Harmony Church Road Rougemont, Person County, North Carolina NCDEQ Incident No: 3831 NCDEQ Contract No: N14007i-C December 13, 2016 EXECUTIVE SUMMARY The subject site is known as the Mt. Harmony Grocery site and is located at 1818 Mt. Harmony Church Road in Rougemont, Person County, North Carolina. The subject site is further identified by Person County Geographic Information Systems (GIS) Parcel Identification Number (PIN) 0943-00-04-1612.000. On October 10, 2016, ECS collected groundwater samples from monitoring well MW-1 through MW-4 and DW-1. The resident located on the subject site (1818 Mt. Harmony Church Road) was not present at the residence at the time of sampling and ECS did not observe an exterior spigot on the house; therefore, ECS did not sample water supply well PW-1. ECS observed water supply wells PW-2 and PW-4. ECS did not observe water supply well PW-3. ECS did not receive written permission to collect groundwater samples from these wells. Additionally ECS performed a receptor survey to identify potable water supply wells located within 1,000 feet of the subject site. Based on the results of the assessment, ECS summarizes and concludes the following:  Laboratory analysis detected benzene at a concentration above its Gross Contamination Level (GCL) in the groundwater sample collected from monitoring well MW-2. Laboratory analysis detected several targeted analytes at concentrations above their respective North Carolina 2L Groundwater Quality Standards (NC2LGWQS) but below their respective GCL in the groundwater sample collected from monitoring well MW-2. Additionally, laboratory analysis detected isopropylbenzene at a concentration above its laboratory reporting limit but below its NC2LGWQS in the groundwater sample collected from monitoring well MW- 2.  Laboratory analysis detected benzene and methyl-tert-butyl ether (MTBE) at concentrations above their respective GCLs in the groundwater sample collected from monitoring well MW-3. Laboratory analysis detected several targeted analytes at concentrations above their respective NC2LGWQS but below their respective GCL in the groundwater sample collected from monitoring well MW-3. Additionally, laboratory analysis detected ethylbenzene at a concentration above its laboratory reporting limit but below its NC2LGWQS in the groundwater sample collected from monitoring well MW-3.  Laboratory analysis detected methyl butyl ketone at a concentration above its NC2LGWQS but below its GCL in the groundwater sample collected from monitoring well DW-1. Laboratory analysis detected isopropyl ether at a concentration above the laboratory reporting limit but below its NC2LGWQS in the groundwater sample collected from monitoring well DW-1.  ECS identified eleven potable water supply wells located within 1,000 feet of the source area;  ECS received a returned water supply well letter from Mr. Billy Barber (1818 Mt. Harmony Church Road; subject site) stating a water supply well (PW-1) is located on the site and the well is currently active; however, ECS has not received additional water supply well letters from the surrounding residences. ECS did not observe water supply well PW-3. ECS did not receive written permission to collect groundwater samples from these wells.  According to the City of Roxboro Public Services, municipal water is not connected, nor available to the site or properties within 1,000 feet of the site.  Based on the result of the risk characterization, the site meets criteria for High Risk ranking. Based on the results of this assessment, ECS recommends the following:  Collecting soil samples for risk-based analysis along the sidewalls of the former UST location to determine the extent of contaminated soils on the subject site. Each soil sample will be collected at depths of five feet below the ground surface (ft bgs), ten ft bgs, and 15 ft bgs.  ECS recommends collecting groundwater samples from monitoring wells MW-1 through MW-3, MW-UST, DW-1, the three newly installed monitoring wells, and water supply wells PW-1, PW-2, PW-4, WSW-3, and WSW-4.  Installing and sampling two additional monitoring wells downgradient of the source area and one monitoring well located to the east of the subject site, across Mt. Harmony Church Road to determine the extent of the contamination associated with the subject site. ECS proposes one monitoring well installed in the NCDOT right-of-way of the property located at 1850 Mt. Harmony Church Road and two monitoring wells installed in the NCDOT right-of-way of the Mt. Tirzah Park located on Mt. Harmony Church Road.  ECS recommends, upon the completion of the installation of the three additional monitoring wells, surveying all monitoring wells associated with the site for top of casing (TOC) elevation.  