Letter Report documenting water supply well sampling activities conducted in June 2023. On June 22, 2023, ATC mobilized to the site sample active potable water supply wells WSW-2, WSW-6, and WSW-19. Based on the results, ethyl alcohol (16.1 micrograms per liter – µg/L) was detected in WSW-6; however, ethyl alcohol does not have a Title 15A NCAC 02L .0202 Groundwater Quality (2L) Standard, but does have a Interim Maximum Allowable Concentration (IMAC) of 4,000 (µg/L). Ethyl alcohol has not been previously detected in WSW-6. No other compounds were detected in the samples. 3.2.2 Giddensville Game Room Property The Giddensville Game Room operated as a UST retail petroleum facility from 1972 to 1980. The UST system consisted of three 1,000-gallon gasoline USTs that were operated by the current property owner’s (Mr. Norwood King) now deceased father. In 1980, Mr. King inherited the property and subsequently, Royal State Construction Company installed one 6,000-gallon diesel above ground storage tank (AST) without permission from Mr. King. In 1987, a diesel spill occurred from the AST which resulted in the removal of the AST and the excavation of impacted soil for off-site disposal. A Preliminary Contamination Assessment was performed in April 1992. During the assessment, the three USTs were gauged for product and soil samples were collected. Soil samples indicated gasoline range (GRO) TPH at concentrations exceeding Action Levels. Samples were collected from monitoring wells GMW-1, GMW-2, and GMW-3, which were installed during previous investigations. Groundwater results indicated concentrations exceeding applicable standards. Note that the wells associated with the site were formally identified as MW-1 through MW-3, but were later renamed when the site was combined Avent’s Grocery. A UST Closure Report was submitted on September 15, 1992. The report documented the closure of the three 1,000-gallon gasoline USTs. In addition, two orphan tanks were found and removed, one 500-gallon UST and one 750-gallon UST both with unknown previous contents. Following tank removal, approximately 540 tons of soils were removed from the site. Over excavation samples indicated soil exceeding GRO and diesel range (DRO) TPH concentrations above Action Levels. A receptor survey and groundwater and water supply well sampling event were performed in May and June 1997. During the sampling event, monitoring wells GMW-1 and GMW-2 were found to be destroyed. Groundwater samples were collected from monitoring wells GMW-3 and GMW-4 and four water supply wells. Petroleum compounds were not detected in any of the monitoring or water supply wells. Groundwater and water supply well sampling events were also performed in November 1997 and April 1998. On December 22, 2006, a Soil and Groundwater Assessment and Water Supply Well Sampling Report was submitted. During the assessment, monitoring wells GMW-1R through GMW-4R were installed and sampled. Results indicated all wells were impacted above 2L Standards. Soil sampling indicated soil at the site was impacted above Action Levels. In addition, a receptor survey identified numerous potable water supply wells within 1,000 feet of the site and one surface water body within 360 feet of the site. Periodic monitoring well and water supply well sampling was performed at the site between August 2009 and April 2013. ATC submitted a Groundwater Monitoring Report on April 15, 2015, documenting a February 2015 groundwater monitoring event. The laboratory analytical results indicated the presence of naphthalene at a concentration above 2L Standard in monitoring well GMW-2R. ATC recommended performing a soil assessment to determine the extent of impacted soil at the site and continued groundwater monitoring at the site. Avent’s Grocery April 5, 2024 Page 7 of 9 ATC Project No. STL1279302 NCDEQ State Lead Program ATC submitted a Groundwater Monitoring Report on July 7, 2016, documenting an April 6, 2016, groundwater monitoring event. The laboratory analytical results indicated the presence of several petroleum compounds in GMW-4R above 2L Standards. ATC recommended the installation of one off-site monitoring well south of the site, installing oxygen releasing socks in GMW-4R, and semi-annual sampling of the monitoring and surrounding water supply wells. ATC submitted a Groundwater Monitoring Report on January 4, 2018, documenting an October 27, 