STATUS OF PERMIT:
A Permit and Registration was issued to V&S Septic Service, Inc. to operate a Septage Management Firm on January
12, 2013.
PURPOSE OF SITE VISIT:
Comprehensive Inspection
STATUS OF PAST NOTED VIOLATIONS:
15A North Carolina Administrative Code 13B .0844(b) states: All permitted septage management firms shall
display decals or lettering on each side of every pumper vehicle operated by the firm. The decals or lettering shall
include the name, address (town name), phone number, and septage management firm permit number. All decals or
lettering on the pumper vehicle shall be no less than three inches in height and plainly visible. All decals or lettering
shall be permanently attached (i.e., no removable signs)
Inspections of the pumper vehicles performed on February 2, 2013 and February 20, 2013 found that the majority of
lettering on each vehicle did not meet the minimum height requirement of 3-inches. Therefore, V&S Septic Service,
Inc. was found in violation of 15A NCAC 13B .0844(b).
Inspection of the pumper vehicles on April 4, 2013 found that the lettering on each vehicle had been replaced with
lettering 3-inches in height meeting the requirements of 15A NCAC 13B .0844(b). Therefore, this violation is
resolved. Thank you for your cooperation in this matter.

ADDITIONAL COMMENTS
1. The purpose of this inspection was to perform a follow-up inspection to determine efforts to comply with the
Notice of Deficiency that was issued to V&S Septage Service, Inc. on February 25, 2013. Please refer to “Status
of Past Noted Violations” for comments.
2. The Septage Firm name shown on the approved permit and application is V&S Septic Service, Inc. However, all
trucks inspected did not have “Inc.” included within the firm name affixed to the pumper vehicles. Therefore,
within 60 days complete one of the following:
a. Submit a revised permit application with the firm name shown as it is currently affixed to the pumper
vehicles, or
b. Provide decals or lettering on all pumper vehicles to display exactly what is shown on the approved
2013 permit and application.

with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.
