S&ME’s approved scope of services included conducting the initial project review and setup, mobilizing to the site and collecting groundwater samples from up to eight monitor wells (MW-1 through MW-7, and DW-1), collecting water samples from up to 11 water supply wells (WSW-1 through WSW-11, including influent and effluent samples from WSW-4), and preparing this groundwater monitoring report. The scope of services for this task was completed with the following exceptions:  Monitor well MW-1 contained approximately 0.25 inches of product and was not sampled;  Water supply wells WSW-2, WSW-3, and WSW-5 have no power and are not in service and were not sampled; and,  Water supply wells WSW-4, WSW-9, and WSW-10 were not sampled because the homeowners were not present at the time of sampling. Groundwater Monitoring Report S&ME Project No. 4305-14-077 TF-1943 Wise Convenience Mart, Wise, Warren County, NC April 21, 2014 2 2.0 SITE HISTORY The former Wise Convenient Mart is located at 2027 US Highway 1 North in Wise, Warren County, North Carolina. The site consists of the former Wise Convenient Mart structure (now vacant), three occupied residential homes, and several storage sheds and outbuildings. The approximate coordinates of the former Wise Convenient Mart structure are 36° 30’ 14.00” North and 78° 10’ 46.25” West. The surrounding area of the site consists of rural residential homes and farmland. The site and vicinity are shown on Figure 1 and the site is shown on Figure 2. According to a Groundwater Monitoring Report prepared by Terraquest Environmental Consultants, P.C. (Terraquest), and dated January 4, 2011, two 550-gallon gasoline underground storage tanks (USTs) were installed at the site prior to 1954. The USTs were reportedly removed from the site in 1988, prior to the existence of UST regulations. The Terraquest report references a Phase II Limited Site Assessment prepared by ATC Associates on May 19, 2006 (S&ME did not receive a copy of this report for review). According to the ATC report, 11 private water supply wells were identified in the vicinity of the site. Municipal water service is reportedly available to the site and surrounding properties. ATC installed five groundwater monitor wells (labeled as MW-1 through MW-5) at the site and collected groundwater and soil samples. The laboratory analtycial results of the groundwater samples collected by ATC indicated that several volatile organic compounds (VOCs) were detected in concentrations exceeding their respective North Carolina 2L Groundwater Standards (2LStandards) in monitor wells MW-1, MW- 2, and MW-5. The laboratory analytical results of soil samples collected by ATC were not available. One additional monitor well (MW-6) was installed to further delineate the extent of groundwater contamination. On November 11, 2010, Terraquest completed a Comprehensive Site Assessment (CSA) at the site. During this investigation, one additional Type II shallow monitor well (MW- 7) and Type III deep monitor well (DW-1) were installed at the site. During the investigation, approximately 0.75 feet of free product was measured in MW-1. Between April 12 and 16, 2010, Terraquest conducted a mobile multi-phase extraction (MMPE) event at the site. During the event, 8,559 gallons of contaminated groundwater were recovered. A total of 708.15 pounds (approximately 131.38 gallons) of petroleum as gasoline were calculated to have been discharged as vapor. On October 22, 2012, Crawford Environmental Services (CES) collected groundwater samples from the monitor well network. The laboratory analytical results of the groundwater samples indicated that several VOCs were detected in concentrations exceeding their respective 2L Standards in the samples collected from MW-1, MW-2, and MW-5. Five water supply wells were sampled (WSW-1, WSW-4, WSW-5A, and WSW-6). Concentrations of VOCs were detected in concentrations below their respective 2L Standards in the samples collected from water supply wells WSW-1, Groundwater Monitoring Report S&ME Project No. 4305-14-077 TF-1943 Wise Convenience Mart, Wise, Warren County, NC April 21, 2014 3 WSW-4 (post POE), and WSW-5A. Limited historical information is provided in Appendix I. Groundwater Monitoring Report S&ME Project No. 4305-14-077 TF-1943 Wise Convenience Mart, Wise, Warren County, NC April 21, 2014 3 3.0 GROUNDWATER MONITORING Site activities were performed in general accordance with a site-specific health and safety plan (HASP) prepared by S&ME. The assessment was conducted in general accordance with the procedures outlined in the NCDENR UST Section Guidelines for the Assessment and Corrective Action for UST Releases, dated July 2008, updated December 1, 2013 (Guidelines). Photographs taken during site activities are included in Appendix II. 3.1 Monitor Well Gauging and Sampling On March 13, 2014, S&ME personnel gauged depth to water and collected groundwater samples from seven groundwater monitor wells at the site (MW-2, MW-3, MW-4, MW- 5, MW-6, MW-7, and DW-1). Monitor well MW-1 contained approximately 0.25 inches of free product and was not sampled. The depth to water was measured in monitor wells using an electronic water meter that emits an audible signal when it contacts water. The depth to groundwater ranged from 16.21 feet below the top of casing in MW-3 to 20.29 feet below the top of casing in MW-5. The groundwater elevations are shown on