Landfill operations began in Phase 1 on this property. Facility Permit 51-03 is a combination of facility permits 51-01 and 51-02. Permit to Construct Johnston County MSW Landfill, Phase 6 issued May 8, 2020. Permit to Operate 5103-MSWLF Phase 4A and 5103-CDLF Area 2 Cells 1 and 2 issued May 8, 2020. Permit for Closure for MSW Phases 1 through 4 and C&D Area 1 issued May 8, 2020. Life of Site Permit for 5103-MSWLF and 5103-CDLF issued May 8, 2020 and will expire August 8, 2057. Permit to Construct Johnston County MSW Landfill Phase 6 issued November 30, 2021. Permit to Operate 5103-MSWLF Phase 4A, 5, and 6A and C&D Area 2, Cells 1 and 2 issued November 30, 2021. Page 1 of 5 NORTH CARnLINAD_E Q�� OepaNnent W Environnnnbl puel� FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. ADDITIONAL COMMENTS Facility Inspection 1. The facility is fenced and is secured by a locking gate when it is not open for operation. 2. The correct signage is at the entrance of the facility listing the facility name, permit number and emergency contact information as well as materials accepted at the facility. 3. The facility accepts waste Monday through Saturday from 8:00 am until 4:45 pm and closed on Sunday. 4. The permit, operations plan, waste screening, and training records were not reviewed during this inspection. 5. The perimeter roadways around the active MSWLF and closed CDLF are being properly maintained. 6. Permanent edge of waste (EOW) markers were observed around the active portion of the MSWLF and closed CDLF. Yard Waste 7. The yard waste has been processed and moved to a staging area near Cell 6 since the last inspection. The yard waste in the western portion of the yard waste area has been windrowed. 8. No areas of standing water were observed in the western portion of the yard waste area. Continue to monitor and repair the yard waste area as needed. 9. Mr. Beasley stated that a load of unacceptable waste was dumped adjacent to the pallet stockpile. Mr. Beasley stated that the unacceptable wastes were removed and taken to the working face as soon as they were identified. Mr. Beasley also stated that conversations would be had with the attendants regarding the unacceptable waste in the yard waste area. Continue to remove and properly dispose of unacceptable identified within the yard waste area. Phase 4A MSWLF 10. Numerous large deep erosion rills were observed throughout the Phase 4A closure. Mr. Beasley stated that the closure contractor would repair the rills and reseed the areas when they returned the following week to finish the closure. Ms. Thompson asked Mr. Beasley to send her photographs of the repairs following their completion. Ensure that the erosion rills have been properly repaired and seeded. Continue to monitor and repair as needed. 11. Vegetation has begun growing on Phase 4A closure. Continue to repair and reseed areas which need additional vegetative cover. 12. Mr. Beasley also stated that the closure contractor would be placing an artificial turf material in a corner drainage area to help with erosion in that area. 13. The stormwater basins located around Phase 4A have been cleaned, sloped, and vegetated with grass. No woody vegetation was observed. Continue to ensure that the stormwater basins are mown and maintained in working order. Phase 5 MSWLF 14. The slopes of Phase 5 have been covered and seeded. Grasses have begun to grow on the eastern side slope. Once the final grade has been achieved on the western side slope, the area will be seeded. 15. Additional cover was being placed on the western side slope during this inspection. 16. Windblown waste was actively being picked up during this inspection. Continue to ensure that the windblown waste is picked up daily. The working face of Phase 6 was small and compact. The working face was actively being covered during this inspection. Cover soil was also stockpiled adjacent to the working face. 18. The northern portion of Phase 6 is almost even with the northern portion of Phase 5. The facility is in the process of tying into Phase 5.
