Dear Mr. Hollingsworth:
The NC Division of Waste Management has reviewed your application to renew Septage Detention and
Treatment Facility permit, SDTF-41-03, in Guilford County. Your application, (FID 1826115), has been
approved in accordance with NC Septage Management Rules and your permit, SDTF-41-03, is attached.
This permit covers the use of four steel lined inground pits for the storage and solidification of domestic
septage, grease septage, and commercial/industrial septage as noted in Permit Condition 7. This permit
shall expire on August 31, 2028. An application for permit renewal must be submitted ninety (90) days
prior to permit expiration.
Please read the permit conditions carefully. When communicating to the Division about this permit, refer
to it as "SDTF-41-03." Failure to operate the septage detention facility in accordance with NC Septage
Management Rules and the enclosed permit could subject you to administrative penalties of up to
$15,000 per violation per day. If you have questions concerning this permit or septage in general, please

is hereby permitted to operate Septage Detention and Treatment Facility with permit # SDTF-41-03 located on
Corporate Park Drive in Guilford County at approximate position 36.16784' N latitude and-79.73484' E
longitude. This site is permitted only for operations that are conducted in accordance with the representations
made in the approved application, with all conditions attached to this permit, and with all of the provisions of 15A
NCAC 1313.0800 -- Septage Management. Failure to operate as permitted may result in the Department
suspending or revoking this permit, initiating action to enjoin the unpermitted operation, imposing administrative
penalties, or invoking any other remedy as provided in Chapter 130A, Article 1, Part 2 of the North Carolina
General Statutes.
This facility is subject to periodic inspection, as deemed necessary by the Division of Waste Management, to
determine if the facility is in compliance with this permit. This permit is subject to review at least annually, or
more often if deemed necessary by the Division of Waste Management, to determine if the operations
permitted are consistent with the Septage Management Rules in effect at the time of the review. Modifications
to the conditions of this permit may be made where necessary to make this permit conform to the requirements
of rules in effect at the time of the review.

CONDITIONS OF OPERATING PERMIT
1. This facility shall be maintained and operated in a manner that will protect the
assigned water quality standards of both surface and ground waters.
2. Leaks and spills are the responsibility of the Permittee and shall be immediately
contained, cleaned up, and treated to prevent any contamination of the surrounding
area. Transfers of septage to and from the detention system shall be conducted in a
manner that prevents leaks or spills of septage as stated within Rule 15A NCAC 13B
.0838(j). Any liquid or solids collected from the spillage or leakage of septage shall be
disposed of in a manner consistent with the NC Solid Waste Management Rules. Major
spills shall be reported to the Division of Waste Management within 24 hours of
occurrence.
3. The method for controlling odors described in the approved application for this permit
must effectively minimize odors created by the operation of this facility as required by
15A NCAC 13B .0838(g). Also, this facility shall be designed, constructed, and
maintained in such a manner as required by 15A NCAC 13B .0838(e)(2), to minimize
the attraction or admittance of vectors.
4. Erosion and runoff control measures shall be implemented to prevent migration of
soil or wastes off this facility. The installation of groundwater monitoring wells or a
leak detection system may be required at the discretion of the Division.
5. This permit shall not affect the applicability of any statutes, rules, regulations, or
ordinances that may be imposed by other local, state, or federal government agencies
that may have jurisdiction over this facility.
6. This permit may be modified or reissued at any time to incorporate any conditions,
limitations, and/or monitoring requirements the Division deems necessary to
adequately protect the environment and public health.

7. This septage treatment facility consists of four steel lined inground pits (two 300
cubic yards and two 100 cubic yards) located within a metal building for the storage
and solidification of domestic septage, grease septage, and commercial/industrial
septage. Two of the pits are 40 feet x 30 feet x 16 feet (62,242 gallons) and two of
the pits are 13 feet x 13 feet x 16 feet (20,229 gallons) for a total capacity of 82,471
gallons. Liquid and solid waste generated at this facility shall be disposed of in a
manner consistent with the approved application and submitted authorizations.
Authorizations for liquid and solid waste disposal shall be submitted to the Division
prior to utilizing a disposal location. This office shall be contacted immediately if a
disposal location revokes the authorization.
8. No one other than the Permittee or an employee of the firm named in this permit
may discharge septage at this facility unless the Division is notified, and approval is
granted by the Division. Only Septage Management Firms permitted to operate in
North Carolina may receive authorizations to discharge at this facility. Each Firm
(other than the Permittee) shall have a properly completed and approved
"Authorization to Discharge Septage at a Septage Treatment or Storage Facility
Permitted to Someone Other than Yourself" on file with the Division before using this
facility.
9. This permit shall expire on August 31, 2028. Renewal or modification of this permit
shall be in accordance with 15A NCAC 13B .0837 (i), (m), (n), or (q). An application for
permit renewal shall be submitted at least ninety (90) days prior to the permit renewal
date. Accurate site drawings (2 copies with a North Carolina PE stamp), a signed &
dated operation and maintenance manual (revised to reflect any physical or
operational changes at the facility), current solids and wastewater disposal
authorizations and a copy of the required facility records shall be submitted with the
renewal application.
10.The storage capacity, construction, treatment methods, and operation plan of this
facility shall be as represented in the permit application. Any changes shall require a
permit modification.
11. Records shall be maintained in accordance with the Septage Management Rule 15A
NCAC 13B .0838(v). These records shall be made available to a representative of the
Division upon request. A copy of these records for the period of time this permit was
valid shall be submitted along with an application for permit renewal or modification.

12. Any duly authorized officer, employee, or representative of the Division may, upon
presentation of credentials, enter and inspect any property, premises, or place on or
related to the facility, at any reasonable time for the purpose of determining
compliance with this permit; may inspect or copy any records that must be kept under
the conditions of this permit; or may obtain samples of groundwater, surface water,
or leachate.
13.The setbacks prescribed by 15A NCAC 13B .0838 (m) shall be maintained at all times.
