Refer to the main text and appendices for a complete reporting of our observations and findings. ESP Associates, Inc. (ESP) has completed a Subsequent Monitoring Report documenting assessment activity performed at the Becky’s Grocery site (Incident No. 3430). The site is an approximately 1.24-acre developed site (Tax Parcel No. 559604947978) located at 20518 Ridgecrest Road in Locust, Stanly County, North Carolina. This report was completed to summarize the findings of assessment activities performed on June 7, 2023, in accordance with North Carolina Department of Environmental Quality (NCDEQ), Task Authorization No. 2, approved on April 14, 2023. The work was performed as part of the State Lead Contract #N70522-D between NCDEQ and ESP. This report was prepared in general accordance with the 15A NCAC 2L .0115 regulations and the NCDEQ Underground Storage Tank (UST) Section Guidelines for Assessment and Corrective Action for UST Releases, January 2021, under the supervision of Nora A. Zirps, North Carolina registered professional engineer (North Carolina License No. 023838). Field activities performed by ESP on June 7, 2023, included:  Obtaining an access agreement to sample water supply well SW-5. Although planned, an access agreement to sample water supply well SW-4 was not obtained because the well is no longer used and the property is connected to municipal water according to the property owner’s daughter.  Gauging static water levels in monitoring wells MW-3 through MW-6 and MW-9.  Collecting groundwater samples from monitoring wells MW-3 through MW-6 and MW-9, including a field duplicate.  Collecting groundwater samples from water supply wells SW-2 and SW-5. Although planned, ESP was unable to obtain a groundwater sample from water supply well SW-4 since it is inactive. Collected water samples were analyzed for volatile organic compounds (VOCs) for comparison to both North Carolina Groundwater Quality Standards 15A NCAC 2L .0202 (2L Standards) and Gross Contaminant Levels (GCLs). Based on results of the above field activities and previous reporting, ESP concludes the following:  Static water level depths ranged from 8.14 to 14.50 feet below top of casing (ft bTOC).  No measurable free product was observed in any of the monitoring wells. Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 iv  The general direction of groundwater flow beneath the site is to the west and southwest.  Four water supply wells (of which only one [SW-2] is active) are located within 500 ft of the source area. SW-2 is located hydraulically cross-gradient of the source area. Municipal water is available to the SW-2 property but not connected.  Benzene was detected at its GCL in monitoring well MW-4.  Multiple constituents exceeded their respective 2L Standards in monitoring wells MW-4, MW-5, MW-6; and MW-9.  No analytes were detected above MDLs in monitoring well MW-3 and water supply wells SW-2 and SW-5.  In general, contaminant concentrations at the site have decreased since 2019. Based on the above results, ESP offers the following recommendations:  Continue to sample monitoring wells MW-4, MW-5, MW-6, and MW-9 along with water supply wells SW-2 and SW-5 on a semiannual basis to further understand concentration trends and plume stability; and  Evaluate results of the Remedial Design Characterization (RDC) event that was conducted by AST Environmental on June 6, 2023, and implement BOS-200 injection, if applicable Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 1 1.0 INTRODUCTION 1.1 Site Setting The Becky’s Grocery site is an approximately 1.24-acre developed site (Tax Parcel No. 559604947978) located at 20518 Ridgecrest Road in Locust, Stanly County, North Carolina (see Figures 1 and 2). The site includes an approximate 2,232-sqft inactive former grocery store building built in 1961 along with paved and gravel surfaces on the western portion and grassy and wooded portions elsewhere. On June 7, 2023, the on-site structure was vacant and undergoing interior electrical renovations. The site is bounded to the north by single-family residences, to the east by woods and farmland beyond, to the south by woods and single-family residences beyond, and to the west by Ridgecrest Road and single-family residences beyond. 1.2 Site Geology and Hydrogeology A consideration of surface and subsurface drainage and geology are of interest since they provide an indication of the direction that contamination, if present on-site or off-site, could be transported. The site is located near the contact between the Yadkin Formation and Floyd Church Formation within the Carolina Slate Belt of the Piedmont Physiographic Province. The Piedmont Physiographic Province generally consists of low to medium elevation, rounded hills, and long rolling northeast to southwest trending ridges with incised creeks and river channels. Earth Materials belonging to the Yadkin Formation are described as Late-Proterozoic-aged meta- graywacke, volcanic sandstone and siltstone interbedded with mafic and intermediate metavolcanic flows and tuffs. Earth Materials belonging to the Floyd Church Formation are described as Late-Proterozoic-aged thin to thick-bedded metasandstone, metaconglomerate, and