PURPOSE OF SITE VISIT: Partial Inspection STATUS OF PAST NOTED VIOLATIONS: NONE OBSERVED VIOLATIONS: NONE The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section Page 2 of 3 ADDITIONAL COMMENTS 1. This facility is a Wood Waste Treatment & Processing Facility, Permit No. 6511-TP. Facility is permitted to accept vegetative land clearing debris, root mat, stumps, limbs, tree trunks, soil, and shrubbery. 2. The facility grinds land clearing debris for use as boiler fuel and mulch. 3. Facility records were not reviewed at this inspection. 4. The operating hours are Monday through Friday 8:00 AM - 5:00 PM. 5. Signs posted indicating hours of operation and waste exclusions. 6. No offsite sedimentation was observed. 7. An interior roadway has been made of all-weather construction and is being maintained in good condition. 8. There is at least a 25 foot access path around the site for fire control vehicles. 9. All setbacks are in place. 10. No standing water was observed during the inspection 11. Digital pictures taken during inspection.
