This Limited Site Assessment (LSA) report has been prepared by Consultech Environmental,
LLC. (Consultech) for the Jerome Dickens (site) located at 209 Saint James Street in Tarboro,
North Carolina. This LSA has been prepared to address the release of petroleum hydrocarbon
compounds from an underground storage tank (UST) system that was located on the site. The
site number of USTs and the size are unknown. Their removal date is unknown. The release was
assigned Incident number 16154 by the North Carolina Department of Environmental Quality
(NCDEQ), UST Section. The results of the LSA are summarized below:

Groundwater was encountered at a depth of 15 feet below ground surface.
Soil and Groundwater Assessment
No VOCs, VPH,orEPHweredetectedinthe soilsamplescollectedfrom aboringdrilledintheformer
tank basin. Chromium, Lead, and one SVOC (Di-n-butyl phthalate) were detected in soil samples
collected from MW-1 at depths of 5 feet and 15 feet. Chromium was the only constituent detected
in the soil samples at concentrations above the North Carolina Department of Environmental Quality
(NCDEQ), Division of Waste Management (DWM) Soil to Water MSCCs.
Numerouspetroleum constituents were detected inthegroundwater samplecollectedfrom wellMW-
1 installed in the former tank basin. Benzene, Naphthalene, Vinyl Chloride, 1-Methylnaphthalene,
Lead, C5-C8 Aliphatics, C9-C10 Aromatics, and C11-C22 Aromatics were detected in the
groundwater in well MW-1 at concentrations above the NC 2L Groundwater Standards. No
constituent was detected in the groundwater in well MW-1 at concentrations above the Gross
Contaminant Levels.
Risk Classification
The site should be classified as low risk since no water supply wells were identified within 1,500
feet of the release and the nearest surface water feature is greater than 500 feet from the source
area.

The site is located at 209 Saint James Street. The site is currently a lot with no physical structures;
the lot is currently being used to store trailers for rental. The site is surrounded by commercial and
residential properties. Thetopography intheregion is relatively flat. Sitepropertyandall surrounding
properties utilize public water for drinking purposes. Surrounding property use can be seen on
Figure 3.

Two surface water bodieswere identified within 1500 feet of the property. Hendricks Creek is located
approximately 800 feet west and downgradient of the site and the Tar River is located approximately
1,400 feet south of the site. The surface water features are listed in Table 1 and their locations are
presented on Figure 3.

Wellhead Protection Areas are administered by the North Carolina Division of Environmental
Quality, Public Water Supply (PWS) Section. According to the PWS, no Wellhead Protection
Areas are located within 1,500 feet of the source area.

The site property is located in the Coastal Plain Physiographic Province. The shallow groundwater
contamination at the site is not likely to have impacted a deep aquifer in the area.

There are no subsurface structures on the property. Utilities are located along Saint James Street
and Trade Street. The utilities included natural gas lines and aboveground power and
communication lines. The utilities are located outside of the former UST location.

The site is a lot used to rent trailers. A list of the adjacent property owners is Included in Table
2.

According to the 1985 Geologic Map of North Carolina, the site is located in the Coastal Plain
Geologic Belt and the underlying rock is classified as the Yorktown Formation: Fossiliferous clay
with varying amounts of fine-grained sand, bluish gray, shell material. Soil encountered at the site
consisted of light brown to medium brown sand to 1.5 feet, dark brown clay to 3.2 feet, brown
sandy clay to 15.2 feet, and gray saturated sand to 20 feet. Based on the topography,
groundwater is expected to flow south and / or southwest from the site.
K UCORNKPI!TGUWNVU!
On October 13, 2023, South Atlantic Environmental Drilling and Construction Co. Inc.
(SAEDACCO) drilled one soil boring (MW-1) within the estimated former UST basin location. The
boring was drilled using hollow stem augers to a depth of 20 feet; water was encountered at 15
feet. The soil boring location is shown in Figure 2.

