Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx 1.0 INTRODUCTION On behalf of Sirron Preservation, LLC, HRP Associates Engineering and Environmental Management, PLLC, a Connecticut limited liability company, doing business in North Carolina as HRP Associates Engineering and Environmental Management, PLLC, referred to herein as HRP, has completed a Limited Phase 2 Environmental Site Assessment (ESA) of the Former Stamey Stores, Inc. Property located at 4726 Fallston Road in Fallston, North Carolina (herein referred to as the “Site”). A Site location map is included as Figure 1. The Site consists of an approximately 1.48-acre parcel of land identified by the Cleveland County Tax Assessor as Parcel 34895. The Site is developed with one (1) approximately 15,400 square foot Site building constructed in 1925. The Site building is of steel frame construction supported by masonry brick walls over a raised foundation. The building was most recently operational as a department store and is currently undergoing renovation for the purpose of operating as an event venue. The remainder of the Site consists of a paved parking lot. On June 25, 2018, HRP conducted a Phase 1 Environmental Site Assessment (ESA) on the Site to assess potential environmental issues. The Phase 1 ESA indicated the following:  The Site building was constructed in 1925 and has historically operated as a department store, butchery, and as a furniture retailer.  The Site was listed on the leaking underground storage tank (LUST) database due to the former operation of a 550-gallon underground storage tank (UST) utilized for the retail sale of kerosene. The UST is located adjacent to the southern exterior wall of the Site building and was installed on March 7, 1946 according to the North Carolina Department of Environmental Quality (NCDEQ) UST Database. A release from the kerosene UST was discovered during soil sampling activities for an in-place UST closure on December 8, 1993. HRP accessed the NCDEQ’s (formerly NCDEHNR & NCDENR) Laser fiche online file database and contacted Mr. Mike Streeter, Hydrogeologist for the NCDEQ UST Division based out of Mooresville, NC to obtain information relating to historical assessment activities. HRP reviewed multiple files including a document titled “Extent of Soil Contamination, Groundwater Analysis, Site Sensitivity Evaluation” by Southeastern Geological Services (SGS) and dated July 13, 1994. The document indicated that five (5) borings were advanced at the Site, one of which was converted to a permanent groundwater monitoring well, between May 31 and June 1, 1994. Field screening results indicated that kerosene had impacted approximately 200 cubic yards of soil from the UST (situated approximately 5 feet below grade) to the water table located approximately 22 feet below the ground surface. The contaminated soil was noted to extend radially around the UST, and that approximately one-third of the estimated contamination plume was located beneath the Site building. Further, the concentrations of benzene, ethylbenzene, xylenes, and naphthalene detected in groundwater exceeded regulatory limits. A “Corrective Action Plan” (CAP) dated April 1995 prepared by SGS recommended utilizing air sparging combined with vapor extraction as a viable means for remediating the Site. The CAP was approved by NCDENR in 1995; however, multiple setbacks occurred in getting the Limited Phase 2 ESA Sirron Preservation, LLC 4726 Fallston Road Fallston, NC Page 2 of 10 H:\S\Sirron Preservation\SIR8000.P2 - 4726 Fallston Rd, Fallston NC P2\05. Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx system operational at the Site. No documents stating that the remediation was successfully setup were identified. However, a correspondence letter dated February, 1997 from SGS to the NCDENR requested a No Further Action (NFA) designation based on a change in regulatory standards. The NCDENR replied with a request to sample the five (5) groundwater wells located on the Site, collect and analyze one (1) soil sample, and submit an updated water well survey. SGS complied with the requests and submitted a final groundwater monitoring report on July 5, 1998. On July 23, 1998, the NCDENR issued a “Notice of NFA” letter to the Site. The NFA letter noted that the soils at the Site did not exceed industrial/commercial criteria, and that the groundwater results were below their applicable standards. The NFA letter further stated that “because contaminated soil was not cleaned up to the residential maximum soil contaminant concentrations, the property containing soil contamination is not suitable for residential use.” The reported remaining soil and groundwater contamination in connection with land use restrictions for residential use is considered a Conditional Recognized Environmental Condition (CREC).  