Services were authorized by the North Carolina Department of Environmental Quality (NCDEQ) Task Authorization (TA) #02, accepted September 4, 2023, and Contract Number N70522-A. The approved scope of services for this Task Authorization included: • Abandon 12 monitoring wells (MW-1 through MW-6, MW-06R, MW-7, P-1 through P-3, and T-1); and • Prepare a letter report documenting the site activities. Monitoring Well Abandonment On October 11, 2023, CATLIN personnel mobilized to the site and abandoned monitoring wells MW-1, MW-3, MW-4, MW-5, MW-06R, P-1, P-2, P-3, and T-1. Monitoring wells MW-2, MW-6, and MW-7 were not able to be located and are presumed destoryed. The remainder of the approved scope of work was completed. Monitoring wells MW-1, MW-3 through MW-06R, T-1, and P-1 through P-3 were abandoned under the supervision of a North Carolina Certified Well Contractor (CWC) according to guidance provided in the North Carolina Administrative Code (NCAC) Title 15A 02C .0113 (Abandonment of Wells). All wells were abandoned in place and packed with bentonite chips to within six inches of the surface while simultaneously pouring water to facilitate hydration. The remaining space was filled with concrete (MW-1, MW-4, T-1, P-1, P-2) or topsoil (MW-3 MW-5, MW-06R, and P-3) to the existing grade. The site and well locations are shown on Figure 1 and Figure 2, respectively (Attachment A). The well abandonment records were submitted to the NCDEQ Division of Water Quality and copies are provided in Attachment B. Photographs of the abandoned wells are included in Attachment C.