Figure 3. Groundwater generally flows to the north. The depth to water measurements and the monitor well construction details are provided in Table 1. Groundwater samples were collected using new polyethylene bailers. Each monitor well was purged of two to three casing volumes of water, and until field parameters of temperature, pH, and electrical conductance stabilized. The field parameter readings are shown in the Groundwater Sampling Forms, provided in Appendix III. The groundwater samples were placed in laboratory-supplied containers, stored in a cooler on ice, and shipped under chain-of-custody procedures to Con-Test Analytical Laboratory (Con-Test), a North Carolina certified laboratory, for laboratory analysis. The samples were analyzed for VOCs by EPA Method 6200B. 3.2 Monitor Well Sampling Results The laboratory analytical results of the groundwater samples collected on March 13, 2014 indicated that the following target contaminant constituents were detected in concentrations exceeding their respective 2L Standards:  Benzene: in MW-2, MW-5, and MW-6;  di-isopropyl ether: in MW-5;  naphthalene: in MW-5;  toluene: in MW-5; and,  total xylenes: in MW-5. Several other VOCs were detected in concentrations exceeding their respective laboratory detection limits but below their respective 2L Standards in MW-2, MW-4, MW-5, MW- 6, and MW-7. The groundwater analytical results shown on Figure 4 and are summarized on Table 2. The laboratory analytical report and chain-of-custody form are included in Appendix IV. Groundwater Monitoring Report S&ME Project No. 4305-14-077 TF-1943 Wise Convenience Mart, Wise, Warren County, NC April 21, 2014 4 3.3 Water Supply Well Sampling On March 13, 2014, S&ME personnel collected samples from six water supply wells (WSW-1, WSW-5A, WSW-6, WSW-7, WSW-8, and WSW-11). Water supply wells WSW-2, WSW-3, and WSW-5 are not in service and were not sampled. Water supply wells WSW-4, WSW-9, and WSW-10 were not sampled because nobody was present at the residences at the time of sampling. Water supply well information is included in Table 3. Water supply well locations are presented on Figure 5. The water supply wells were collected from exterior spigots closest to the actual location of the water supply wells. The water supply wells were allowed to operate for approximately 15 minutes before sampling. The water supply well samples were placed in laboratory-supplied containers, stored in a cooler with ice, and shipped under chain-of-custody procedures to Con-Test to be analyzed for VOCs by EPA Method 6200B. 3.4 Water Supply Well Sampling Analytical Results The laboratory analytical results of the water supply wells samples collected on March 13, 2014 indicated that methyl tert-butyl ether (MTBE) was detected in concentrations below the 2L Standard in the water supply well samples collected from WSW-1 and WSW-7. In addition, 1,2-dichloroethane and di-isopropyl ether were detected below their respective 2L Standards in the water sample collected from WSW-5A. On March 28, 2014, S&ME’s Michael Pfeifer informed Mr. Mark Petermann (NCDENR’s Incident Manager) of the results of the water supply well samples via an email message. A summary of current and historical groundwater analytical results and a comparison to NCDENR’s regulatory standards is included on Table 4. The water supply well constituent concentrations are presented on Figure 5. A copy of the groundwater sampling form is included in Appendix III. A copy of the laboratory analytical results and chain-of-custody form is included in Appendix IV. Groundwater Monitoring Report S&ME Project No. 4305-14-077 TF-1943 Wise Convenience Mart, Wise, Warren County, NC April 21, 2014 5 4.0 CONCLUSIONS AND RECOMMENDATIONS  Two 550-gallon gasoline USTs were installed at the site in 1954 and removed from the site in 1988.  Municipal water is available to the site and surrounding properties. Nine active potable water supply wells are located within 1,000 feet of the site.  On March 13, 2014, S&ME collected groundwater samples from seven groundwater monitor wells at the site (MW-2, MW-3, MW-4, MW-5, MW-6, MW-7, and DW-1). Monitor well MW-1 contained approximately 0.25 inches of free product and was not sampled. The laboratory analytical results of the groundwater samples indicated that several VOCs were detected in concentrations exceeding their respective 2L Standards in MW-5. In addition, benzene was also detected above the 2L Standard in monitor wells MW-2 and MW-6.  On March 13, 2014, S&ME collected water samples from six water supply wells (WSW-1, WSW-5A, WSW-6, WSW-7, WSW-8, and WSW-11). Six other water supply wells (WSW-2, WSW-3, WSW-4, WSW-5, WSW-9, and WSW-10) were not sampled because they were either inactive or the homeowner was not present at the time of sampling. The laboratory analytical results of the water supply well samples indicated that MTBE was detected below the 2L Standard in WSW-1 and WSW-7 and 1,2-dichloroethane and di-isopropyl ether were detected below their respective 2L Standards in the water sample collected from WSW-5A. S&ME Recommends:  Determine if water supply well owners will connect to the municipal service;  Evaluate the potential for vapor intrusion to the building near the impacted groundwater plume;  Continue sampling of selected water supply wells on a semi-annual basis; and,  Evaluating free product and source area remediation alternatives.
