STATUS OF PERMIT:
A Permit and Registration was issued to V&S Septic Service, Inc. to operate a Septage Management Firm on January
12, 2013.
PURPOSE OF SITE VISIT:

OBSERVED VIOLATIONS
15A North Carolina Administrative Code 13B .0844(b) states: All permitted septage management firms shall
display decals or lettering on each side of every pumper vehicle operated by the firm. The decals or lettering shall
include the name, address (town name), phone number, and septage management firm permit number. All decals or
lettering on the pumper vehicle shall be no less than three inches in height and plainly visible. All decals or lettering
shall be permanently attached (i.e., no removable signs).
Inspection of the pumper vehicles found that the majority of lettering on each vehicle did not meet the minimum
height requirement of 3-inches. Therefore, V&S Septic Service, Inc. is violation of 15A NCAC 13B .0844(b).

To achieve compliance, within 30 days of receipt of the Notice of Deficiency, V&S Septic Service, Inc. must replace
all decals or lettering below the applicable height limit with decals or lettering that satisfies the height requirement as
described in 15A NCAC 13B .0844(b).
ADDITIONAL COMMENTS
1. The purpose of this inspection was to complete a comprehensive review of all fleet trucks and records.
2. All tanks appeared to be structurally sound and adequately secured to the frame.
3. Valves and access ports were secure and appeared to be in proper working order. Caps were present on the access
ports to prevent leaking
4. During the inspection event on February 7, 2013, it appeared that the vehicle with the license tag #: YA104039
had a leak originating from the main seal of the septage tank. It was raining at the time of the inspection.
However, no leak was observed during the inspection performed on February 20, 2013. Continue to monitor this
seal. If a leak is indeed discovered, the subject vehicle must be taken out of service until the necessary repairs are
made.
The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

5. According to the approved 2013 permit application, the following vehicles were approved for operation.