Soil samples were collected from boring MW-1 at five-foot intervals (5, 10, and 15 feet). The soil
samples were placed into appropriate laboratory-supplied containers, the samples were then
placed on ice, and shipped under chain-of-custody to Eurofins Environment Testing in Pensacola,
FL for analysis for volatile organic compounds (VOCs) by EPA Method 8260, semivolatile organic
compounds (SVOCs) by EPA Method 8270, extractable petroleum hydrocarbons (EPH), and
volatile petroleum hydrocarbons (VPH) by the MADEP method.
No VOCs, VPH, or EPH were detected in the soil samples from boring MW-1. Low concentrations
of lead and one SVOC (di-n-butyl phthalate) were detected in two of the soil samples (MW-1 at 5
feet and MW-1 at 15 feet); these constituents were detected at concentrations below applicable
Soil to Water Maximum Soil Contamination Concentrations (MSCCs). Chromium was detected
in the two soil samples from boring MW-1 at concentrations (19.6 milligrams per kilogram - mg/kg
and 12.5 mg/kg) above the Soil to Water MSCC (5.4 mg/kg) but below the Residential MSCC (47
mg/kg). No other contaminant concentrations were detected above laboratory detection limits.
Soil analytical data is summarized in Table 3 while the soil laboratory analytical report is contained
in Appendix A.
On October 13, 2023, SAEDACCO installed a monitoring well (MW-1) in the borehole to a total
depth of 18 feet. The monitoring well location is shown on Figure 2. A well completion form is
contained in Appendix B. A groundwater sample was collected from MW-1 on October 13, 2023.
The sample was placed into appropriate laboratory-supplied containers, the samples were then
placed on ice, and shipped under chain-of-custody to Eurofins Environment Testing in Pensacola,
FL for analysis for VOCs by EPA Method 8260, SVOCs by EPA Method 625.1, EPH/VPH by the
MADEP method, and lead by USEPA Method 6010D.
Numerous constituents were detected in the groundwater sample collected from well MW-1. A
complete list of detections is presented in Table 4. Benzene, Naphthalene, Vinyl Chloride, 1-
Methylnaphthalene, Lead, C5-C8 Aliphatics, C9-C10 Aromatics, and C11-C22 Aromatics were
detected in the groundwater in well MW-1 at concentrations of 10.6 micrograms per liter (ug/L),
21.8 ug/L, 5.15 ug/L, 3.98 ug/L J, 205 ug/L, 1300 ug/L, 463 ug/L, and 346 ug/L, respectively.
These constituents were detected at concentrations above the NC 2L Groundwater Standards.
No constituents were detected in the groundwater sample from well MW-1 at concentrations
above the NCDEQ Gross Containment Levels (GCLs). Groundwater analytical data is
summarized in Table 4 and the groundwater laboratory analytical report is presented in Appendix
A.
L EQPENWUKQPU!CPF!TGEQOOGPFCVKQPU!
The following conclusions were reached regarding the release from the former Jerome Dickens
facility:
’The site previously contained USTs of unknown size or contents.
’The site property and surrounding properties utilize public water for drinking purposes. No water
supply wells were identified within 1,500 feet of the site. Surface water features were observed
approximately 800 and 1400 feet from the former UST basin.
’No VOCs, VPH, or EPH were detected in the soil samples collected from a boring drilled in the
former tank basin. Low concentrations of lead and Di-n-butyl phthalate were detected in two of
the soil samples collected from the boring; these constituents were detected at concentrations
below applicable soil to water MSCCs. Chromium was detected in the soil samples collected

from boring MW-1 at concentrations above the Soil to Water MSCC but below the Residential
MSCC.
’Numerous petroleum constituents were detected in the groundwater sample collected from
the well installed in the former tank basin. Benzene, Naphthalene, Vinyl Chloride, 1-
Methylnaphthalene, Lead, C5-C8 Aliphatics, C9-C10 Aromatics, and C11-C22 Aromatics
were detected in the groundwater in well MW-1 at concentrations above the NCAC 2L
groundwater quality standards. No constituents were detected in the groundwater sample at
concentrations above the GCLs.
’Consultech recommends the incident be assigned a No Further Action status after the
preparation and submittal of a Notice of Residual Petroleum restricting the use of groundwater
in the area.
