Be advised that pursuant to N.C.G.S. 130A-22, an administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit, or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules. Page 1 of 3 NORTH CARnLINAD_E Q�� o.n.mm.m m c.wro..w.t.l a.l� ADDITIONAL COMMENTS Pumper vehicle(s) inspected: FACILITY COMPLIANCE INSPECTION REPORT Division of Waste Management Solid Waste Section LICENSE PLATE VIN # TANK CAPACITY (gal) TS-5091 1GBM7D1Y1EV117898 1,500 Comments: N/A 1) 15A NCAC 13B .0835 (a)(1): The tank is constructed of metal and is affixed to the vehicle as required. 2) 15A NCAC 13B .0835 (a)(2): Valves appeared to be in working order and were closed at the time of inspection. 3) 15A NCAC 13B .0835 (a)(3): Ports were closed and sealed at the time of inspection. 4) 15A NCAC 13B .0835(a): No leaks were noted during the inspection. 5) 15A NCAC 13B .0835 (b): All lettering was at least three (3) inches in height and listed the company name, town name, phone number, and permit number as required. The lettering was permanently affixed as required. The lettering does not stand out enough in contrast to the background. We discussed ways to make the lettering more apparent. Mr. Motsinger had the lettering fixed following the inspection and sent photos to Ms. Williams. Please see photos for details. 6) Additional comments: At the time of inspection, the permit fee had not been paid for the second truck. Mr. Motsinger sent the fee following the inspection and it was received on February 29th by Division of Waste Management.