metavolcanic rock. In-place chemical and physical weathering of parent rocks typically form residual soils in the Piedmont region. Weathering is facilitated by the presence of fractures, joints, and less resistant minerals in the rock. In areas not altered by erosion, alluvial deposition or anthropomorphic, the typical residual soil profile consists of clay-rich unconsolidated materials at the surface, transitioning to sandy silts and silty sands at depth. Typically, the boundary between soil and rock is not sharply defined. This transitional zone is termed “partially weathered rock” or saprolite. Information obtained from the Web Soil Survey for Stanly County depicts soils on the site as Kirksey silt loam, 0 to 6 percent slopes (Kkb). In the Piedmont, groundwater generally occurs in pore spaces within soils and in the structural features present in the underlying rock (i.e., joints, fractures, and faults). Recharge to the water table is primarily by precipitation infiltrating the surficial soils and percolating downward under the influence of gravity to the water table. Typically, the water table is not a level surface, but an approximate reflection of the land surface. The depth to the water table is not consistent and is dependent upon several factors that include the amount of rainfall, permeability of the soil, the extent of fracturing in the underlying rock, and the influences of groundwater pumping. The direction and movement of groundwater is generally dependent on soil type, the presence of relict structures, and the textures of surrounding earth materials. Fractures, faults, folds, and foliation planes affect the migration of groundwater in rock. No major geologic features (faults, etc.) are present at or near the site. Groundwater typically flows in directions parallel to the ground Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 2 surface, and under the influence of gravity, migrates to discharge points such as surface water features, the toes of slopes, or natural springs. Fluctuations in the depth of the water table can be expected depending on variations in precipitation, surface water run-off and other factors. Normally, the highest levels of the water table occur in late winter and spring and the lowest levels occur in the late summer and fall. Based on historical data and June 7, 2023, water level measurements, groundwater at the site is expected to flow generally to the west and southwest. 1.3 Surface Water Two unnamed tributaries are located approximately 715 feet (ft) west and 700 ft northeast of the source area (see Figures 1 and 6). The stream to the west drains into Stony Run Creek, while the stream to the northeast drains into Big Bear Creek. 1.4 Water Supply Wells Based on a previous receptor survey conducted by others, a total of seven water supply wells are located within 1,000 ft of the source area, four of which (SW-1 through SW-4) are located within 500 ft of the source area (see Table 1 and Figure 6). Two water supply wells (SW-1 and SW-3) are located hydraulically downgradient of the source area. Both SW-1 and SW-3 are inactive, and the associated properties are connected to municipal water. Based on contact with the PW-4 property owner’s daughter on April 24, 2023, when requesting access to sample the water supply well, it was learned that PW-4 was inactive, and the property is connected to municipal water. Although municipal water is available to the area surrounding the site, water supply well PW-2 is reportedly still used for potable water. As of the most recent water supply well survey update, municipal water was reportedly not available to the SW-5 property. Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 3 2.0 SITE HISTORY The following description of historical site activities was obtained from a report titled “Subsequent Monitoring Report, Becky’s Grocery - Incident No. 3430, 20518 Ridgecrest Road, Locust, Stanly County, North Carolina” dated February 3, 2022, by Terracon Consultants Inc. (Terracon):  According to a Report of Phase I Review and Investigation by EA Engineering, Science and Technology, Inc. (EA) dated January 1989, a 1,000-gallon gasoline underground storage tank (UST) was removed in January 1986 due to noted product inventory losses. After the tank was removed, corrosion holes and pitting were observed on the surface of the tank. Reportedly, a new tank was installed at the site as a replacement, and soils excavated during the removal of the previous tank were used as backfill.  In May 1987, gasoline constituents (free product) were reportedly detected in a nearby water supply well (Hatley’s) located about 70 feet northeast of the source area. In 1987, the North Carolina Department of Environmental Quality (NCDEQ) listed the owner of the UST and Becky’s Grocery as Mr. Vernon Hatley. The report also identified the tank as a 1,000-gallon steel gasoline UST approximately 20 years old. In August 1988, EA and NCDEQ personnel reportedly collected groundwater samples from the on-site well and three nearby off-site water supply wells (C. Whitley, L. Burris, V. Hatley and V. Eudy). Analytical results from the C. Whitley and L. Burris wells indicated petroleum compounds were not detected above the method detection limits (MDLs). Analytical results from the V. Hatley and V. Eudy wells detected petroleum constituents.  