Dear Mr. Osman, CATLIN Engineers and Scientists (CATLIN) is pleased to provide you with this letter report documenting field activities performed in general accordance with CATLIN proposal number 220279.03P, prepared February 5, 2024. Services were authorized by the North Carolina Environmental Quality (NCDEQ) Task Authorization (TA) #03, accepted February 5, 2024, and Contract Number N70522-A. The approved scope of services for this Task Authorization included:
• Update the Site Receptor Survey by determining the use of previously identified wells.
For any wells previously listed as inactive, determine if there is an occupied residence.
Additionally, verify the existence/lack of municipal water available to the site.
• If it is determined WSW-1 and WSW-7 are no longer in use, and don’t intend to be used,
send access agreements to the property owners to abandon the two wells.
• Determine the dimensions of WSW-1 and WSW-7, if possible; and
• Prepare a letter report documenting the findings.
The Callicutt Grocery site is located at 709 Abner Road, Star, Montgomery County, North Carolina (Figures 1 and 2 in Attachment A). CATLIN conducted field activities on February 27, 2024 in general accordance with the approved TA. Potential hazards were evaluated before beginning
field work and are documented on the Daily Activities Log, which is provided in Attachment B. Recent site photographs are presented in Attachment C.

Receptor Information
An updated field survey was conducted on February 27, 2024, wherein CATLIN personnel walked a 500-foot (or greater) radius from the site and performed a windshield survey of all properties located 500 to 1,500 feet from the site. Additionally, Request for Water Supply Well Information Survey letters were sent to all property owners within 1,000 feet of the site. Information obtained from the field survey and letter responses was compiled
with water supply well information provided by S&ME (2020). According to conversations with residents, and verified by Montgomery County personnel, municipal water is not available to the site vicinity. Ten water supply wells have been identified within 1,500 feet of the site. Within 500 feet of the site, two active potable wells
and two inactive non-potable wells were identified. The two inactive non-potable wells are WSW-1 and WSW-7, and information from one resident would suggest that they will not be returned to active potable use. Six additional active potable wells are located between 500 to 1,500 feet from the site.
One surface water body was located within 1,500 feet of the site. A pond which was created by damming an intermittent stream is located 500 feet east of the subject site. CATLIN personnel were able to determine the approximate dimensions of WSW-1, while access to the interior of WSW-7 was not feasible due to a very heavy concrete cover.
Both wells are of similar concrete construction, being 20-inches inner diameter with a 20-inch stickup above the land surface. The total depth of WSW-1 was approximately 39 feet below the top of the concrete casing and the depth to water was 21-feet 7-inches. It is assumed that WSW-7 will have a similar depth.
Potential receptors are illustrated on Figure 2 and receptor information is shown in Table 1 in Attachment D. Communication with Property Owners CATLIN sent certified letters to the property owners of WSW-1 and WSW-7 to request their permission to abandon the wells. As of the date of this report, CATLIN has not received
responses. While on site on February 27, 2024, CATLIN attempted to speak with the property owner for WSW-1 but was unable to locate the owner at the address. Water supply well WSW-7 is located adjacent to the driveway of the house that uses WSW-2 and the renter (who is a relative of the owner) stated that WSW-7 didn’t produce water and WSW-2 was subsequently installed. Her statement was that she didn’t think it would be used in the future. In addition, based on the property owner of WSW-3, her husband, brother-in-law, and sister-in-law (who lived at parcels
with WSW-3, WSW-9 and WSW-10) have all passed away from the same form of cancer. Her son is now living in one of the houses with his children and she indicated that she would like her wells sampled, if possible.

Recommendations

CATLIN recommends to:
• Continue to try to pursue abandonment of WSW-1 and WSW-7; and
• Complete a round of groundwater sampling, including water supply wells WSW-2, WSW-3,
WSW-4, WSW-9, and WSW-10.
