The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) This facility consists of eight above ground steel tanks —one 4,000-gallon, five 8,000-gallon, and two
10,000-gallon — for a total storage capacity of 64,000 gallons. The facility is permitted for the storage of
domestic septage and grease septage.
2) 15A NCAC 13B .0838(b): The access road is well maintained for all weather conditions.
3) 15A NCAC 13B .0838(c): No holes or cracks which may allow the escape of septage were observed in
tanks that were in use at the time of inspection.
4) 15A NCAC 13B .0838(e)(2) and 15A NCAC 13B .0838(g): No vector or odor issues were noted at the time
of inspection.
5) 15A NCAC 13B .0838(i): Some vegetation was observed within 5-feet of the tanks. Please keep the area
within 5-feet of the tanks clear at all times.
6) 15A NCAC 13B .0838(k): Appropriate signage was noted at the site.
7) 15A NCAC 13B .0838(m): Setback requirements appear to have been met.