In June 1989, EA conducted a soil vapor assessment in the vicinity of the UST basin of Becky’s Grocery. Analytical results were below the detection limits. EA also installed eight groundwater monitoring wells to evaluate the release.  In 1991, the NCDEQ submitted a letter to Mr. Hatley informing him that analytical results from a 1990 sampling event indicated high levels of petroleum compounds were detected in his water supply well, and the NCDEQ recommended water from the well not be used for any purpose. An activated carbon adsorption water treatment system was installed at the Hatley well. Subsequent sampling of the well after the treatment system was installed did not detect petroleum constituents.  In May 1993, Delta Environmental Consultants, Inc. collected groundwater samples from water supply wells on the properties adjacent to Becky’s Grocery. Analytical results from the T. Eudy, C. Whitley and V. Hatley wells indicated petroleum constituents were not detected. Analytical results from the Donnie J. Whitley well detected petroleum constituents below the drinking water standards. Analytical results from the L. Burris and V. Eudy wells detected petroleum constituents exceeding the maximum contaminant levels (MCLs) for drinking water.  In 2003, Stanly County Utilities installed a municipal water line along Ridgecrest Road allowing residences adjacent to Becky’s Grocery to connect to municipal water supplies. In February 2005, water supply wells SW-1 and SW-3 (see Table 1 and Figure 6) were removed from service, and the residences were connected to municipal water.  In April 2006, Hart & Hickman (H&H) collected groundwater samples from various water supply wells and the eight on-site monitoring wells. H&H also conducted a metal detector Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 4 survey to determine the presence of USTs and to observe fill ports, vent pipes or other evidence of existing USTs which were not previously discovered. Analytical results indicated low levels of methyl tert-butyl ether (MTBE) were only detected in water supply well SW-4. Various petroleum constituents were detected in the monitoring wells located near the source area above the North Carolina Groundwater Quality Standards 15A NCAC 2L .0202 (2L Standards) including benzene above its gross contaminant level (GCL). Groundwater flow direction was estimated to the west. In May 2008, H&H collected groundwater samples from various water supply wells in the area of the site. Analytical results indicated petroleum constituents were not identified in the water supply wells above the MDLs.  In September 2012, Crawford Environmental Services (CES) collected groundwater samples from seven of the eight on-site monitoring wells (MW-3 could not be located) and three water supply wells (SW-4, SW-5 and SW-13). CES also conducted a receptor survey. Analytical results indicated various petroleum constituents were detected in the monitoring wells located near the source area above the 2L Standards including benzene above its GCL in MW-4. MTBE and sec-butylbenzene were detected in water supply well SW-4 below the 2L Standards.  In January 2014, S&ME collected groundwater samples from six of the eight monitoring wells (wells MW-3 and MW-7 could not be located) and water supply wells SW-2, SW-4 and SW 5. Analytical results indicated various petroleum constituents were detected in the monitoring wells located near the source area above the 2L Standards including benzene above its GCL in well MW-4. Petroleum constituents were also reported in water supply well SW-4 below 2L Standards.  In March 2015, S&ME collected groundwater samples from monitoring wells MW-4, MW 5, MW-6, MW-7 and MW-8 and water supply wells SW-2, SW-4 and SW-5. Analytical results indicated various petroleum constituents were detected in the monitoring wells located near the source area above the 2L Standards including benzene above its GCL in well MW-4. MTBE and sec-butylbenzene were detected in supply well SW-4 below the 2L Standards.  In October 2016, S&ME collected groundwater samples from monitoring wells MW-4, MW 5, MW-6 and MW-8 and water supply wells SW-2, SW-4, SW-5 and SW-12. Analytical results indicated various petroleum constituents were detected in the monitoring wells located near the source area above the 2L Standards including benzene above its GCL in MW-4. MTBE was detected in water supply wells SW-4 and SW-12 below the 2L Standards. Wells MW-1, MW-2, MW-3 and MW-7 were not located.  In March 2018, S&ME collected groundwater samples from monitoring wells MW-4, MW 5 and MW-6 and water supply wells SW-2, SW-4, SW-5 and SW-12. Analytical results indicated various petroleum constituents were detected in the monitoring wells located near the source area above the 2L Standards. None of the constituents exceeded the GCLs. Sec- butylbenzene was detected in water supply well SW-4 and MTBE was detected in water supply well SW-12, both below the 2L Standards. Wells MW-1, MW-2, MW-3, MW-7 and MW-8 were not located.  