2017, groundwater monitoring event. Monitoring well GMW-1R could not be located and a vehicle without wheels was parked over monitoring well GMW-4R. Samples were collected from monitoring wells GMW-2R and GMW-3R. Benzene was detected in GMW-3R exceeding the 2L Standards. ATC submitted a Groundwater Sampling Letter Report on December 6, 2018, documenting an October 2018 groundwater monitoring event. During the sampling event, GMW-1R and GMW-2R were unable to be located. Laboratory analysis detected benzene concentrations in GMW-4R exceeding the 2L Standards. ATC submitted a Groundwater Sampling Letter Report on June 7, 2019, documenting an April 1, 2019, groundwater monitoring event. Monitoring wells GMW-1R and GMW-2R could not be located during the sampling event. Laboratory analysis detected several petroleum constituents in GMW-3R and GMW-4R exceeding their 2L Standards. Additionally, indeno(1,2,3-cd)pyrene was detected in GMW-3R above GCLs. Based on conversations with NCDEQ, ATC recommended closing the Giddensville Game Room incident and continuing assessment under the Avent’s Grocery incident. A Notice of Residual Petroleum was filed with the Sampson County Register of Deeds on August 5, 2019, and the incident was issued a Notice of No Further Action. ATC submitted a Groundwater Monitoring Report on January 16, 2023, documenting a December 2022 groundwater monitoring event. On December 8, 2022, groundwater samples were collected from monitoring wells MW-1, MW-10, MW-11, and GMW-4R and water supply wells WSW-6 and WSW-19. Results from the groundwater sampling event indicate petroleum constituents in monitoring wells MW-1, MW-10, and GMW- 4R exceeding applicable 2L Standards. No compounds were detected above 2L Standards in MW-11. No compounds were identified exceeding their respective GCLs. No compounds were detected in water supply wells WSW-6 or WSW-19. 4.0 GROUNDWATER ASSESSMENT ACTIVITIES 4.1 GROUNDWATER LEVELS ATC mobilized to the Avent’s Grocery and Giddensville Game Room properties on January 30, 2024, to collect groundwater samples from monitoring wells MW-1, MW-10, MW-11, and GMW-4R. Depth to groundwater measurements ranged from 1.88 feet below ground surface (bgs) in MW-1 and 2.65 feet bgs in MW-11. LNAPL was not detected at the site, nor has it been detected at the site historically. Based on the January 2024 groundwater gauging event, groundwater flows to the southwest across the sites, which is consistent with historical data. Monitoring well construction details are summarized in Table 2 and current and historical groundwater elevation data are includes in Table 3. A groundwater elevation contour map is included as Figure 4. Field notes from the groundwater sampling event are included as Appendix B. 4.2 GROUNDWATER QUALITY On January 30, 2024, groundwater samples were collected from monitoring wells MW-1, MW-10, MW-11, and GMW-4R. The groundwater samples were placed in laboratory-supplied containers, entered into chain-of- custody control, placed on ice, and shipped to SGS North America Inc. (SGS) in Lafayette, Louisiana, for analysis. Groundwater samples were analyzed for VOCs by Standard Method 6200B. Avent’s Grocery April 5, 2024 Page 8 of 9 ATC Project No. STL1279302 NCDEQ State Lead Program The laboratory analytical results detected the following compounds at concentrations above the 2L Standards:  MW-1: benzene (3.3 µg/L);  MW-10: benzene (5.8 µg/L), ethylbenzene 604 µg/L), total xylenes (782 µg/L), naphthalene (114 µg/L); and  GMW-4R: benzene (12.5 µg/L), naphthalene (72 µg/L) No compounds were detected above the GCLs. No compounds were detected above 2L Standards in well MW-11. Current groundwater analytical data is summarized in Table 4. Historical groundwater analytical data is summarized in Table 5. Field parameters collected during the sampling event are shown in Table 6. Dissolved benzene and naphthalene isoconcentration contour maps are included as Figure 5 and Figure 6, respectively. The laboratory analytical report and chain-of-custody forms are included in Appendix C. ATC utilized GSI Mann-Kendall Toolkits to determine concentration trends for wells MW-1, MW-10, and GMW-4R. Concentrations for well MW-1 indicate a decreasing trend for benzene, no trend for toluene, ethylbenzene, and total xylenes, and a stable trend for naphthalene. Concentrations in well MW-10 indicate a decreasing trend for benzene and no trend for