PURPOSE OF SITE VISIT: Follow-up Inspection STATUS OF PAST NOTED VIOLATIONS: A. 15A NCAC 13B .0405 (a)(10), "Windblown waste: Site staff shall conduct daily inspection for windblown waste on the site property. Windblow litter from the site operations discovered during the daily inspections or observed on adjacent properties shall be picked up and containerized for disposal by the end of the operating day, unless the landowner of the adjacent property denies access to site staff." Corrective Actions in Progress: During this inspection, windblown waste was actively being picked up around the transfer station building. The windblown waste that was previously identified in the perimeter fencing had been picked UP. OBSERVED VIOLATIONS: None. Page 1 of 4 NORTH CARnLINAD_E Q�� o.n.mm.m m c.w.mmen.i a.i� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS On April 26, 2024, Mr. Jacob Parker contacted Ms. Thompson via phone to report an incident that had occurred the day prior, April 25, 2024. Mr. Parker stated that a tire had blown on the yard dog and the transfer trailer had tipped over with the waste spilling onto the ground. Mr. Parker stated that the driver had just left the scale and was making a U-turn in the road when the tire blew. The weight in the trailer shifted which caused the trailer to tip over and spill the MSW in the ground. The driver was transported to the hospital for treatment. Mr. Parker also stated that numerous local emergency agencies were on -site to assist with the emergency response. A local large wrecker company assisted with the up righting of the truck and trailer. The MSW waste was cleaned up and placed in a different trailer prior to the staff leaving at the end of the day. During the conversation, Mr. Parker also stated that other items discussed during the February 2024 inspection had been addressed. Ms. Thompson stated that she was in the area and would stop by the facility to view the repairs and to check on the driver. Mr. Parker stated that the driver was still in the hospital at the time and would let me know when he was able go home to his family. Mr. Parker stated that the yard dog started back up approximately one hour after being righted by the wrecker. The yard dog was moving trailers around the facility during the inspection. 1. This facility has a Life of Site Permit that was issued December 21, 2023. Facility is required to submit an annual facility report on or before August 1 annually. 2. This facility is permitted to receive Municipal Solid Waste as defined in NCGS 130A — 290 (a)(18a). 3. This facility is secured by fences with locking gates. 4. Signs are posted on the fence before the scales with all required information. 5. The permit, operations plan, and waste screenings were not reviewed during this inspection. 6. The interior roadways are made of all-weather construction and well maintained. 7. Windblown waste was observed adjacent to the transfer trailer loading bay. Windblown waste was actively being picked up during this inspection. Ensure that the windblown waste has been removed from the facility prior to the end of the operating day. 8. Mr. Parker sent Ms. Thompson a photograph of the metal repairs that have been made to the damaged metal above the transfer trailer loading bay on April 29, 2024. Additional repairs or replacements to the transfer station would be addressed in the redesign. 9. The high-water light for the oil -water separator was still on at the time of this inspection. Mr. Parker stated that an electrician had inspected the system and had identified a short in the light. The parts to repair the system had been ordered. Mr. Parker also stated that the electrician had also installed an alarm to identify staff of high levels in the oil -water separator system. Mr. Parker also stated that the sewer department had come out and utilized a camera system to identify where the system actually flows to. The leachate is collected in the oil -water separator, pumped up and into the manhole in the transfer trailer loading bay ramp, which is connected to the sewer system. 10. Staining was observed during the February 2024 inspection, on the concrete retaining wall and in the riprap from the outfall of drains from the apron. The drains from the apron have now been piped down the wall and across the grassy area to the storm drain, which is connected to the sewer system. 11.