ECS recommends that a copy of this report be submitted to the current property owner. Harmony Church Road Rougemont, Person County, North Carolina NCDEQ Incident No: 3831 NCDEQ Contract No: N14007i-C December 13, 2016 1 SITE HISTORY The subject site is known as Mt. Harmony Grocery and is located 1818 Mt. Harmony Church Road in Rougemont, Person County, North Carolina (Figures 1 and 2). According to information provided to ECS by the North Carolina Department of Environment Quality (NCDEQ), the site historically operated as a convenience store and gasoline station with two 1,000-gallon gasoline USTs, which were removed from the site in March 1990 (Table 1). In March of 1990, Groundwater Management Associates Inc. (GMA) conducted soil and groundwater sampling at the site. GMA collected ten soil samples from the walls and floor of the excavation after the removal of the two USTs. Composite soil samples from the walls (S1 through S5) were taken from depths of three to nine feet below ground surface (ft bgs). Grab soil samples from the bottom of the excavation (S6 through S10) ranged from six to nine feet bgs. Laboratory analysis detected total petroleum hydrocarbons gasoline range organics (TPH- GRO) above the current action level of 50 mg/kg in soil samples S1 and S4 through S10. Laboratory analysis also detected TPH-GRO above the laboratory reporting limit but below the action level of 50 mg/kg in soil samples S2 and S3. Additionally, GMA collected three groundwater samples from water supply wells PW-2 through PW-4. Laboratory analysis detected benzene, toluene, and total xylenes at concentrations above their respective North Carolina 2L Groundwater Quality Standards (NC2LGWQS) but below their respective Gross Contamination Levels (GCL) in groundwater collected from water supply well PW2. Laboratory analysis detected xylenes above the laboratory reporting limits but below its NC2LGWQS in the groundwater collected from PW3. Historical soil and water supply well analytical results are summarized in Table 2 and Table 5, respectively. In September of 2005, GeoLogix conducted a Phase I and II Limited Site Assessment (LSA). During the Phase I portion of the LSA, Geologix installed a monitoring well (MW-UST) within the former location of the USTs (Figure 3). Soil samples were collected at depths of eleven and twenty-one ft bgs. Laboratory analysis detected benzene and methyl tert-butyl ether (MTBE) at concentrations above their respective GCLs in the groundwater sample collected from MW- UST. Based on the results of the Phase I LSA, Geologix determined a Phase II LSA was required. During Phase II LSA, four monitoring wells, one deep/vertical extent well (DW-1) and three groundwater monitoring wells (MW-1 through MW-3), were constructed. Geologix collected groundwater samples from DW-1 and MW-1 through MW-3. Laboratory analysis detected MTBE at a concentration above its NC2LGWQS but below its GCL in the groundwater collected from monitoring well DW-1. Laboratory analysis detected benzene above its GCL in the groundwater collected from monitoring well MW-1 through MW-3. Laboratory analysis detected several additional analytes above their respective NC2LGWQSs but below their respective GCLs in the groundwater collected from monitoring wells MW-1 through MW-3. Based on the levels of benzene and MTBE in the groundwater from monitoring wells MW-1 through MW-3, Geologix stated it appears the highest level of petroleum contamination is directed to the southeast and may be an indicator of groundwater flow; however, Geologix did not calculate the groundwater flow direction consistent with Phase II LSA requirements. Geologix identified three water supply wells adjacent to the subject site. Geologix stated laboratory analysis of the water supply well (PW-2) located on the Latta property (adjacent to the south) detected several targeted analytes in the groundwater sample. Additionally, minor detections of xylenes were detected in the water supply well (PW-3) located adjacent to the south of the subject site. SITE GEOLOGY AND HYDROGEOLOGY As determined from the USGS Topographic Maps, Moriah and Timberlake, North Carolina Quadrangles dated 2016 (Figure 1), the site is at an approximate elevation of 655 feet above mean sea level. Historically, surface runoff on the site appeared to be generally to the southeast. Estimated groundwater flow direction is shown on Figure 4. The subject site is situated in the Piedmont Physiographic Province. The soils encountered in this area are the residual product of in-place chemical weathering of rock presently underlying the site and/or historic depositional events. In general, shallow unconfined groundwater movement within the overlying soils is controlled largely by topographic gradients. However, as the groundwater percolates downward, it becomes controlled by the subsurface geologic conditions. Thus, the direction of groundwater movement in the deeper aquifers may not be consistent with the reflecting topography. In general, shallow unconfined groundwater movement within the overlying soils is controlled largely by topographic gradients. Recharge occurs primarily by infiltration along higher elevations and typically discharges into streams or other surface water bodies. The elevation of the shallow water table is transient and can vary with seasonal fluctuations in precipitation. Movement of shallow groundwater is generally from higher to lower elevations. RECEPTOR INFORMATION Risk Characterization The answers in the following questionnaire will help with the assignment of the risk classification for the site. Part I - Groundwater/Surface Water/Vapor Impacts High Risk 1. Has the discharge or release contaminated any water supply well including any used for non-drinking purposes? YES Historically, a water supply well (PW-2) located on the property adjacent to the south was contaminated. 