Site Map Appendices Appendix A: Risk Characterization Form Appendix B: Soil Boring Log Appendix C: Laboratory Analytical Reports and Chain-of-Custody Documentation Introduction This report presents the results of a Limited Site Assessment (LSA) conducted for the closure-in-place of one 20,000-gallon heating oil underground storage tank (UST) at Blue Ridge Regional Hospital located at 125 Hospital Drive in Spruce Pine, Mitchell County, North Carolina. This LSA was required by a Notice of Regulatory Requirements (NORR) from the North Carolina Department of Environment Quality (NCDEQ). The location of the site is indicated on a USGS topographic map in Figure 1. Figure 2 presents a site map that shows the location of the soil boring and monitoring well. A 7,000-gallon hydraulic UST is located immediately to the south of the heating oil UST. A closure-in-place permit was issued by NCDEQ on April 19, 2023. On April 20, 2023, the old UST was filled with flowable concrete fill by Petroleum Specialty, Inc. of Canton, North Carolina. According to Petroleum Services, Inc., the UST was already emptied of all contents. The location of the UST on the property and the soil and groundwater sampling locations are shown in Figure 2. The UST Closure soil analytical results are summarized in Table 1. Groundwater analytical results are summarized in Table 2. Analytical results detected elevated DRO concentrations in one soil sample (SB-9) and the groundwater concentrations were below the NCAC 2L groundwater standards. Risk Characterization The four-page risk characterization form provided in the Guidelines for Assessment and Corrective Action; North Carolina Underground Storage Tank Section, April 2001 (guidelines) has been completed, and is contained in Appendix A. Additionally, the risk classification criteria outlined on pages 4-4 through 4-6 of the guidelines has been reviewed in-regards to this site. Based on these risk classification guidelines, the property meets the criteria for low risk. Receptor Information Singleton Environmental, Inc. personnel completed a water supply well survey near the site in July 2023, by searching surrounding properties for wells, water meter connections, and contacting Spruce Pine water department officials. No water supply wells were located within 1,500 feet of the site. The following bullets summarize other receptor information regarding this site: • Public water supply is available in the vicinity of the site from municipal water supplies. • The site is not located in a wellhead protection area. • No water supply wells are located within 1,500 feet from the source area. • The North Tow River is located 1,130 feet to the west and downgradient from the site. The property containing the release is zoned as a commercial. The surrounding properties contain residential, commercial, and industrial. Site Geology and Hydrogeology According to the Geologic Map of North Carolina (1985), the site is located within the Grandfather Mountain Window and Alligator Back Formation. The bedrock underlying the site is gneiss. A soil boring log is included in Appendix B. On June 29, 2023, one monitoring well (MW-1) was installed to a depth of 20 feet in the location of soil sample SB-9. The depth to groundwater in MW-1 is 16.85 ft-bgs. Groundwater flow is assumed to flow to the west from the source area toward the North Toe River. Sampling Results Soil - LSA On May 1, 2023, Singleton Environmental, Inc. (SEI) mobilized to the site with a GeoProbe to collect soil and groundwater samples around the closed-in-place 20k UST. Ten soil samples (SB-1 through SB-10) were collected along three sides of the closed-in-place UST. The eastern side was not sampled due to the location of the building. The soil borings were advanced to approximately 15 feet in depth. The soil samples were obtained at a depth of approximately 8 to 10 ft-bls due to the presence of groundwater. The samples were collected by advancing decontaminated soil tubes with new clear PVC liners at each soil boring. The soils in each boring were screened with a Photoionization Detector (PID). Soil borings SB-2 through SB-8 did not detect elevated PID readings, however, soil borings SB-1, SB-9 and SB-10 detected elevated PID readings at depths ranging from 8 to 10 ft-bls. All samples were immediately placed on ice and submitted for laboratory analysis of Gasoline Range Organics (GRO) and Diesel Range Organics (DRO). Soil analytical results and sample depths are summarized in Table 1 and corresponding soil sampling locations can be found in Figure 2. Analytical results detected a DRO concentration of 3,260 mg/kg in soil sample SB-9. The remaining soil samples were non-detect for GRO and DRO concentrations. On June 29, 2023, SEI mobilized to the site with a Geoprobe Drill Rig to install MW-1 in the vicinity of former soil sample SB-9 at a depth of 8 ft-bls. The sample was collected by advancing soil tubes with a new clear PVC liner at the location illustrated in Figure 2. Once the soil sample was collected, it was submitted to Pace Analytical to be analyzed for EPA Method 8260. • MW-1-Soil - detected concentrations of p-isopropyltoluene above the soil-to-groundwater MSCCs. None of the detected compounds exceeded the residential or industrial MSCCs. The soil analytical results are summarized in Table 3. Groundwater - LSA A UST closure groundwater sample was collected in the SB-1 location due to the highest PID field readings. The UST closure groundwater sample was collected and analyzed for EPA Method 6200B. Groundwater analytical results did not detect any concentrations above the NCAC 2L Groundwater Standards. The UST closure groundwater analytical results are located in Table 2. On June 30, 2023, monitoring well MW-1 was sampled. The well was purged of three well volumes and sampled for analysis using EPA Method 6200B and 625. • MW-1 detected a concentration of naphthalene above the NCAC 2L MCLs. The concentration did not exceed the NCAC 2L GCLs. The LSA groundwater analytical results are summarized in Table 4. Conclusions and Recommendations Based on the results of this investigation at the Blue Ridge Hospital the following items have been determined: • The soil at the site exceeds the soil-to-groundwater MSCCs; however, it does not exceed the residential or commercial MSCCs standards. • Only naphthalene exceeds the NCAC 2L MCLs in MW-1. The GCLs were not exceeded. • There are no water supply wells located within 1,500 feet of the release area. • The North Tow River is located approximately 1,130 feet to the west. • The results of the receptor survey indicate the site should be classified as low risk. SEI recommends no-further-action at the site with a groundwater notice of residual petroleum attached to the property deed.
