(ESP) is pleased to present this Well Abandonment Letter Report documenting the abandonment of monitoring wells Former Smith MW-1, Kluttz MW-1, MW-2, and MW-3 at the subject site, Arnold Kluttz Property, in accordance with North Carolina Department of Environmental Quality (NCDEQ) Task Authorization No. 2 approved on October 19, 2023. The subject site is located at 28384 NC Highway 73, Albemarle, North Carolina. Monitoring wells Former Smith MW-1, Kluttz MW-1, MW-2, and MW-3 were abandoned on December 1, 2023, by Mr. Brian Ewing (North Carolina Well Contractor Certification Number 4240-B), a North Carolina licensed driller employed by South Atlantic Environmental Drilling and Construction Co. Inc. (Saedacco) located in Fort Mill, South Carolina. Prior to abandonment, ESP checked the wells’ identification plates and/or their locations as shown on the attached site map (Figure 1) to confirm the wells’ identities. Monitoring wells Former Smith MW-1 and Kluttz MW-1 were 39-foot deep, 2-inch diameter polyvinyl chloride (PVC) wells with 15-foot screens, while MW-2 and MW-3 were 40-foot deep, 2-inch diameter PVC wells with 20-foot screens. The four monitoring wells were abandoned by removing the wells’ protective covers and concrete surface pads, followed by force injecting neat cement grout from the bottom of the well to the ground surface via a tremie pipe. Top soil was used to backfill the areas where the well pads were removed, and each location was graded, seeded, and wheat strawed. The approximate locations of the abandoned monitoring wells are depicted on attached Figure 1. The well abandonment records (WARs) prepared and signed by Mr. Ewing of Saedacco are provided in Attachment A. Saedacco also submitted the WARs to the NCDEQ Division of Water Resources. Field notes and photographs of the four monitoring wells before and after abandonment are provided in Attachment B.
