These activities have been conducted following the release of petroleum which occurred in the vicinity of a cracked underground FRP product return line for a diesel generator underground storage tank (UST). All activities were conducted in accordance with NCDEQ guidelines and the requirements of 15A NCAC 2N. Mr. Neal, if you have questions regarding this report please contact my office. Free Product: Free product was observed at this site in monitoring well MW-2 at a maximum thickness of 0.01’. Free product removal via hand baling and absorbent socks has apparently resulted in remediation. Free product was not present in MW-2 during the latest 2 sampling event conducted in 2023. 2. Contaminant Mass: Contaminant mass calculations cannot be completed at this time since the vertical and horizontal extent of contamination has not yet been defined. 3. Remedial Action Plan: No CSA or CAP activities have been conducted for this site. The NCDEQ has approved for CSA activities to be delayed until after UST closure and new AST installation activities are completed at this release site. To date, free product activities including hand baling and introduction of oil absorbent socks into monitoring well MW-2 have resulted in apparent remediation of oil floating atop the shallow water table, which was noted in this well at a maximum thickness of 0.01’. It is suspected that the free product was a result of contaminant migration through the MW-2 borehole during installation. 7. Impacted Receptors: (6) potable and (2) irrigation water supply wells are located within 1000’ of the release source at this facility. No supply wells located within 250’ of the release source. Supply well SW-1 (backup hospital water supply) and SW-6 (Buckberry Falls community well) were sampled in conjunction with groundwater monitoring activities and analyzed via Method 6200B last on July 11, 2023. Results for these 2 receptors was below laboratory detection limits for all petroleum related 6200B constituents on that date. Figure 1 illustrates the location of this facility on the Highlands Quadrangle U.S.G.S. Topo Map. This facility contains multiple permanent structures which are used for medical hospital operations. Figure 4 illustrates the overall layout of this site and the UST location. Information regarding the UST in use at this facility is contained in Table 1. A release from the UST system was confirmed upon inventory reconciliation on 8/18/2022 which indicated an estimated loss of 3,400 gallons of fuel. Initial abatement action activities have been conducted pursuant to the release confirmed by excavation of product piping which revealed petroleum product beneath a cracked FRP elbow on the generator return line. Municipal water supply is available to the medical center and some properties within 1,500’ of the release source at this facility; however, most nearby property owners are not connected to the municipal water supply and are served by private wells. Figure 2 illustrates a receptor map, with well owner information summarized on Table 5. Abatement activities conducted at the site were reported to the NCDEQ in an Initial Abatement Action report dated November 16, 2022. A Phase II LSA report was prepared and submitted to the NCDEQ for this site on December 13, 2022. 1.2 Scope of Services Contained herein are the results of a groundwater sampling event as approved by the NCDEQ. This Groundwater Monitoring Report contains documentation concerning the following activities which have been conducted at this facility: o Sampling of new and existing monitoring wells conducted by Henry Nemargut Engineering on July 11, 2023. o Laboratory analyses of groundwater samples conducted by Pace Analytical, Inc. of Huntersville, NC. Page - 2 2.0 Presentation of Current Site Assessment Information / Comparison to Historical Assessment Information 2.1 Historical Soil Assessment Information The soils at the project site consist of silt, clay, sand & weathered rock of varying proportions. Soil boring logs are contained on well construction records and boring logs contains in the previously submitted IAA and LSA reports prepared for this site. On August 29-30, 2022, Henry Nemargut Engineering and Ransier Environmental Drilling mobilized drilling equipment to the site to conduct a free product check and assess conditions of soil and groundwater in the vicinity of the UST system. On August 30, 2022, UST area monitoring well (B1/MW-1) was advanced to assess subsurface conditions adjacent to the pea-gravel filled UST basin and piping source area soil boring/monitoring well (P1/MW-2) was advanced at the piping leak location. Soil samples were collected on August 30, 2022 during advancement of the borings B1/MW-1 and P1/MW-2 at 5’ to 10’ intervals to check for any vertical migration of release liquids which could have migrated through the pea gravel filled basin. The UST area boring was advanced to an approximate depth of 78’ below ground surface where competent bedrock was encountered. A separate monitoring well was installed