Box 1375 Reidsville, NC 27323 Subject: Pagetown Exxon, 8035 Cherry Grove Rd., Reidsville, Caswell County, Incident Number: 44302, Risk Classification: High, Ranking: 365 Dear Madam or Sir, Thank you for submitting Groundwater Monitoring Report, dated July 30, 2019. The report was approved on December 4, 2019 with the condition that the following additional information be included in the next submitted monitoring report. 1.) Please analyze for the oxygenates for the next sampling event. 2.) Page I: Under “Release Information” include the dispenser island as a source of release in addition to the kerosene UST. After the Size and Contents line, modify to include the gas dispenser island. 3.) Site figure: a. Include wsw designations to correlate with table B-5. b. Include the locations of the other off-site and on-site potential sources. c. Include property boundaries. d. Include all present and former USTs, ASTs, lines and dispensers. Label tanks to correlate with table B-1. Note: A second figure, including only the site, may be provided to include some of the requested information if too much information on Figure 2 will make it illegible. 4.) Page 4: Written permission is not required from a current property owner to do a notice of residual petroleum. Please modify. 5.) Table B-1: a. Include the incident number rows as they appear in Table B-1 in Guidelines for Assessment and Corrective Action. 6.) Table B-2: a. Please update to include the new owner. 7.) Table B-4: a. Please make a note, on the table, about the name change of wsw-14 to wsw-15. 8.) Table B-5: a. Provide a column that includes the wsw name that goes with each map ID. b. Include the date that the last receptor survey/updated receptor survey was done at the top of the table. 9.) Table B-5 and Figure 4 have wsw’s located on both property #14 and #15. However, Table B-4 indicates only a wsw (wsw-15) on property 15. Do the wsw ID numbers correlate with the property numbers? Please modify the tables and figures to clarify. 10.) Please confirm the location of the cross-sections on Figure 10. 11.) Table B-7: a. Include historical information. 12.) It is indicated, in the text a few times in the report, that contaminants were detected in water supply wells. Please indicate the actions that were taken after each sampling event when contaminants, both above and below the action levels, were detected in wsw’s (health-risk evaluations and recommendations, alternate water provided and date provided, POE systems and date provided, property hooked to a different wsw, etc.) 13.) It is noted, on page 10, that the user of wsw-15 was being provided bottled water prior to her death. Please double check whether her home is connected to wsw-15 or if her home was hooked up to her son’s wsw. a. If her home was connected to her son’s wsw, please determine if her home is still connected to her son’s wsw. b. If her home is presently not connected to her son’s wsw, please determine if a new occupant is using wsw-15. 14.) Include the location of the automotive repair facility on figure 14.