In June 2019, S&ME collected groundwater samples from monitoring wells MW-4, MW-5 and MW-6 and water supply wells SW-2, SW-4 and SW-12. Analytical results indicated various petroleum constituents were detected in the monitoring wells located near the source Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 5 area above the 2L Standards including benzene above its GCL in wells MW-4 and MW-6. Petroleum constituents were reported in water supply wells SW-2, SW-4 and SW-12, including bromodichloromethane above its 2L Standard in SW-4.  In December 2019, S&ME installed monitoring well MW-9 near the former dispenser island and sampled the well on January 8, 2020. Analytical results indicated 1,2-dibromoethane (EDB) was detected in the MW-9 above its GCL while other petroleum compounds exceeded the 2L Standards.  On December 30, 2020, Terracon mobilized to conduct a groundwater sampling event on monitoring wells MW-1 through MW-9 and water supply wells SW-2, SW-4, SW-5, SW-12, and SW-13. MW-1, MW-2, and MW-7 could not be located; and MW-8 appeared to have been destroyed. Terracon was unable to access MW-3 due to animals on the well and an electric fence surrounding the property. Terracon was only able to gain access to water supply well SW-13. Multiple petroleum compounds exceeding 2L Standards were found in monitoring wells MW-4, MW-5, MW-6, and MW-9. No petroleum compounds were found exceeding the GCLs. No petroleum constituents were detected above the MDLs in SW-13.  On January 6, 2022, Terracon mobilized to conduct a groundwater sampling event on monitoring wells MW-3 through MW-7 and MW-9 and water supply wells SW- 2, SW-5, SW-12 and SW-13. Monitoring well MW-7 was not located during the sampling event, and Terracon did not receive permission to access water supply well SW-12. Based on the water level measurements and elevation data collected during this event, groundwater flow was estimated to be towards the south-southwest. No free product was present in the on-site wells. Multiple petroleum compounds exceeding 2L Standards were found in wells MW-4, MW-5, MW-6, and MW-9, including benzene above its GCL No constituents of concern were detected in monitoring well MW-3 or water supply wells SW-2, SW-5, and SW- 13 above MDLs. Historical groundwater elevations and analytical results are provided in Appendix A. Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 6 3.0 FIELD ACTIVITIES Field activities performed by ESP on June 7, 2023, included:  Obtaining an access agreement to sample water supply well SW-5 (see Appendix B). Although planned, an access agreement to sample water supply well SW-4 was not obtained; the well owner’s daughter reported that SW-4 is no longer used, and the property is connected to municipal water (see Appendix B).  Gauging static water levels in monitoring wells MW-3 through MW-6 and MW-9.  Collecting groundwater samples from monitoring wells MW-3 through MW-6 and MW-9, including a field duplicate.  Collecting groundwater samples from water supply wells SW-2 and SW-5. Although planned, ESP was unable to obtain a groundwater sample from water supply well SW-4 because access approval was not given and the well was reportedly no longer in use (as stated above). Sample locations are depicted on Figures 2 and 6. Field logbook entries along with water level survey forms, groundwater sampling forms, and field instrument calibration forms were completed in the field to document site activities (see Appendix C). Photographs are also provided in Appendix C. Details of field activities are provided below. 3.1 Water Level Gauging On June 7, 2023, prior to purging or collecting groundwater samples, depth to groundwater from the top of casing (TOC) within each monitoring well was measured and recorded using a decontaminated electronic water level meter. The water level measurements were collected by first removing covers and plunger caps from all site monitoring wells and allowing water within the wells to equilibrate. The electronic water level meter probe was then lowered into the well and the depth to water measured relative to the well’s TOC. As noted on Table 2, static water level depths ranged from 8.14 ft to 14.50 ft below top of casing (ft bTOC). Figure 3 depicts the groundwater elevations based on an unknown benchmark established by a previous consultant. As depicted on Figure 3, groundwater flow beneath the site is generally toward the west and southwest. Free product was not present in any of the gauged on-site monitoring wells. 