in the piping leak source area to a depth of 65’ to assess for free product/groundwater impact (MW-2). Following installation, groundwater was measured at an approximate depth of 52.02’ below top of casing in MW-1 and at a depth of 47.62’ below top of casing in MW-2. Additional soil borings were advanced along the product piping trench to assess for horizontal migration of release liquids on August 31, 2022. One sample was obtained from pea gravel where the product line enters the tank top STP sump since some liquids accumulated in that sump. Figure 3/3A illustrate the locations of monitoring wells and soil borings installed during site check activities. To confirm conditions of the UST site, samples collected from in-situ soils during advancement of the monitoring wells and piping trench borings were submitted to Pace Analytical, Inc. for analyses according to EPA Methods TPH-GRO/DRO as recommended by the NCDEQ for diesel UST sites. An additional sample was collected from the P1/MW-2 boring for risk-based analyses via Methods 8260/8270/VPH/EPH as recommended for diesel release sites. All soil and water samples collected were placed in laboratory supplied glassware, labeled with sample location, analysis to be performed, time, date and sampler's name. The sample jars were then immediately placed in a cooler, chilled with ice to approximately 4oC in preparation of transportation to an analytical laboratory utilizing EPA approved chain-of- custody procedures. Results from TPH soil analyses indicate soils above NCDEQ Action Levels in the P1 and P2 piping sample locations. No soil contaminants were detected above laboratory detection limits in any other sample locations at the site. Results from the risk-based soil sample collected from the P1/MW-2 boring were above the current Lowest MSCCs for (8) compounds/compound groups. No compounds were detected exceeding Residential of Commercial/Industrial Standards during the IAA investigation. Table 2/2A summarize the laboratory results for the TPH/Risk-Based soil samples from 190 Hospital Page - 3 Drive in Highlands, NC. Copies of the soil analytical report and the chain of custody records for the TPH and risk-based soil samples collected at 190 Hospital Drive are contained in the previously submitted IAA report. 2.2 Ground Water Flow Direction and Gradient In order to estimate the groundwater flow direction in the vicinity of the petroleum release, elevations of the monitor well casings were measured using surveying equipment. During the July 11, 2023 sampling event, static water levels were obtained from the monitor wells (see Table 3) and the data was used to compute a potentiometric surface map. Figure 6 illustrates the current potentiometric surface map as calculated based on the measurements obtained on July 11, 2023. Groundwater data and monitoring well information were also used to estimate the hydraulic gradient at the project site. Using the horizontal distance between the monitor wells and the static water levels, the groundwater was calculated to flow toward the west with a hydraulic gradient of approximately 0.0969 ft/ft. 2.3 Monitor Well Sampling and Analyses On July 11, 2023, monitor wells MW-1 through MW-6 were sampled for analyses utilizing Standard Method 6200B and MADEP VPH. Before collecting the samples, the monitor wells were purged by removing a minimum of three well volumes to ensure that the samples were representative of the actual groundwater conditions. The wells were developed and sampled using a 12 volt submersible pump. All samples were placed in laboratory supplied glassware, labeled with sample location, analysis to be performed, time, date and sampler's name. The sample jars were then immediately placed in a cooler, chilled with ice to approximately 4oC in preparation of transportation to an analytical laboratory utilizing EPA approved chain-of-custody procedures. Laboratory analytical results indicate the presence of benzene above its current listed 2L Standard in monitor well MW-1 at a concentration of 1.2 ug/l (2L Standard = 1.0 ug/l). Total 6200B compounds were detected only in MW-1 at a concentration of 26.4 ug/l. No other 6200B or VPH compounds were present in any other wells at the site. Table 4 summarizes the laboratory analytical results, and Appendix A contains a copy of the laboratory analytical report and chain of custody records. Figure 7 contains a benzene iso-concentration map, and Figure 8 contains a Total 6200B concentration map. 2.4 Supply Well Sampling and Analyses No water supply wells were sampled during this sampling event. Page - 4 2.5 Regional / Site Geology and Hydrogeology The site is situated in the eastern Blue Ridge Belt. According to the Geological Map of North Carolina local geology of the region is underlain with quartz diorite to granodiorite Weathered bedrock was encountered at depths ranging from 5 to 40 feet below surface during subsurface investigations at this site. The partial Highlands Quadrangle U.S.G.S. Topographic Map included as Figure 1 indicates that the release site is located on a slight slope of a knoll at an approximate elevation of 3967’ amsl. Surface waters in the vicinity of the release area generally drain northwest towards the Big Creek located 510’ north at its closest distance to the piping release source area. Big Creek feeds Lake Sequoyah which lies within the Cullasaja River basin. 