1.0 INTRODUCTION AND BACKGROUND INFORMATION
Antea USA of North Carolina, Inc. (Antea USA of NC) is pleased to present the following Groundwater Monitoring
Report for GPM Southeast, LLC. (GPM) site 3089 located at 3045 Tynecastle Highway, Banner Elk, Avery County,
North Carolina (the site). This report was prepared on behalf of GPM and documents the results of the January
2016 and June 2016 groundwater monitoring event.
1.1 Site History
The property is owned by the Norwood Family Limited Partnership and currently operates as a winter ski retail
store and gem mining store (Figure 2). The site is approximately 18.02‐acres and is located at the intersection of
Arnett Road and North Carolina Highway 184 (Tynecastle Highway) in Banner Elk, North Carolina. Historically the
site was owned and operated by Worsley Companies, Inc. as a retail service station from 1983 until 2005. Worsley
operated five underground storage tanks (UST) including: three 6,000‐gallon gasoline USTs, one 4,000‐gallon
kerosene UST, and one 4,000‐gallon diesel UST.
During UST closure assessment activities it was determined that a gasoline release had occurred. Consequently,
the UST Section requested that a Phase I Limited Site Assessment (LSA) be performed for the site. LSA activities
were initiated by Geological Resources, Inc. (GRI) of Charlotte, North Carolina, on July 11, 2007, with the
installation of one source area monitoring well (MW‐1). The LSA indicated the presence of petroleum compounds
above the North Carolina groundwater quality standards as defined in North Carolina Administrative Code Title
15A Chapter 2L .0202 (2L Standards). On February 18, 2008, Best Geologic and Environmental Consulting, P.A.
(Best) advanced five soil probes to define the extent of impacted soil. Best installed four new 2‐inch Type II
monitoring wells (MW‐2, MW‐3, MW‐4, and MW‐5) on March 13 and 14, 2008, to complete the horizontal
delineation of the petroleum compounds in groundwater. One Type III groundwater monitoring well (MW‐1D) was
installed to determine the vertical extent of impacted groundwater. One additional monitoring well (MW‐6) was
installed by Best on June 5, 2008 to further define the extent of impacted groundwater. Laboratory analyses of the
groundwater sample collected from MW‐6 indicated that the horizontal delineation of groundwater impact had
been completed. On June 16, 2010, Best installed a 2‐inch Type II monitoring well (MW‐3R) to replace MW‐3 which
was damaged beyond repair during parking lot improvement activities. In addition to the installation of MW‐3R,
the manways for monitoring wells MW‐1 and MW‐5 were replaced. A total of seven groundwater monitoring wells
have been installed at the site to assess the release.

2.0 FIELD ACTIVITIES
The following tasks were completed during the January 26, 2016 and June 22, 2016 site visits:
On January 26, 2016, depth‐to‐groundwater measurements and groundwater samples were obtained
from monitoring wells MW‐1, MW‐1D, MW‐2, MW‐3R, MW‐5, and MW‐6 and groundwater samples were
also obtained from water supply well WSW‐4.
On June 22, 2016, groundwater samples were collected from water supply wells WSW‐1 and WSW‐3.
Groundwater monitoring well MW‐4 was not located during the December 8, 2014 and January 26, 2016 sampling
events and is believed to be buried beneath a sluice that was constructed for gem mining operations. In January
2016 water supply wells WSW‐1 and WSW‐3 could not be located. A site visit was conducted in June 2016 and
wells WSW‐1 and WSW‐3 were located and sampled. A water supply well table has been provided as Table 1. For
reference purposes, a Site Location Map and a Site Map are included as Figures 1 and 2, respectively.
3.0 GROUNDWATER ELEVATIONS
On January 26, 2016, an Antea USA of NC representative conducted gauging activities at the subject site. Depth to
groundwater measurements were collected from monitoring wells MW‐1, MW‐1D, MW‐2, MW‐3R, MW‐5, and
MW‐6 using a decontaminated electronic oil/water interface meter with an accuracy of 0.01 feet. Free product
was not detected in any of the wells gauged on January 26, 2016. Disposable nitrile gloves were worn and changed
between measurements. Groundwater elevation data is summarized in Table 2 and groundwater sampling records
are included in Appendix A.
Groundwater flow direction in the shallow aquifer was estimated by developing a groundwater contour map based
on water table elevation data obtained from the wells on January 26, 2016 (Figure 3). Interpretation of the water
table contour map suggests that the shallow water table across the Site flows generally toward the west.
4.0 GROUNDWATER SAMPLING
On January 26, 2016, an Antea USA of NC representative collected groundwater samples from monitoring wells
MW‐1, MW‐1D, MW‐2, MW‐3R, MW‐5, MW‐6, and from water supply well WSW‐4. The monitoring wells were
purged of at least three volumes of water, or until dry, using new disposable bailers. Groundwater sampling
records that document the sampling process are included in Appendix A. The water supply wells during the
January 2016 and June 2016 sampling events were purged for 15 minutes each prior to collection of samples.

