EXECUTIVE SUMMARY
As required, the following executive summary provides the most pertinent information for this assessment.
For additions information please refer to subsequent sections of this report.
• CES of NC, PLLC (CES) was retained by GPM Southeast, LLC (GPM) to complete a groundwater monitoring event in December 2019. The activities were requested in a “Notice of Regulatory Requirements” dated February 12, 2019 issued by the North Carolina Department of Environmental Quality (NCDEQ). The services were provided in general accordance with the NCDEQ
“Guidelines for Assessment and Corrective Action for UST Releases” and approved task authorization 28327-29.
• Historical site assessments include a Limited Site Assessment, Comprehensive Site Assessment and multiple groundwater monitoring events. Most recently, a monitoring event completed in December 2018 indicated groundwater contamination in excess of groundwater quality standards in monitoring wells MW-3R and MW-6.
• CES personnel mobilized to the site on December 4, 2019 for completion of a comprehensive groundwater monitoring event. Monitoring wells MW-1, MW-1D, MW-2, MW-3R and MW-6 were gauged and sampled for volatile organic compounds including oxygenates by Standard Methods 6200B and for Lead using Method 6010B. Water supply wells WSW-1, WSW-3 and WSW-4 were also sampled.
• Groundwater samples collected from site monitoring wells indicated groundwater impacts in excess of groundwater quality standards, however no detections were reported above GCLs. Free product was not encountered in site monitoring wells. Groundwater samples collected from the water supply wells indicated no contaminant impacts above groundwater quality standards.
• Shallow groundwater flow direction was interpreted to trend over all to the north-west.
• In conclusion, CES recommends a groundwater monitoring event to include geochemical parameters for natural attenuation. Results of the monitoring event should be utilized to develop a
groundwater fate and transport model to predict any potential impacts to receptors. If the modeling indicates favorable conditions under natural attenuation, the incident should be considered for closure.

1.0 BACKGROUND INFORMATION
The following provides background information pertaining to the site location, characteristics, and history regarding assessment for the release from the former UST system.
1.1 SITE LOCATION
The GPM 3089 facility (Former Scotchman #89), herein referred to as the site, is located at 3045 Tynecastle Highway, Banner Elk, Avery County, North Carolina. Figure 1 is a topographic vicinity map of the surrounding area and Figure 2 is a site map depicting pertinent site features. The property is owned by the
Norwood Family Limited Partnership and currently operates as Sugar Creek Gem Mine. Historically the site was owned and operated by Worsley Companies, Inc. as a retail service station from 1983 until 2005. Worsley operated five underground storage tanks (UST) including: three 6,000-gallon gasoline USTs, one 4,000-gallon kerosene UST, and one 4,000-gallon diesel UST; the USTs were removed in 2005. The history of the former underground storage tank (UST) systems is summarized in Table 1.
1.2 SITE HISTORY
Detailed site history has been provided in previous reports submitted to the NCDEQ. During UST closure assessment activities in June 2005, it was determined that a gasoline release had occurred. Consequently, the NCDEQ requested that a Phase I Limited Site Assessment (LSA) be performed for the site. LSA
activities were initiated by Geological Resources, Inc. (GRI) of Charlotte, North Carolina, on July 11, 2007, with the installation of one source area monitoring well (MW-1). The LSA indicated the presence of petroleum compounds above the North Carolina groundwater quality standards as defined in North Carolina
Administrative Code Title 15A Chapter 2L .0202 (2L Standards). On February 18, 2008, Best Geologic and Environmental Consulting, P.A. (Best) advanced five soil probes to define the extent of impacted soil. Best installed four new 2-inch diameter Type II monitoring wells (MW-2, MW-3, MW-4, and MW-5) on March 13 and 14, 2008, to complete the horizontal delineation of the petroleum compounds in groundwater. One Type III groundwater monitoring well (MW-1D) was installed to determine the vertical extent of impacted groundwater. One additional monitoring well (MW-6) was installed by Best on June 5, 2008 to further define the extent of impacted groundwater. Laboratory analyses of the groundwater sample collected from MW-6 indicated that the horizontal delineation of groundwater impact had been completed. On June 16, 2010, Best installed a 2-inch Type II monitoring well (MW-3R) to replace MW-3 which was damaged beyond repair during parking lot improvement activities.
In addition to the installation of MW-3R, the manways for monitoring wells MW-1 and MW-5 were replaced. A total of seven groundwater monitoring wells have been installed at the site to assess the release.
Most recently, CES personnel performed a groundwater monitoring event in December 2018. Groundwater samples collected from site monitoring wells indicated groundwater impacts in excess of groundwater quality standards, however no detections were reported above GCLs. Free product was not encountered in
site monitoring wells. Groundwater samples collected from the water supply wells indicated no contaminant impacts above groundwater quality standards.
2.0 POTENTIAL RECEPTOR INFORMATION
CES evaluated receptors based on historically available information and current site conditions as discussed below.

