EXECUTIVE SUMMARY On October 26, 2015, S&ME collected groundwater samples from eight monitor wells (MW-1 through MW-7, and DW-1) located at the former Wise Convenient Mart site, located at 2027 US Highway 1 North in Wise, Warren County, North Carolina. S&ME also collected samples from two water supply wells WSW-1 and WSW-11. The following summarizes the results of the sampling event:  Free product has historically been reported in monitor well MW-1. SCOPE OF SERVICES 3.1 Contract Information The scope of services for this report was performed in general accordance with S&ME’s proposal number 43-1500743, dated March 3, 2015. The scope of services was authorized by the North Carolina Department of Environment Quality (NCDEQ) Task Authorization TA-01, dated March 10, 2015, and within contract number N14005i. Project delays were encountered because of multiple attempts by NCDEQ to obtain access agreements. 3.2 Approved Scope of Services S&ME’s approved Scope of Services for this Task Authorization included installing two monitor wells (MW-8 and MW-9) collecting groundwater samples from up to 10 monitor wells (MW-1 through MW-9, and DW-1), collecting samples from up to eight water supply wells (WSW-1, WSW-4 (influent and effluent), WSW-5A, WSW-7, WSW-8, WSW-10, and WSW-11), and preparing this report. These services were completed with the following exceptions:  NCDEQ was unable to obtain access agreements to install two new monitor wells MW-8 and MW-9, therefore no groundwater samples were collected.  S&ME was unable to gain permission to sample water supply wells WSW-4 (influent and effluent), WSW-5A, WSW-7, WSW-8, and WSW-10; therefore, no samples were collected from these wells. SITE HISTORY AND CHARACTERIZATION The subject site is located at 2027 US Highway 1 North in Wise, Warren County, North Carolina. A vicinity map of the site is shown on Figure 1, and the site and pertinent features are shown on Figure 2. Photographs of the site are provided in Appendix I. The site consists of the former Wise Convenient Mart structure (now vacant), three occupied residential homes, and several storage sheds and outbuildings. The adjoining properties consist of rural residential development and agricultural fields. According to a Groundwater Monitoring Report prepared by Terraquest Environmental Consultants, P.C. (Terraquest), and dated January 4, 2011, two 550-gallon gasoline underground storage tanks (USTs) were installed at the site prior to 1954. The USTs were reportedly removed from the site in 1988, prior to the existence of UST regulations. The Terraquest report references a Phase II Limited Site Assessment prepared by ATC Associates on May 19, 2006 (S&ME did not receive a copy of this report for review). According to the ATC report, 11 private water supply wells were identified in the vicinity of the site. Municipal water service is reportedly available to the site and surrounding properties. ATC installed five groundwater monitor wells (labeled as MW-1 through MW-5) at the site and collected groundwater and soil samples. The laboratory analytical results of the groundwater samples collected by ATC indicated that several volatile organic compounds (VOCs) were detected in concentrations exceeding their respective North Carolina 2L Groundwater Standards (2LStandards) in monitor wells MW-1, MW-2, and MW-5. The laboratory analytical results of soil samples collected by ATC were not available. One additional monitor well (MW-6) was installed to further delineate the extent of groundwater contamination. On November 11, 2010, Terraquest completed a Comprehensive Site Assessment (CSA) at the site. During this investigation, one additional Type II shallow monitor well (MW-7) and Type III deep monitor well (DW-1) were installed at the site. During the investigation, approximately 0.75 feet of free product was measured in MW-1. Between April 12 and 16, 2010, Terraquest conducted a mobile multi-phase extraction (MMPE) event at the site. During the event, 8,559 gallons of contaminated groundwater were recovered. A total of 708.15 pounds (approximately 131.38 gallons) of petroleum as gasoline were calculated to have been discharged as vapor. On October 22, 2012, Crawford Environmental Services (CES) collected groundwater samples from the monitor well network. The laboratory analytical results of the groundwater samples indicated that several VOCs were detected in concentrations exceeding their respective 2L Standards in the samples collected from MW-1, MW-2, and