Following purging of the wells, groundwater samples were collected in laboratory‐supplied containers, placed into
an ice‐filled cooler, and transported under chain‐of‐custody to FTS Analytical Services (FTS) in Norcross, Georgia.
Disposable nitrile gloves were worn during the sampling of each well to minimize the potential for cross‐
contamination during the sampling process. At the laboratory, the January 2016 samples were analyzed by EPA
Method 6200B for volatile organic compounds (VOCs) Total lead samples were inadvertently not analyzed during
the January 26, 2016 sampling event. Total lead will return to the list of analyses during the next sampling event.
The June 2016 water supply well samples were analyzed for VOCs by SM 6200B and EPA Method 6010 for total
lead.
5.0 LABORATORY ANALYTICAL RESULTS
The laboratory analytical reports and chain‐of‐custody record for the groundwater samples collected during the
January 2016 and June 2016 sampling events are provided in Appendix B. Groundwater analytical data is
summarized in Table 3.
MTBE was detected in the groundwater samples collected from monitoring wells MW‐1 (44.1 micrograms per liter
[µg/L]) and MW‐3R (30.1 µg/L) at concentrations exceeding the 2L Standard of 20 µg/L. Benzene was detected in
the sample collected from monitoring well MW‐3R at an estimated concentration of 1.30 µg/L, exceeding the
respective 2L Standard of 1 µg/L. Other VOCs were detected below 2L Standards in the groundwater samples
collected from monitoring wells MW‐1, MW‐3R, and MW‐5. No additional VOCs were detected in any of the
groundwater samples analyzed at concentrations exceeding laboratory method detection limits (MDLs).
Total lead samples were inadvertently not collected during the January 26, 2016 sampling event. During the
previous sampling event (December 8, 2014) total lead was detected at concentrations exceeding the 2L standard
of 15 µg/L in monitoring wells MW‐1 (58.8 µg/L) and MW‐6 (42.1 µg/L). Total lead will return to the list of analyses
during the next sampling event.
The laboratory results of groundwater samples collected at water supply wells WSW‐1, WSW‐3, and WSW‐4 did
not reveal target compounds above laboratory method detection limits.
6.0 RECOMMENDATIONS
Based on the data collected during groundwater monitoring activities, Antea USA of NC recommends continued
annual groundwater monitoring with the next sampling event conducted in June 2017. Groundwater samples
should be collected from monitoring wells MW‐1, MW‐1D, MW‐2, MW‐3R, MW‐4 (if accessible), MW‐5, MW‐6,
and from water supply wells WSW‐1, WSW‐3, and WSW‐4 for analysis of EPA Method 602 compounds including
benzene, toluene, ethylbenzene, total xylenes, methyl tert‐butyl ether, diisopropyl ether, and naphthalene.

available upon request. Should insufficient sample be provided to the laboratory to meet the method and
NELAC Matrix Duplicate and Matrix Spike requirements, then the data will be analyzed, evaluated and
reporting using all other available quality control methods.
The validity and integrity of this report will remain intact as long as it is accompanied by this letter and
reproduced in full, unless written approval is granted by FTS Analytical Laboratories. This report will be filed
for at least 5 years in our archives after which time it will be destroyed without further notice, unless
otherwise agreed upon. The samples received, and described as recorded in Work Order A6F0123 will be
filed for 60 days, and after that time they will be properly disposed without further notice, unless otherwise
agreed upon. We reserve the right to return to you any unused samples, extracts, or solutions if we

outside of the laboratory control limits due to possible matrix /chemical interference, or a concentration of target analyte high enough
to affect the recovery of the spike concentration. This condition could also affect the relative percent difference in the MS/MSD.
B A target analyte or common laboratory contaminant was identified in the method blank. Its presence indicates possible field or
laboratory contamination.