2.1 WATER SUPPLY AND SURFACE WATER As part of the Comprehensive Site Assessment (CSA) reported August 18, 2008, Best updated the receptor survey by walking the area within 500 feet of the subject site to identify any water supply wells. In addition, Best mailed a Water Supply Well Survey to each property owner within 500 feet of the source area. Thirty-
one water supply wells have been identified within 1,500 feet of the source area, and 21 water supply wells have been identified within 1,000 feet of the source area. Two water wells have been identified within 250 feet of the source area. These wells are used as sources of potable water but are situated up gradient of the
source area. The locations of the identified water supply wells are shown on Figure 5, and the owner information for the sampled wells WSW-1, WSW-3, and WSW-4 is included in Table 2.
Carolina Water of North Carolina (Carolina Water) provides potable water to the subject site and the area west and northwest of the subject site. Water service is not available east of the subject site or south along Tynecastle Highway. Carolina Water obtains water from 23 fractured bedrock wells. Carolina Water operates a community well on behalf of the Ski Country development. This well is located 785 feet northwest of the source area. According to the North Carolina Division of Water Quality Wellhead Protection website, the source area is not located within a designated wellhead protection area as defined in 42 USC 300h-7(e) and is not
located in the Coastal Plain physiographic region. CES reviewed a United States Geological Survey (USGS) 7.5 Minute Quadrangle Topographic Map
(Figure 1) to identify possible surface water bodies in the vicinity of the site. Two surface water bodies are present within 500 feet of the source area. The nearest surface water body is an unnamed tributary of the Elk River located approximately 25 feet north of the source area. A pond is located 150 feet northeast of the source area. The Elk River is classified as B and C waters and designated as public trout waters.
2.2 DESCRIPTION OF SUBSURFACE GEOLOGY As reported by previous consultants, the subject site is located along the northern margin of the Grandfather Mountain Window in the Blue Ridge Tectonic province. The site is underlain by Grandfather Mountain Formation. The site is located immediately south of a perennial stream near its confluence with the Elk River. The site is subject to flooding and was severely impacted in 2004 when the remnants of a hurricane passed through the area.
The source area is covered by asphalt pavement and slopes from east to west. The grade of the site appears to have been raised by the placement of fill material. Fill material described as poorly sorted boulders,
cobbles, gravel, wood within a matrix of clayey sandy silt was encountered from the ground surface to depths ranging from eight to 15 feet below the ground surface. Beneath the fill material, alluvial soils consisting of cobbles, gravel and sandy silt were encountered. Partially weathered rock was encountered at
a depth of 28 feet and competent rock was encountered at a depth of 40 feet during the drilling of vertical extent well MW-1D. The groundwater flow appears to be influenced by the poorly sorted fill material and the proximity to the stream. The hydraulic gradient at the source area was estimated at 0.093 feet per foot, while the hydraulic gradient on the western part of the site was 0.125 feet per foot (as reported in the August 18, 2008 CSA). On March 14, 2008, slug tests were performed on wells MW-2 and MW-4 to estimate the hydraulic conductivity (K) of the regolith aquifer. Prior to the start of the test, the static water level of each well was measured. A slug of water was removed using a new disposable bailer. The recovery of the well water over time was recorded. The estimated hydraulic conductivity and the hydraulic gradient were used to estimate

an average linear groundwater velocity of 4.92 feet per year at the source area and 60.23 feet per foot on the western part of the site.
2.3 SUBSURFACE STRUCTURES An underground natural gas line is present immediately north and northwest of the source area. A storm
water culvert is situated 60 feet west of the source area. It does not appear that these structures have influenced contaminant migration.
2.4 SURROUNDING LAND USE The subject site is zoned for highway business. Land use for the surrounding area is primarily commercial along Tynecastle Highway. The majority of the land to the north and east is single-family residential. The land to the west across Tynecastle Highway is part of the Sugar Mountain ski area. The nearest residence is a single-family dwelling located approximately 160 feet east of the source area. The nearest public gathering place is the Banner Elk Church of Christ located 550 feet west of the source area. 3.0 RECENT ACTIVITIES
CES performed a groundwater monitoring event in accordance with the NCDEQ approved pre-approval
task authorization (PATA) 28327-29. A copy of the approved PATA form is included in Appendix A.
3.1 GROUNDWATER SAMPLING On December 4, 2019 CES collected groundwater samples from monitoring wells MW-1, MW-1D, MW-2, MW-3R and MW-6, and from water supply wells WSW-1, WSW-3 and WSW-4. Please note that during the December 2019 sampling event MW-1D was misidentified as MW-5 on the well sampling form and in the analytical report. Monitoring wells MW-4 and MW-5 were unable to be located and believed to be under a gem mining exhibit on the site.
The monitoring wells were located, accessed and gauged for depth to water and total well depth through the use of an electronic oil-water interface probe after allowing the water levels in the wells time to equilibrate at atmospheric pressure. At this time, free product was not detected at the site. A summary of
well construction and recent gauging data is included in Table 3. Following gauging, each monitoring well was purged of at least three well volumes, or until dry, using dedicated disposable bailers. To reduce the potential of cross contamination, new, clean, disposable nitrile gloves were worn during all phases of sample handling. Groundwater sampling logs indicating purge
volumes, sample times, and physical parameters recorded at the time of sampling by a calibrated groundwater quality multi-parameter meter are included in Appendix B. Samples were placed into laboratory supplied containers pre-filled with appropriate preservatives and labeled with indelible ink. Following collection, samples were immediately placed on wet ice in laboratory supplied coolers and transported to Pace Analytical Services, LLC (Pace) located in Huntersville, NC, a North Carolina certified laboratory, for chemical analysis. The samples were analyzed for volatile organic compounds (VOCs) including ethanol and the five oxygenates by Standard Methods 6200B and for Lead using Method 3030C.
3.2 GROUNDWATER OCCURRENCE AND FLOW DIRECTION Depth to groundwater ranged from 6.22 (MW-6) to 13.52 (MW-2) feet below top of casing (TOC) during
the groundwater sampling event completed on December 4, 2019. Liquid phase hydrocarbons (LPH) were not were detected during the December 4, 2019 sampling event. Table 3 presents historical groundwater elevation data. Figure 3 presents a Groundwater Elevation Contour Map constructed using data from the