naphthalene. Concentrations in well GWM-4R indicate increasing trends for ethylbenzene, total xylenes, and naphthalene, a stable trend for benzene, and a decreasing trend for toluene. GSI Mann-Kendall Toolkits are included as Appendix D. As part of ATC’s quality assurance and quality control (QA and QC) measures, a data review of the laboratory report was conducted. During the review, it was concluded that samples were received by the laboratory in acceptable condition and properly preserved. The samples were analyzed within the correct holding times, and the chain of custody was properly completed by all parties. The laboratory QC report shows an internal blank was analyzed for each method that was used for the original samples. The laboratory fortified blank (LFB) and laboratory control sample (LCS) data results were also provided for each analytical method. Refer to the laboratory report in Appendix C for details regarding QA results for the laboratory samples and any noted exceptions. Overall, ATC concludes that the laboratory results are usable and any exceptions do not significantly impact the conclusions presented in this report. 4.3 WATER SUPPLY WELLS Twenty water supply wells are located within 1,000 feet of the site. On January 30, 2024, ATC collected a groundwater sample from water supply well WSW-6. ATC attempted to sample wells WSW-2A and WSW- 19 during the event. However, the owner of WSW-2A stated that the well is currently inactive. Additionally, based on field observations, the site occupant may have vacated the property and shutdown electricity to the on-site water supply well. Samples were collected in laboratory-approved containers, packed in ice, and delivered under chain-of-custody protocol to SGS to analyze for VOCs by Method 6200B. Acetone and ethyl alcohol were detected at estimated concentrations in water supply well WSW-6. These compounds have been historically detected in WSW-6 and may be laboratory artifacts. The QA/QC measures and conditions applicable to the monitoring well sampling also applies to the water supply well sampling. Table 7 presents the current and historical water supply well analytical results. The laboratory analytical report is included in Appendix C. 5.0 CONCLUSIONS AND RECOMMENDATIONS On behalf of the Trust Fund Branch of the UST Section, ATC has completed a Groundwater Monitoring Report documenting a sampling event performed in January 2024 for the Avent’s Grocery and Giddensville Game Room properties, located the intersection of Giddensville Road and Goshen Church Road in Giddensville, Sampson County, North Carolina. On January 30, 2024, groundwater samples were collected from monitoring wells MW-1, MW-10, MW-11, and GMW-4R. Results from the groundwater sampling event indicate petroleum constituents in monitoring wells MW-1, MW-10, and GMW-4R above the 2L Standards. No compounds were detected above 2L Standards in MW-11. No compounds were identified exceeding the GCLs. Compounds for the wells are generally stable, decreasing, or have no discernable trend, with the exception of ethylbenzene, total xylenes, and Avent’s Grocery April 5, 2024 Page 9 of 9 ATC Project No. STL1279302 NCDEQ State Lead Program naphthalene in well GWM-4R, which indicate an increasing trend. As a result, it appears natural attenuation may not be occurring at the site. During the sampling event, a groundwater sample was collected from water supply well WSW-6. ATC was unable to collect groundwater samples from WSW-2A or WSW-19. The onsite building associated with WSW-19 was vacant and the well did not have power. During the event, the owner of WSW-2 stated that the well pump was inactive and could not be sampled; however, public water is not available in the area. Laboratory analytical results indicated concentrations of acetone and ethyl alcohol at estimated concentrations in water supply well WSW-6. These compounds have been historically detected in WSW-6. Additionally, acetone and ethyl alcohol were also detected in the trip blank sample at similar concentrations and therefore may be laboratory artifacts. If funding allows, ATC recommends installing one vertical extent well next to MW-1 to delineate the vertical extent of the groundwater plume and assessing remedial alternatives for the site. In the interim, ATC recommends semi-annual water supply well sampling and annual sampling of select groundwater monitoring wells.