MW-5. Five water supply wells were sampled (WSW-1, WSW-4, WSW-5A, and WSW-6). Concentrations of VOCs were detected in concentrations below their respective 2L Standards in the samples collected from water supply wells WSW-1, WSW-4 (post POE), and WSW-5A. On March 13, 2014, S&ME collected groundwater samples from seven groundwater monitor wells at the site (MW-2, MW-3, MW-4, MW-5, MW-6, MW-7, and DW-1). Monitor well MW- 1 contained approximately 0.25 inches of free product and was not sampled. The laboratory Groundwater Monitoring Report S&ME Project No. 4305-15-094 TF-1943 Wise Convenience Mart, Wise, Warren County, NC March 2, 2016 8 analytical results of the groundwater samples indicated that several VOCs were detected in concentrations exceeding their respective 2L Standards in MW-5. In addition, benzene was also detected above the 2L Standard in monitor wells MW-2 and MW-6. On March 13, 2014, S&ME collected water samples from six water supply wells (WSW-1, WSW-5A, WSW-6, WSW-7, WSW-8, and WSW-11). Six other water supply wells (WSW-2, WSW-3, WSW-4, WSW-5, WSW-9, and WSW-10) were not sampled because they were either inactive or the homeowner was not present at the time of sampling. The laboratory analytical results of the water supply well samples indicated that MTBE was detected below the 2L Standard in WSW-1 and WSW-7 and 1,2-dichloroethane and di-isopropyl ether were detected below their respective 2L Standards in the water sample collected from WSW-5A. 4.1 UST Owner and Operator Information. The former owner/operator of the UST system in unknown. The property is now owned by Mr. Donald Paynter. Information about the owner/operator of the UST system and the dates of operation are provided in Table 1. 4.2 UST Information According to a Groundwater Monitoring Report prepared by Terraquest Environmental Consultants, P.C. CURRENT SITE ASSESSMENT INFORMATION 5.1 Current Liquid Levels On October 26, 2015, S&ME personnel measured depth to groundwater from the top of casing in monitoring wells (MW-1 through MW-7, and DW-1). Depth to groundwater was measured using an electronic interface probe, which emits an audible tone when encountering free product, and a distinctly different tone when encountering groundwater. The depth to groundwater ranged from 15.18 feet below top of casing (ft-btoc) in monitor well MW-3 to 19.06 ft-btoc in monitor well MW-5. S&ME used the depth to groundwater measurements to generate a depth to groundwater elevation map (Figure 3). The monitor well construction details, depth to groundwater measurements are provided in Table 3. 5.2 Groundwater Monitoring Events On October 26, 2015, S&ME collected groundwater samples from eight monitor wells (MW-1 through MW-7, and DW-1). The monitor wells were purged and sampled using new polyethylene bailers. The wells were purged approximately three casing volumes or bailed dry and allowed to recover prior to sampling. The field parameters consisting of temperature, pH, and specific conductance were measured and recorded for each casing volume removed. The groundwater samples were collected directly into laboratory-supplied containers, placed in a cooler on ice, and shipped under standard chain-of-custody protocol to Con-Test Analytical Services, Inc. (Con-Test) of East Longmeadow, Massachusetts, a North Carolina certified laboratory, for analysis. The samples were analyzed for VOCs by EPA Method 6200B. The field sampling sheets are provided in Appendix II. 5.3 Water Supply Wells S&ME was granted permission to collect a sample from water supply wells WSW-1 and WSW- 11. S&ME was unsuccessful in obtaining permission from the property owners associated with water supply wells WSW-4 (influent and effluent), WSW-5A, WSW-7, WSW-8, and WSW-10. Information pertaining to the water supply wells and adjacent property owners is included in Table 4. Well locations are shown on Figure 4. 5.4 Water Supply Well Monitoring Event On October 26, 2015, S&ME sampled WSW-1 and WSW-11. The samples from the water supply wells was collected from spigot at the well houses. The tap was opened and the water was allowed to run for approximately 15 minutes prior to sample collection. The samples were collected directly into laboratory-supplied containers, placed in a cooler on ice, and shipped under standard chain-of-custody procedure to Con-Test for analysis. The samples were analyzed for VOCs by EPA Method 6200B. The field sampling sheets are provided in Appendix II. Water supply well information is provided in Table 4. 