3.2 Monitoring Well Sampling On June 7, 2023, groundwater samples were collected from monitoring wells MW-3 through MW 6 and MW-9. Groundwater samples were collected using dedicated and disposable polyvinyl chloride (PVC) bailers (1.5” x 3’) attached to braided nylon rope. Prior to groundwater sample collection, each well was purged using the bailer. During purging, water quality parameters including pH, conductivity, temperature, and turbidity were measured using calibrated field water quality instruments. Readings were taken at the start of purging and periodically until a minimum of three well volumes were removed from the well. The water quality parameters were recorded Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 7 on groundwater sampling forms along with time, water level depth, and cumulative volume of water purged (see groundwater sampling forms in Appendix C). Following purging, a groundwater sample for laboratory analysis was collected from the well using the bailer. Groundwater samples were collected following Guidelines for Sampling by the UST Section, Division of Waste Management, NCDEQ (NCDEQ, 2022). Groundwater samples were placed in laboratory-prepared vials preserved with hydrochloric acid (HCl), labeled, and stored on ice in a cooler for subsequent delivery under standard chain-of-custody (COC) procedures to Pace Analytical (Pace), a North Carolina-certified laboratory, located in Huntersville, North Carolina, for analysis. The groundwater samples were analyzed for volatile organic compounds (VOCs) by EPA Method 6200B including MTBE, EDB, and diisopropyl ether (IPE). 3.3 Water Supply Well Sampling On June 7, 2023, groundwater samples were collected from water supply wells SW-2 and SW-5. SW-2 was obtained from a spigot directly connected to the well, and SW-5 was obtained from a spigot connected to the house on the same property as the well. Prior to sampling, the wells were purged by allowing water to flow from the open spigot for 15 minutes. During purging, water quality parameters including pH, conductivity, temperature, and turbidity were measured using calibrated field water quality instruments. Readings were taken at the start of purging and periodically until fifteen minutes had elapsed. Following purging, a groundwater sample for laboratory analysis was collected directly from the spigots. It should be noted that at the time of collection, water supply wells SW-2 and SW-5 were referred to as WSW-2 and WSW-5 on the field forms (See Appendix C). Groundwater samples were placed in laboratory-prepared vials preserved with HCl, labeled, and stored on ice in a cooler for subsequent delivery under standard COC procedures to Pace for analysis. The groundwater samples were analyzed for VOCs by EPA Method 6200B including MTBE, EDB, and IPE. 3.4 Field Quality Control Sampling and Analysis A field duplicate groundwater sample was collected on June 7, 2023, from MW-6 and analyzed for VOCs by EPA Method 6200B including MTBE, EDB, and IPE to assess variance between collected samples. Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 8 4.0 ANALYTICAL RESULTS Results of the laboratory analysis of groundwater samples from monitoring wells MW-3 through MW-6 and MW-9 and water supply wells SW-2 and SW-5 are tabulated in Table 3. As noted in Table 3, the analytical results are compared to both 2L Standards and GCLs. The Pace laboratory analytical report is provided in Appendix D. One analyte was detected at a concentration equal to its GCL in one monitoring well and various analytes were detected at concentrations greater than their respective 2L Standards in four of the sampled monitoring wells (see Table 3 and Figures 4 and 5). A summary of the analytical results is as follows:  Benzene was detected at its GCL of 5,000 micrograms per liter (µg/L) in monitoring well MW-4. Benzene was also detected above its 2L Standard of 1 µg/L in monitoring wells MW-5 (196 µg/L), MW-6 (1,470 µg/L), and MW-9 (26.7 µg/L).  Various other analytes were detected above their 2L Standards in monitoring wells MW 4, MW-5, MW-6, and MW-9.  No analytes were detected above MDLs in monitoring well MW-3 or water supply wells SW-2 and SW-5. A comparison of groundwater laboratory analytical results from the June 7, 2023, sampling event to previous sampling events reveals that the general trend in groundwater contaminant concentrations at the site has been decreasing since 2019. Subsequent Monitoring Report ESP Project No. 23-00200-005 Becky’s Grocery (Incident #3430) October 3, 2023 9 5.0 CONCLUSIONS AND RECOMMENDATIONS Based on results of the above field activities and previous reporting, ESP concludes the following:  Static water level depths ranged from 8.14 to 14.50 ft bTOC.  No measurable free product was observed in any of the monitoring wells.  The general direction of groundwater flow beneath the site is to the west and southwest.  Four water supply wells (of which only one [SW-2] is active) are located within 500 ft of the source area. SW-2 is located hydraulically cross-gradient of the source area. Municipal water is available to the SW-2 property but not connected.  Benzene was detected at its GCL in monitoring well MW-4.  Multiple constituents exceeded their respective 2L Standards in monitoring wells MW-4, MW-5, MW-6; and MW-9.  No analytes were detected above MDLs in monitoring well MW-3 and water supply wells SW-2 and SW-5.  In general, contaminant concentrations at the site have decreased since 2019. Based on the above results, ESP offers the following recommendations:  Continue to sample monitoring wells MW-4, MW-5, MW-6, and MW-9 along with water supply wells SW-2 and SW-5 on a semiannual basis to further understand concentration trends and plume stability; and  Evaluate results of the Remedial Design Characterization (RDC) event that was conducted by AST Environmental on June 6, 2023, and implement BOS-200 injection, if applicable.
