(Terracon) is pleased to submit this letter report for limited soil sampling activities completed at the Shamrock Nursing Center, located at 2727 Shamrock Drive in Charlotte, North Carolina (site), at the request of Mr. Tommy Medlin with Care Group NC (Client). Terracon conducted the limited soil sampling in general accordance with Terracon Proposal No. P71237558, dated January 2, 2024. Terracon mobilized to the site on January 3, 2024, to collect one soil sample, S-913. At the direction of the Client, sample S-913 was collected in the vicinity of soil samples S-9 and S-9A, collected during prior site activities by other consultants. Terracon advanced a decontaminated hand auger to approximately 6 inches below ground surface and collected the sample from the same interval. The soil sample was placed directly into laboratory -supplied containers, labeled, placed on ice in, and transferred under standard chain -of -custody to Pace Analytical in Huntersville, North Carolina for analysis of Extractable Petroleum Hydrocarbon (EPH) by Massachusetts Department of Environmental Protection (MADEP). Soil analytical results were compared to the North Carolina Department of Environmental Quality (NCDEQ) Maximum Soil Contaminant Concentrations (MSCC). According to the laboratory analytical report, C19-C36 Aliphatics were detected in soil sample S-913 at a concentration of 16 milligrams per kilogram (mg/kg), below the Residential MSCC of 31,200 mg/kg. The new sample results from the S-9 area (below residential with no soil-to-gw standard for C19-C36) qualify the site for closure. The report is missing the required seal from a licensed geologist or professional engineer. A pdf of the sealed report can be emailed when available. Have a great day, Trudy D. Ms. Beverly — I left a voicemail a few moments ago. On behalf of our client, Care Group NC, I am providing you with the attached letter report documenting the results generated from a recent soil sampling event at the subject site. Our client would like to see about getting regulatory closure for this incident; we believe this request is merited based on the results. If you don't mind reviewing the attached and giving me a call to discuss, I would very much appreciate it. Thank you in advance! George K.
