with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an

or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

1) 15A NCAC 13B .0835 (a)(1): The tank is constructed of metal and is affixed to the vehicle as required.
2) 15A NCAC 13B .0835 (a)(2): Valves appeared to be in working order and were closed at the time of
inspection.
3) 15A NCAC 13B .0835 (a)(3): Ports were closed and sealed at the time of inspection.
4) 15A NCAC 13B .0835(a): No leaks were noted during the inspection.
5) 15A NCAC 13B .0835 (b): The company name, phone number and town name was at least three (3)
inches in height as required. At the time of inspection, the permit number was under two (2) inches in
height. Mr. Goodwin had the permit number fixed and sent pictures to Ms. Williams following the
inspection. The lettering was permanently affixed as required.
