ADDITIONAL COMMENTS
1. All photographs were taken by Summer Justice during the inspection. 2. The site was secured with a gate to prevent unauthorized access. 3. Traffic markers were posted around the site for traffic control.
4. Access roads at the facility were of all-weather construction and maintained. 5. Durable, weather-resistant signs that stated the operating hours, permit number, and that no hazardous or liquid waste is accepted were posted at the entrance to the facility.

8. Tonnage records from the 2022-2023 Facility Annual Report indicated that the facility received 7,505 tons of waste. Facility scale records from July 1, 2023- January 31, 2024 were available for review and indicated that the facility received 5,150.17 tons of waste for the period.
9. Waste screening forms were available for review for the period of July 5, 2023-March 28, 2024. The forms showed the final disposition of unacceptable waste that was encountered. 10. Semi-annual groundwater monitoring reports were available for review in Laserfiche.
11. Landfill gas monitoring reports for the period of March 2023-February 2024 were sent via email on April 1, 2024. No exceedances were observed. 12. According to facility staff and the approved operational plan, the facility is not currently accepting asbestos.
13. The facility was not able to provide cover logs for review. According to 15A NCAC 13B .0542(n)(F), notation of date and time of placement of cover material shall be kept in the operating record.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any

14. The facility is currently filling in Phase 4. At the time of inspection, Phase 1A appeared to have appropriate intermediate cover and established vegetation. Phase 2A was developing vegetation, however, small rills were observed in the intermediate cover, and some waste which appeared to have been tracked by equipment was
present. Continue to monitor erosion of intermediate cover, repair rills, and remove tracked waste as necessary. 15. The working face was small, compact, and no unapproved waste was observed.
16. The observed ground water monitoring wells were locked and affixed with identification tags. 17. Windblown litter was very well maintained at the facility. The working face is equipped with netting to prevent windblown litter.
18. The approved operations plan part 8. Access and Safety Requirements states that “Access roads will be watered periodically, as needed, to minimize dust.” However, facility staff stated that the site is not equipped with a water truck. While dust was not observed at the time of inspection, ensure that the facility is equipped with the necessary machinery to remain in compliance. 19. Standing water was observed in the southeast corner of Phase 4 where waste has not been placed. According to 15A NCAC 13B .0542(l)(2) and 15A NCAC 13B .0542(l)(3), Surface water shall not be impounded over or in waste, and solid waste shall not be disposed of in water. If changes to the approved landfill design are necessary to keep water from collecting in the cell, please discuss these changes with Solid Waste Permitting prior to implementation. Ensure that the collection of standing water is resolved prior to waste placement in that area.