2.6 Plume Development The monitoring well network at this release site was installed between August 30, 2022 and October 25, 2022. In the initial sampling events conducted at the site, 2L violations for benzene, naphthalene, and all MADEP class compounds or the presence of floating product were noted in monitor well MW-2. The MW-2 maximum free product thickness was 0.01’. During the last 2 sampling events conducted on April 4, 2023 and July 11, 2023 no 2L violations were noted in MW-2. It is suspected that the free product was a result of contaminant migration through the MW-2 borehole during installation which how now been purged with continued development of this well during sampling events. 6200B compounds were present in MW-1 for the first time on April 4, 2023, and have continued to increase in concentration during the July 11, 2023 sampling event. Benzene is now present in MW-1 at a concentration above its 2L Standard, which suggests that dissolved contaminants are migrating downgradient and 2L violations now extend horizontally beyond the currently installed monitoring well network. Since the vertical and horizontal extent of soil and groundwater contamination has not been defined at this site, accurate comparisons cannot be made concerning the total contaminant mass or seasonal fluctuations at this site at this time; however, dissolved constituent concentrations are no longer present in piping release source area monitoring well MW-2 but were present in downgradient well MW-1, suggesting that migration is occurring. 3.0 Monitored Natural Attenuation 3.1 Natural Source Zone Depletion Status Historically, 2L violations for 6 compounds/compound groups or free product have been observed in MW-2 at this site. No free product or dissolved compounds were noted in MW-2 during the latest monitoring event, which suggests that well development and oil absorption efforts have been successful in free product remediation of MW-2. Page - 5 4.0 Conclusions 4.1 General Summary A Groundwater Monitoring event at Highlands-Cashiers Medical Center located at 190 Hospital Drive in Highlands, NC has been completed. From a review of all information gathered during this monitoring project, Henry Nemargut Engineering makes the following conclusions: o During the latest sampling event conducted on the July 11, 2023, laboratory analytical results indicate the presence of benzene above its current listed 2L Standard in monitor well MW-1 at a concentration of 1.2 ug/l (2L Standard = 1.0 ug/l). No other 6200B or VPH compounds were present in any other wells at the site. o During the initial sampling events conducted at the site, 2L violations for benzene, naphthalene, and all MADEP class compounds or the presence of floating product were noted in monitor well MW-2. The MW-2 maximum free product thickness was 0.01’. During the last 2 sampling events conducted on April 4, 2023 and July 11, 2023 no 2L violations were noted in MW-2. Benzene is now present in MW-1 at a concentration above its 2L Standard, which suggests that dissolved contaminants are migrating downgradient and 2L violations now extend horizontally beyond the currently installed monitoring well network. o Since the vertical and horizontal extent of soil and groundwater contamination has not been defined at this site, accurate comparisons cannot be made concerning the total contaminant mass or seasonal fluctuations at this site at this time. 4.2 Recommendations Based on information obtained during this sampling event, Henry Nemargut Engineering Services makes the following recommendations for this site: o (2) additional shallow monitoring wells should be installed to define the horizontal extent of 2L violations at this site in the approximate locations depicted in Figure 8. o Additional sampling should be conducted at this site to confirm contaminant levels in monitoring and local supply wells on a quarterly basis until the UST at this facility is properly closed and a comprehensive site assessment (CSA) can be completed at this site. 4.3 Limitations Page - 6 This report has been prepared for the exclusive use of MH Highlands-Cashiers Medical Center, LLP for the specific application to the referenced site located in Macon County, North Carolina. The assessment was conducted based on the scope of work and level of effort desired by the client. Our findings have been developed in accordance with generally accepted standards in the practice of Groundwater Monitoring Reports in the State of North Carolina, available information, and our professional judgment. No other warranty is expressed or implied. 5.0 Statements and Certification 5.1 Statements The monitoring report was due on August 15, 2023.
