The item(s) listed above were observed by Section staff and require action on behalf of the facility in order to come into or maintain compliance
with the Statutes, Rules, and/or other regulatory requirements applicable to this facility. Be advised that pursuant to N.C.G.S. 130A-22, an
administrative penalty of up to $15,000 per day may be assessed for each violation of the Solid Waste Laws, Regulations, Conditions of a Permit,
or Order under Article 9 of Chapter 130A of the N.C. General Statutes. Further, the facility and/or all responsible parties may also be subject to
enforcement actions including penalties, injunction from operation of a solid waste management facility or a solid waste collection service and
any such further relief as may be necessary to achieve compliance with the North Carolina Solid Waste Management Act and Rules.

2) 15A NCAC 13B .0835 (a)(1): The tank was affixed to the vehicle as required.
3) 15A NCAC 13B .0835 (a)(2): Valves appeared to be in working order and were closed. One valve was
missing a cover at the time of inspection. Mr. Tatum added a cover and sent photos to Ms. Williams
following the inspection.
4) 15A NCAC 13B .0835 (a)(3): Ports were closed and sealed at the time of inspection.
5) 15A NCAC 13B .0835(a): No leaks were noted during the inspection.
6) 15A NCAC 13B .0835 (b): All lettering was at least three (3) inches in height and listed the company
name, town name, phone number, and permit number as required. The company name on the vehicle
reads "Erik's Grading and Septic," whereas the application reads "Erik's Grading & Septic." A note has
been made on the application in our records regarding the discrepancy.
7) The tank was full at the time of inspection. Mr. Tatum was able to arrange disposal with the WWTP and
he worked with permitting to have the permit issued so he could dump the waste that was in the tank.
8) Truck was in compliance with the Septage Rules.