Two (2) adjacent and up-gradient properties with confirmed UST releases were identified to have historically impacted the Site. Recent assessment activities indicate that petroleum constituents are still present above regulatory standards on the western adjoining properties. The purpose of this Phase 2 ESA was to assess soil and groundwater in the area of the on-Site UST and to assess groundwater down-gradient of the off-Site LUST facilities. Work conducted by HRP in connection with the performance of this Limited Phase 2 ESA was performed in accordance with the HRP proposal dated August 1, 2018, authorized on August 9, 2018. HRP’s proposal provided the guidelines with respect to evaluation of the areas of environmental concern. It should be noted that the westernmost boring (HA-1) could not be advanced to groundwater due to the inability to use the drill rig with the limited clearance allowed by the awning on the western side of the Site building. Additionally, the groundwater monitoring well located southwest of the Site building (MW-7) could not be sampled due to the well being dry. These limitations are not anticipated to affect our assessment of this area as the remaining borings and wells were successfully sampled. This report presents information regarding the following tasks:  Utility Markout and a Ground Penetrating Radar (GPR) Survey;  Limited Subsurface Soil and Groundwater Investigation; and,  Data Summary and Evaluation. Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx 2.0 UTILITY MARKOUT AND GROUND PENETRATING RADAR SURVEY The following section provides information on how subsurface utilities were located. 2.1 Utility Mark-out North Carolina 811 (a utility protection service) was contacted to identify public underground utilities at the Site prior to initiation of the subsurface investigation. HRP contacted North Carolina 811 on August 9, 2018 and was issued ticket # A182210995. As a result, eight (8) utility providers were notified to mark their assets, if present, on the Site. 2.2 Ground Penetrating Radar A GPR survey was performed at the Site prior to drilling activities to protect unidentified subsurface utility lines and other unforeseen subsurface obstructions. GPR involves scanning the subsurface with a device that emits and records the return signals of electromagnetic energy in the radar wavelength. HRP representatives established a survey grid of the anticipated areas of investigation in an attempt to identify utility lines or other buried objects located in the area prior to advancement of soil borings. A site-wide GPR survey was not completed as the survey grid was limited to the approximate location of each boring. No unusual anomalies were identified during the GPR survey. At the time of the GPR Survey, an approximately 2 foot wide by 2 feet deep excavation area located adjacent to the southern wall of the Site building was observed. The excavation area was reportedly constructed for the installation of new plumbing lines for the Site building. The excavation debris was observed to be in the area of the abandoned-in-place UST; as well as in an area of a proposed boring. Therefore, the GPR was unable to accurately locate the area of the UST. Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx 3.0 LIMITED SOIL AND GROUNDWATER INVESTIGATION The following section outlines the methods used to investigate the subsurface. 3.1 Soil Boring Investigation On August 19, 2018, one (1) soil boring and two (2) hand auger borings were advanced on the Site to assess soil and groundwater conditions. The location of each of the borings is shown on Figure 2. A track-mounted Geoprobe® 7822DT drill rig was used to install the soil boring (SB-1) in an up- gradient location. Continuous soil samples were collected beginning at ground surface in four- foot HDPE liners during boring advancement. A manually-operated hand auger was used to collect soil samples at each of the two (2) hand auger borings (HA-1 and HA-2). Each soil-core was screened for volatile organic vapor at one- or two-foot intervals using a MiniRae 3000 photo-ionization detector (PID). Soil logs are included in Appendix A. Table 1 provides a description of each location, boring depth, whether groundwater was encountered, and the maximum PID reading at each location. A brief description of each boring is provided below:  SB-1: located in the southern portion of the site, up-gradient area, highest PID detection of 0.6 ppm, no odor was noted in the soil, one (1) soil and one (1) groundwater sample were collected based on its up-gradient location;  HA-1: located west of the Site building, highest PID detection of 0.9 ppm, no odor was noted in the soil, one (1) soil sample was collected to assess soil quality;  HA-2: located adjacent to the southern exterior wall in the area of the UST, highest PID detection of 909.1 ppm, heavy petroleum odor was noted in the soil, one (1) soil sample was collected to measure level of petroleum contamination; According to the United States Department of Agriculture (USDA), the soils at the Site consist primarily of Cecil sandy clay loam (USDA 2018). Subsurface observations during the field investigation indicated that soils at the Site consist of primarily sandy fat clay and silty sand. Bedrock was not encountered at the Site. Boring logs are provided in Appendix A. Based on the topographic map presented as Figure 1, the ground surface slopes to the northeast towards the Stillwater River. Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx 3.2 Groundwater Investigation Groundwater was encountered in one (1) direct-push boring and within three (3) existing permanent (MW-4, SE-MW-4, and MW-5) monitoring wells at depths ranging from 23.56 ft below ground surface (bgs) to 34.08 ft bgs. The direct-push boring was converted to a temporary groundwater monitoring well identified as “TMW-1.” The existing permanent monitoring wells were identified with the “MW” nomenclature as identified in Figure 2. The temporary monitoring well was constructed of a 1-inch PVC riser with a 10-foot screen. The existing permanent monitoring wells were constructed of a 2-inc PVC riser with a 5-foot screen. One (1) groundwater sample was collected from the temporary monitoring well using a low-flow sampling technique with a peristaltic pump. Groundwater samples were collected from MW-4, SE-MW-4, and MW-5 well using a volume purge technique with a typhoon pump. Water quality parameters pH, conductivity, temperature, and turbidity were measured prior to sample collection. The Well Installation Log is included with HRP’s Field Data Sheets, included in Appendix A. Groundwater flow direction was inferred to be to the northeast based on topography and Site observations. A calculated groundwater flow gradient and flow direction could not be determined based on Site conditions and lack of a Site survey. 3.3 Boring Abandonment Following the collection of all soil and groundwater samples, each of the three (3) borings drilled by HRP was abandoned in accordance with the North Carolina well abandonment standards. 3.4 Investigation Derived Wastes Soil cuttings and purge water generated during this assessment were temporarily staged on- Site in one 55-gallon steel drum and positioned on the eastern Site boundary. A label was placed on the drum noting its contents. The drum was sampled and proper disposal is pending. The laboratory analytical reports are included in Appendix B. 3.5 Equipment Decontamination Reusable down-hole investigative equipment was decontaminated before first use and between sampling locations. Disposable HDPE liners were used to collect soil during borehole advancement. Samples were collected from the cores using disposable nitrile gloves and all appropriate measures were taken to ensure the proper collection and handling of samples in order to provide representative Site data. Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx 4.0 ANALYTICAL RESULTS The following section describes the analytical procedures and data generated during the Limited Phase 2 ESA. The laboratory analytical reports are included in Appendix B. 4.1 Soil Analytical Results A total of three (3) soil samples were submitted under chain-of-custody to a laboratory certified in North Carolina and analyzed for the following constituents:  Volatile Organic Compounds (VOCs list) via EPA Method 8260B; and  Polycyclic Aromatic Hydrocarbons (PAHs) via EPA Method 8270 SIM Analytical data from the soil samples were compared to the Environmental Protection Agency’s (EPA) Residential Soil Screening Levels (SSL) and the North Carolina Department of Environmental Quality (NCDEQ) Maximum Soil Contaminant Concentration Levels (MSCCs) (NCDEQ 2017). HRP believes that these screening criteria are appropriate for determining environmental risk at the Site related to past releases at the Site and adjoining properties. Table 2 provides a tabulation of the constituents detected above their laboratory detection limit in each soil sample.  VOCs: A total of 12 VOCs were present above the laboratory detection limit in the three (3) soil samples collected. Naphthalene was detected at a concentration exceeding the NCDEQ MSCC in the sample collected from HA-2.  PAHs: A total of seven PAHs were present above the laboratory detection limit in the three (3) soil samples collected. However, none of the PAHs exceeded either the NCDEQ MSCCs or the US EPA Residential SSLs. The concentration of naphthalene from the VOC analysis in the sample collected from HA-2 was 7,120 micrograms per liter (µg/kg), greater than the US EPA Residential SSL of 3,800 µg/kg. This sample was collected from a depth of 7-8 feet bgs and was located adjacent to the abandoned-in-place UST. Naphthalene was also detected in the PAH analysis of this sample, at a concentration of 334 µg/kg. The VOC result is considered more accurate. No other exceedances were detected. 4.2 Groundwater Analytical Results A total of four (4) groundwater samples were submitted under chain-of-custody to a laboratory certified in North Carolina and analyzed for the following constituents:  VOCs via EPA Method 8260B; and  PAHs via EPA Method 8270 SIM. Table 3 provides a summary of analytical data from the groundwater samples, conservatively compared to NCDEQ 15A NCAC 02L Groundwater Standards and the EPA’s Maximum Limited Phase 2 ESA Sirron Preservation, LLC 4726 Fallston Road Fallston, NC Page 7 of 10 H:\S\Sirron Preservation\SIR8000.P2 - 4726 Fallston Rd, Fallston NC P2\05. Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx Contaminant Level (MCL). HRP believes that these screening criteria are appropriate for determining environmental risk at the Site related to past releases at the Site and adjoining properties.  VOCs: A total of nine VOCs were present above the laboratory detection limit in the four (4) groundwater samples collected. Benzene, 1,2-Dichloroethane, methyl tert-butyl ether (MTBE), and naphthalene were detected at concentrations exceeding the NCDEQ 2L groundwater standards in the sample collected from MW-5. Chloromethane was detected at a concentration slightly greater than the NCDEQ 2L groundwater standard within TMW-1.  PAHs: PAHs were not present above detection limits in samples collected from TMW-1, MW-4, and SE-MW-4. A total of four (4) PAHs (anthracene, 1-methylnaphthalene, 2- methylnaphthalene, and naphthalene) were present above the laboratory detection limit in MW-5. Of these, 2-methylnaphthalene and naphthalene were detected at concentrations exceeding the NCDEQ 2L groundwater standards. No other exceedances were observed. 4.3 Quality Assurance/Quality Control Field instruments used as part of this assessment were calibrated in accordance with HRP’s internal quality control procedures and laboratory certification requirements. Typical laboratory quality assurance/quality control (QA/QC) procedures were conducted and included a laboratory blank, standard, duplicate, and spike samples. The results of the laboratory QA/QC samples indicate that the laboratory data for the submitted soil and groundwater samples are considered reliable, valid and sufficient for the purpose of this assessment. Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx 5.0 CONCLUSIONS A Limited Phase 2 Environmental Site Assessment was performed at the former Stamey Stores, Inc. Property on August 19, 2018 to assess soil and groundwater in the area of the on-Site UST and to assess groundwater down-gradient of the off-Site LUST sites. Based upon the results of this investigation, HRP concludes the following:  Three soil borings were advanced at the Site, one of which was converted to a temporary monitoring well. One soil sample was collected from each boring and analyzed for VOCs and PAHs. Three existing permanent monitoring wells located on the Site and one temporary monitoring well were sampled for VOCs and PAHs.  A total of seven PAHs and twelve VOCs were present above the laboratory detection limit in the three soil samples collected. No PAHs were detected in concentrations exceeding the NCDEQ MSCCs or the US EPA Residential SSLs; however, one constituent (naphthalene, as a VOC) was detected at a concentration exceeding the US EPA Residential SSL in the sample collected from HA-2.  Thirteen constituents were detected in one of the groundwater samples (MW-5) collected at the Site. Two PAHs (2-methylnaphthalene and naphthalene) were present at concentrations exceeding the NCDEQ 2L groundwater standards. The remaining three groundwater samples did not contain any PAHs at concentrations above method detection limits.  A total of nine VOCs were present above the laboratory detection limit in the four groundwater samples. Benzene, 1,2-Dichloroethane, MTBE, and naphthalene were detected in exceedance of the NCDEQ 2L groundwater standards in the sample collected from MW-5. Chloromethane was detected in exceedance of the NCDEQ 2L groundwater standard in the sample collected from TMW-1. The remaining two groundwater samples did not contain any constituents at concentrations above reporting limits. The soil data obtained during this Limited Phase 2 ESA is generally consistent with historical data that formed the basis of the No Further Action (NFA) designation that was issued by NCDEQ in 1998. The groundwater data indicates that VOC and PAH concentrations have increased in well MW-5, which is reportedly a bedrock well, since the 1998 sampling event conducted by Southeastern Geological Services (Southeastern Geological Services, 1998). It is important to note that the up-gradient property (“Lewis Service Station”) continues to have significantly high concentrations of VOCs in groundwater, with benzene as high as 5,900 µg/L as recently as August 2017. A report prepared by Terracon states that constituent concentrations appear to be increasing at the Lewis Service Station property and that the plume appears to be migrating vertically downward (Terracon, 2017). Reports\Sirron Preservation Fallston NC P2 Report 9 20 18.docx Based on the above, HRP infers from the minimal amount of data obtained during this Limited Phase 2 ESA that the impacted groundwater in the vicinity of MW-5 is attributable to the up- gradient/off-site plume and is not related to the on-site kerosene UST that was closed in 1993.