December 2019 sampling event. According to the data, groundwater flows to the northwest at the subject site.
3.3 LABORATORY RESULTS
Benzene was detected above the NCAC 2L Standard of 1 ug/L in MW-3R (13.1 ug/L), MTBE was detected above the NCAC 2L Standard of 20 ug/L in MW-3R (25.6 ug/L), Tert-Butyl Alcohol (TBA) was detected above the NCAC 2L Standard of 10 ug/L in MW-3R (53.1 ug/L) and Lead was detected above the NCAC 2L Standard of 15 ug/L in MW-1, MW-2, MW-3R and W-6 with a maximum concentration of 286 ug/L in MW-1. None of the analytes tested for were detected above NCAC 2L Standards in WSW-1, WSW-3 or WSW-4. Free product was not detected during the December 4, 2019 groundwater monitoring event. Analytical results are provided in Table 4 and presented on Figure 4. Chain-of-custody documentation and laboratory data reports area included as Appendix C. 4.0 CONCLUSIONS AND RECOMMENDATIONS
4.1 CONCLUSIONS The following conclusions are provided based upon the information and data collected during the December
2019 sampling event and evaluated by CES;
• Free product was not detected during the December 4, 2019 groundwater monitoring event.
• Benzene, MTBE and TBA were detected above the NCAC 2L Standards in MW-3R and Lead was detected above the NCAC 2L Standards in MW-1, MW-2, MW-3R and MW-6.
• Detections of Lead may be attributed to turbidity in the samples and/or elevated natural background concentrations.
• None of the analytes tested for were detected above NCAC 2L Standards in water supply wells WSW-1, WSW-3 or WSW-4.
• CES was unable to locate MW-4, and MW-5 during the December 4, 2019 groundwater monitoring event.
4.2 RECOMMENDATIONS Based on the data collected during groundwater monitoring activities, CES recommends a comprehensive groundwater monitoring event with inclusion of geochemical parameters for natural attenuation from select monitoring wells. Due to the lack of impact at receptors to date and the apparent stability of the plume, results from the monitoring event should be utilized in groundwater fate and transport modeling to determine any potential future impact to receptors under natural attenuation as a path to incident closure. Groundwater samples should be collected from monitoring wells MW-1, MW-1D, MW-2, MW-3R, MW-
4, MW-5, MW-6, and from water supply wells WSW-1, WSW-3, and WSW-4 for analysis of lead and EPA Method 602 compounds including benzene, toluene, ethylbenzene, total xylenes, methyl tert-butyl ether, diisopropyl ether, and naphthalene. Additionally, a surface water sample should be collected from the creek
downgradient of the source area and analyzed as above. CES will make reasonable efforts to locate and sample MW-4 and MW-5. Lead was detected above the NCAC 2L Standard in MW-1, MW-2, MW-3R and MW-6 during the December 2019 sampling event. The June 2013 Groundwater Monitoring Report submitted by Best (dated July 29, 2013) suggests lead
exceedances detected at the site may be naturally occurring and stated they do not correspond to their understanding of the gasoline plume. Further testing for metals should be conducted via low flow sampling

techniques to determine if the lead exceedances are due to turbidity and to evaluate concentrations of lead suspended in and moving within the aquifer. 5.0 REMARKS
The recommendations contained in this report represent CES of NC, PLLC’s professional opinions based
upon the currently available information and are arrived at in accordance with currently accepted professional standards. This report is based upon a specific scope of work requested by the client. The contract between CES of NC, PLLC and its client outlines the scope of work, and only those tasks
specifically authorized by that contract or outlined in this report were performed.