5.5 Summary of Groundwater and Free Product Assessment Information The laboratory analytical results for the groundwater samples collected from monitor wells MW- 1 through MW-7, and DW-1 indicated that the following constituents were reported at concentrations above the 2L Standards:  Benzene in monitor wells MW-1, MW-2, MW-5, and MW-6; Groundwater Monitoring Report S&ME Project No. 4305-15-094 TF-1943 Wise Convenience Mart, Wise, Warren County, NC March 2, 2016 10  1,2-dibromoethane (EDB) in MW-5, and MW-6;  Ethylbezene in MW-1;  P-Isopropyltoluene in MW-1;  Naphthalene in MW-1, MW-2, and MW-5;  N-propylbenzene in MW-1;  Toluene in MW-1;  1,2,4-Trimethylbenzene in MW-1;  1,3,5-Trimethylbenzene in MW-1; and,  Total Xylenes in MW-1 and MW-5. Additional constituents were reported in groundwater samples collected from MW-1, MW-2, MW-5 and MW-6 in concentrations below their respective 2L Standards. No targeted constituents were reported at concentrations above the laboratory analytical method detection limits in the groundwater samples collected from monitor wells MW-3, MW-4, MW-7 and DW- 1. No free product was measured in any of the monitor wells. A summary of the groundwater analytical results is provided in Table 5, and are shown in Figure 5. A copy of the laboratory analytical report and chain-of-custody record in provided in Appendix III. 5.6 Summary of Water Supply Well Assessment Information No targeted constituents were reported at concentrations above the laboratory analytical method detection limits in the groundwater samples collected from WSW-1 and WSW-11. A summary of the water supply well analytical results is presented in Table 6. A copy of the laboratory analytical report and chain-of-custody record are provided in Appendix III. 5.7 Regional and Site Geology and Hydrogeology 5.7.1 Soil and Bedrock According to the Geologic Map of North Carolina, dated 1985, the site lies within Raleigh Belt of the Piedmont Physiographic Province, which is categorized by broadly sloping plains of moderate topographic relief with good drainage and soil development. The site is underlain by Permian to Pennsylvanian granitic rock. 5.7.2 Hydrogeology S&ME used the depth to groundwater measurements to generate a depth to groundwater elevation map (Figure 3). Groundwater flow direction at the site is generally north. The hydraulic conductivity at the site has not been measured; therefore, the linear groundwater velocity was not calculated. 5.8 Evaluation of Groundwater Assessment Information 5.8.1 Current Extent and Severity of Contamination Historically, the highest concentrations of contaminants have been detected in monitor well MW-1. MW-1 is located in front of the Wise convenient mart store. No contaminants have been Groundwater Monitoring Report S&ME Project No. 4305-15-094 TF-1943 Wise Convenience Mart, Wise, Warren County, NC March 2, 2016 11 detected at concentrations exceeding the Gross Contaminant Levels (GCLs) and free product was not been detected in any monitor well during this event. However, free product was previously measured in MW-1 (March 2014). Contaminant concentrations in monitor wells MW-1, MW-2, MW-5 and MW-6 have decreased. No contaminants were detected in monitor well MW-3, MW- 4, MW-7, and DW-1 at concentrations above their laboratory method detection limits. CONCLUSIONS AND RECOMMENDATIONS Based on the current and historical assessment information for the site, the following conclusions are presented: 1. The UST Incident meets the following conditions for a High Risk classification: a. There are active drinking water supply wells within 1,000 feet of the source area. b. Contaminant levels in the groundwater exceed the 2L Standards 2. Groundwater contaminant concentrations do not appear to be decreasing in a consistent trend. Based on the above, the following recommendations are presented: 1. Determine if water supply well owners will connect to the municipal service; 2. Delineate the extent of soil contamination by advancing additional soil borings and collecting soil samples for laboratory analysis. 3. Delineate the extent of groundwater contamination by obtaining access to surrounding properties and installing additional groundwater wells either on the adjacent property to the north of the site, or, if permission cannot be obtained, within the NCDOT right-of- way to the north of the site; and, 4. Continue periodic sampling of water supply wells located down-gradient from the site (WSW-4, WSW-5A, WSW-6).