2. Is a water supply well used for drinking water located within 1,000 feet of the source area of the discharge or release? YES According to the City of Roxboro Public Services municipal water is not connected, nor available to the subject site or the surrounding areas within 1,000 feet of the source area; therefore, ECS has identified eleven active water supply wells within 1,000 feet of the source area. Is a water supply well used for any purpose (e.g., irrigation, washing cars, industrial cooling, filling swimming pools) located within 250 feet of the source area of the release or discharge? YES Two active water supply wells are located within 250 feet of the source area. Water supply well PW-1 is located approximately 135 north of the former source area of the release and water supply well PW-2 is located approximately 150 feet south of the source area of the release. 4. Does the groundwater within 500 feet of the source area of the discharge or release have the potential for future use in that there is no other source of water supply other than the groundwater? YES According to the City of Roxboro Public Services, municipal water is not connected, nor available to the subject site and is not available to properties within 1,000 feet of the source area of the release. 5. Do vapors from the discharge or release pose a threat of explosion because of accumulation of the vapors in a confined space or pose any other serious threat to public health, public safety, or the environment? NO Based on the depth to groundwater beneath the site (approximately 20 feet) petroleum vapors would not be expected be a vapor concern for the site. 6. Are there any other factors that would cause the discharge or release to pose an imminent danger to public health, public safety, or the environment? NO Intermediate Risk 7. Is a surface water body located within 500 feet of the source area of the discharge or release? NO 8. Is the source area of the discharge or release located within a designated wellhead protection area as defined in 42 USC 300h-7(e)? NO The NCDEQ wellhead protection mapping tool indicated the subject site is not located within a wellhead protection area. The link to the NCDEQ website is http://www.ncwater.org/?page=63. 9. Is the discharge or release located in the Coastal Plain Physiographic region as designated on a map entitled “Geology of North Carolina” published by the Department in 1985? NO If YES, is the source area of the release located in an area in which there is recharge to an unconfined or semi-confined deeper aquifer that is being used or may be used as a source of drinking water? Do the levels of groundwater contamination for any contaminant exceed the gross contamination levels established by the Department of Environmental Quality? YES Laboratory analysis in October of 2016 detected benzene and MTBE at concentrations above their respective GCLs in groundwater collected from monitoring wells MW-2 and MW-3. Part II - Land Use Property Containing Source Area of Discharge or Release 1. Does the property contain one or more primary or secondary residences (permanent or temporary)? YES Currently, a primary residence is located on the subject site. 2. Does the property contain a school, daycare center, hospital, playground, park, recreation area, church, nursing home, or other place of public assembly? NO 3. Does the property contain a commercial (e.g., retail, warehouse, office/business space, etc.) or industrial (e.g., manufacturing, utilities, industrial research and development, chemical/petroleum bulk storage, etc.) enterprise, an inactive commercial or industrial enterprise, or is the land undeveloped? YES The site previously operated as a convenience store and gasoline station. 4. Do children visit the property? YES The site contains a primary residence; therefore, children visit the property. 5. Is access to the property reliably restricted consistent with its use (e.g., by fences, security personnel, or both)? NO 6. Does pavement, buildings, or other structures cap the contaminated soil? NO 7. What is the zoning status of the property? According to Person County Planning and Zoning Department the property is zoned residential. 8. What is the distance from the source area of the release to the nearest primary or secondary residence (permanent or temporary)? A primary residence is located approximately 90 feet north of the source area. 10. What is the distance from the source area of the release to the nearest school, daycare center, hospital, playground, park, recreation area, church, nursing home or other place of public assembly? The Mt. Tirzah Park is located adjacent to the east and approximately 115 feet of the source area. 11. What is the zoning status of properties in the surrounding area? According to Person County Planning and Zoning Department, properties located in the vicinity of the site are designated as residential and rural conservation (RC). 