(S&ME) submits this Summary of Environmental Consulting Services to document the closure activities of a presumed non -regulated, non-commercial, former home heating oil underground storage tank (UST), located at 1642 Garden Terrace, Charlotte, North Carolina. Services were performed in general accordance with the Consulting Services Agreement dated May 15, 2014, established between Carolinas HealthCare System (now Atrium Health) and S&ME. Project Background On August 9, 2022, Mr. Luke Pokrajac with Atrium Health contacted Mr. Brian Wilson with S&ME, via text message and informed him that a presumed former home heating oil UST had been discovered at a former residential location on Garden Terrace by grading contractors while working on a proposed roadway project for Atrium Health (Atrium). Additional information provided by Mr. Pokrajac to Mr. Wilson indicated that the former UST had been removed by the grading contractor and was staged on -site adjacent to the former excavation. S&ME was requested by Atrium to schedule a site visit to document as -found conditions and advise Atrium of potential North Carolina Department of Environmental Quality (NCDEQ) regulatory requirements. Summary of Field Activities Initial Assessment S&ME personnel, as requested by Mr. Pokrajac, visited the site on August 10, 2022, met with Atrium and Rodgers Builders personnel, documented as -found site conditions, and performed a limited assessment of existing site conditions. The presumed home heating oil tank appeared to be an approximate 550-gallon capacity tank. The S&ME, Inc. 12016 Ayrsley Town Boulevard, Suite 2-13 1 Charlotte, NC 28273 1 p 704.523.4726 1 www.smeinc.com Summary of Limited Environmental Assessment Atrium Health — Garden Terrace Fuel Tank Assessment 1560 Garden Terrace, Charlotte, Mecklenburg County, North Carolina S&ME Project No. 22350597, Ph 01 tank had been removed from the ground, was staged adjacent to the former excavation, and was observed to have large holes in the top caused by the excavator and small rust holes on the end approximately 6 — 8 inches from the bottom. Tank contents, consisting of liquid with a petroleum odor and debris (soil, sticks, etc.) were observed within the tank. The former excavation had been partially backfilled by the grading contractor upon tank removal. At the request of Mr. Pokrajac, the grading contractor removed the lose soil fill placed in the former UST excavation to expose the approximate excavation sidewalls and bottom. No visible staining was observed in the soil sidewalls or from the bottom of the excavation, however, a slight petroleum odor was noted. At the request of Mr. Pokrajac, two soil samples were collected from the presumed bottom of the former UST excavation near the approximate ends of the former tank. The soil samples were collected in labeled zip top plastic bags and placed in a cooler of ice. The soil samples were transported by S&ME personnel to Pace Analytical Laboratories in Huntersville, North Carolina, transferred into laboratory provided sample containers and submitted to be analyzed for Total Petroleum Hydrocarbons, Gasoline Range Organics (TPH-GRO) using EPA Method 8015/5035, and Total Petroleum Hydrocarbons, Diesel Range Organics (TPH-DRO) using EPA Method 8015/3550 with a "Rush" 24-hour turnaround time requested for laboratory analytical results. Atrium personnel requested S&ME coordinate the disposal of the tank contents and the tank. S&ME contacted CCI Environmental Services (CCI) and scheduled these services. For additional information, please refer to the attached Environmental Field Report, dated August 10, 2022, and site photos presented in the Photo Log included in the Attachments. Analytical Results Analytical results were received by S&ME on August 11, 2022 and indicate a TPH-DRO concentration of 20.0 milligrams per kilogram (mg/kg) for soil sample SS#1. TPH-GRO constituents for soil sample SS#1 were not detected above laboratory reporting limits. Analytical results for soil sample SS#2 indicate a TPH-GRO concentration of 158 mg/kg and a TPH-GRO concentration of 10.8 mg/kg. TPH-DRO analytical results for soil sample SS#2 exceed the NCDEQ action level for regulatory requirements of 100 mg/kg. No other analyzed constituents exceed NCDEQ action levels. Laboratory analytical reports are included in the Attachments. Material Disposal On August 19, 2022, CCI Environmental Services (CCI) visited the subject site and pumped approximately 45- gallons of petroleum impacted liquid and mud from the former UST. The liquid and mud were transported by CCI to their facility in Thomasville, North Carolina for disposal. Additionally, the empty tank was loaded onto a trailer and transported by CCI to their facility to be cleaned and recycled. The tank was recycled by D. H. Griffin Wrecking Company in Greensboro, North Carolina. Discussion As requested during a conference call between Atrium and S&ME on August 16, 2022, S&ME contacted Ms. Trudy Beverly, Incident Manager, NCDEQ, Division of Waste Management, UST Section, to discuss the findings and requested NCDEQ recommendations regarding the UST closure. Since the orphan UST is classified as a non- commercial, non -regulated UST, no visible soil staining was observed, no evidence of potential groundwater impacts, minor petroleum soil contamination is present based on laboratory analysis, and no known water supply wells are located within 150 feet of the former UST location, Ms. Beverly stated that the incident should qualify for a Low -Risk classification. Additionally, future property use by Atrium will be a new asphalt paved roadway with no residential structures or groundwater usage. Based on our understanding of NCDEQ Guidelines, this incident may be eligible for closure by filing a Notice of Residual Petroleum (NORP). Atrium requested S&ME collect soil samples from beneath the former UST. Two samples were collected, placed in zip top bags and placed in a cooler of ice.
