The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

ADDITIONAL COMMENTS
1) This facility consists of one above ground 10,000 gallon tank, four above ground 3,000 gallon tanks and
two buried 900 gallon tanks for the total capacity of 23,800 gallons for the storage of domestic septage.
Currently this facility is only being utilized for septage detention and the Division shall be notified and
approval granted before any treatment process is implemented. At the time of inspection, Mr. Blankenship
stated that tank three (3) is the only tank currently in use.
2) 15A NCAC 13B .0838(b): An all-weather access road is maintained.
3) 15A NCAC 13B .0838(c): No holes or cracks which may allow the escape of septage were noted.
4) 15A NCAC 13B .0838(e)(2) and 15A NCAC 13B .0838(g): No vector or odor issues were noted.
5) 15A NCAC 13B .0838(i): No debris or vegetation was noted within 5 feet of the tank that is currently in
use. Some debris was noted around the 10,000 gallon tank that is not currently in use. Ms. Williams and Mr.
Blankenship discussed the debris and that it will need to be moved if the tank is put into service again.
6) 15A NCAC 13B .0838(k): Proper signage was posted on site.
7) 15A NCAC 13B .0838(m): Setback requirements appear to have been met.