12. Briefly characterize the use and activities of the land in the surrounding area. The surrounding area is utilized for residential, agricultural, and recreational purposes. Water Supply Wells On October 10, 2016, ECS performed a walking and vehicular reconnaissance of properties within 1,000 feet of the site. Water supply wells were observed at residences along Mt. Harmony Church Road within 1,000 feet of the site. ECS contacted the City of Roxboro Public Services and according to municipal records, municipal water supply wells are not located within 1,000 feet of the subject site. Additionally, water supply well surveys were mailed via United States Postal Service (USPS) to the property owners located within 500 feet of the source area. ECS received a returned water supply well letter from Mr. Billy Barber (1818 Mt. Harmony Church Road; subject site) stating the property contains an active water supply well (PW-1) and is not connected to municipal water. ECS observed water supply wells PW-2 and PW-4 but did not observe water supply well PW-3. ECS did not receive returned water supply well letters from the owners of these wells. Copies of completed questionnaires are included in Appendix A. According to the City of Roxboro Public Services, municipal water is not connected, nor available to the subject site and not available to the surrounding area; therefore; ECS assumes properties with residences or buildings located within 1,000 feet of the subject site to contain water supply wells. ECS identified eleven potable water supply wells within 1,000 feet of the site. The closest waterline is located that could be run to the area is approximately 3.5 miles south of the subject site along Helena-Moriah Road. The names and addresses of the property owners with water supply wells located within 1,000 feet of the site are included in Table 3 and shown on Figure 5. Environmental Services Report – Mt. Historical water supply well analytical data is included in Table 5. Public Water Supplies ECS contacted the City of Roxboro Public Services to inquire if municipal water is available for the subject site and surrounding area. According to the City of Roxboro Public Services municipal water is not connected to the subject site and not available to the surrounding area. The closest waterline is located that could be run to the area is approximately 3.5 miles to the south of the subject site along Helena-Moriah Road. Surface Water As determined from the USGS Topographic Map, Moriah and Timberlake, North Carolina Quadrangles dated 2016 (Figure 1), the site is at an approximate elevation of 655 feet above mean sea level. Surface runoff on the site is generally to the southeast towards Rock Fork Branch. Subsurface Structures ECS did not identify subsurface structures associated with the subject site. Land Use The subject site is developed with the former grocery store building and a single-family residence. The surrounding area consists of residential, agricultural, and recreational properties. FIELD ACTIVITIES On October 10, 2016, ECS collected groundwater samples from monitoring well MW-1 through MW-4 and DW-1. The resident located on the subject site (1818 Mt. Harmony Church Road) was not home at the time of sampling and ECS did not observe an exterior spigot on the house; therefore, ECS did not sample water supply well PW-1. Additionally ECS performed a receptor survey to identify potable water supply wells located within 1,000 feet of the subject site. Site photographs are included in Appendix B. Field notes associated with field activities are included in Appendix C. Prior to monitoring well sampling activities, the static groundwater level was measured in each monitoring well. Groundwater ranged from 21.25 feet below the top of casing (ft btoc) to 25.50 ft btoc. Well construction details and groundwater level data is summarized in Table 6 and the locations of site monitoring wells are shown on Figure 3. Purging and sampling were accomplished using a peristaltic pump with dedicated tubing. Each monitoring well was purged until the following geochemical field measurements had stabilized; temperature, pH, conductivity, and dissolved oxygen. ECS utilized a YSI model 556MPS water quality meter calibrated using the manufacturers recommendations. Monitoring well sampling sheets completed during field activities are included in the Appendix C. Immediately after Environmental Services Report – Mt. Harmony Grocery 1818 Mt. Harmony Church Road Rougemont, Person County, North Carolina NCDEQ Incident No: 3831 NCDEQ Contract No: N14007i-C December 13, 2016 7 collection, the groundwater samples were transferred into sample containers provided by the laboratory. The sample containers were labeled with ECS project number, sample identification, sample date and time, and requested analytical analysis. The containers were placed into protective packaging material and placed into a cooler with ice to maintain the samples at approximately 4 Celsius (C). The groundwater and soil samples were submitted to Prisms Laboratories, Inc. located in Charlotte, North Carolina for chemical analysis. Groundwater samples were analyzed for volatile organic compounds (VOCs) using Standard Method 6200B. ECS maintained proper chain of custody (COC) procedures throughout the sample collection and transportation process. LABORATORY ANALYTICAL RESULTS Laboratory analysis detected several targeted analytes above their respective laboratory reporting limits but below their NC2LGWQS in the groundwater samples collected from monitoring well MW-1. Laboratory analysis detected benzene at a concentration above its GCL in the groundwater sample collected from monitoring well MW-2. Laboratory analysis detected several targeted analytes above their respective NC2LGWQS but below their respective GCL in the groundwater sample collected from monitoring well MW-2. Additionally, laboratory analysis detected isopropylbenzene at a concentration above the laboratory reporting limit but below its NC2LGWQS in the groundwater sample collected from monitoring well MW-2. Laboratory analysis detected benzene and MTBE at concentrations above their respective GCLs in the groundwater sample collected from monitoring well MW-3. Laboratory analysis detected several targeted analytes above their respective NC2LGWQS but below their respective GCL in the groundwater sample collected from monitoring well MW-3. Additionally, laboratory analysis detected ethylbenzene at a concentration above the laboratory reporting limit but below its NC2LGWQS in the groundwater sample collected from monitoring well MW-3. CONCLUSIONS Based on the results of this Groundwater Sampling Event, ECS concludes the following:  Laboratory analysis detected benzene at a concentration above its GCL in the groundwater sample collected from monitoring well MW-2. Laboratory analysis detected several targeted analytes at concentrations above their respective NC2LGWQS but below their respective GCL in the groundwater sample collected from monitoring well MW-2. Harmony Church Road Rougemont, Person County, North Carolina NCDEQ Incident No: 3831 NCDEQ Contract No: N14007i-C December 13, 2016 8 analysis detected isopropylbenzene at a concentration above the laboratory reporting limit but below its NC2LGWQS in the groundwater sample collected from monitoring well MW-2.  Laboratory analysis detected benzene and MTBE at concentrations above their respective GCLs in the groundwater sample collected from monitoring well MW-3. Laboratory analysis detected several targeted analytes at concentrations above their respective NC2LGWQS but below their respective GCL in the groundwater sample collected from monitoring well MW-2. Additionally, laboratory analysis detected ethylbenzene at a concentration above the laboratory reporting limit but below its NC2LGWQS in the groundwater sample collected from monitoring well MW-3.  Laboratory analysis detected methyl butyl ketone at a concentration above its NC2LGWQS but below its GCL in the groundwater sample collected from monitoring well DW-1. Laboratory analysis detected isopropyl ether at a concentration above the laboratory reporting limit but below its NC2LGWQS in the groundwater sample collected from monitoring well DW- 1.  ECS identified eleven potable water supply wells located within 1,000 feet of the source area; ECS received a returned water supply well letter from Mr. Billy Barber (1818 Mt. Harmony Church Road; subject site) stating a water supply well (PW-1) is located on the site and the well is currently active; however, ECS has not received additional water supply well letters from the surrounding residences.  According to the City of Roxboro Public Services, municipal water is not connected, nor available to the site and not available to the surrounding area. The closest waterline is located that could be run to the area is approximately 3.5 miles to the south of the subject site along Helena-Moriah Road.  Due to the result of the risk characterization, the site meets criteria for High Risk ranking. RECOMMENDATIONS  ECS recommends collecting soil samples for risk-based analysis along the sidewalls of the former UST location to determine the extent of contaminated soils on the subject site. Each soil sample will be collected at depths of five ft bgs, ten ft bgs, and 15 ft bgs.  ECS recommends collecting groundwater samples from monitoring wells MW-1 through MW-3, MW-UST, DW-1, the three newly installed monitoring wells, and water supply wells PW-1, PW-2, PW-4, WSW-3, and WSW-4.  ECS recommends installing three additional monitoring wells downgradient of the source area to determine the extent of the contamination associated with the subject site.  ECS recommends, upon the completion of the installation of the three additional monitoring wells, surveying all monitoring wells associated with the site for TOC elevation.  ECS recommends that a copy of this report be submitted to the current property owner.
