According to the laboratory analytical report,EDB (120 µg/L)was detected in well MW-1R at a concentration exceeding the NCAC 15A 2L Standard and the Gross Contamination Level (GCL). The following analytes were detected in well MW-1R at concentrations above the 2L standards:2- hexanone (1,600 µg/L), MIBK (270J µg/L), MTBE (230 µg/L), naphthalene (150 µg/L), 1,2,4- trimethylbenzene (2,800 µg/L), 1,3,5-trimethylbenzene (960 µg/L), and total xylenes (10,900 µg/L). Various other constituents were detected in well MW-1R at concentrations below the 2L standards. Petroleum constituents were not detected in wells MW-2 through MW-6 at concentrations exceeding the laboratory reporting limits. Based on information obtained during the completion of these assessment activities, Terracon provides the following conclusions and findings. Several petroleum constituents were detected at concentrations exceeding the 2L standards in well MW-1R. EBD was detected at a concentration exceeding the GCL in well MW-1R. Petroleum constituents were not detected at concentrations exceeding the laboratory reporting limits in wells MW-2 through MW-6. The estimated groundwater flow direction is to the north. Light non-aqueous phase liquids (LNAPL) was not detected in the wells at the site. Based on the findings of our assessment activities, the following recommendations are provided for the site: Terracon recommends an Aggressive Fluid Vapor Recovery (AFVR) event be performed on well MW-1R to reduce petroleum constituent concentrations in the groundwater. Sample on-site monitoring wells MW-1R, MW-3 and MW-5 on a semi-annual basis. C.SITE HISTORY AND CHARACTERIZATION The purpose of this report is to evaluate the current status of the groundwater quality at the Parrish Grocery site (Incident No. 11953)located at 6090 Pine Hall Road in Walnut Cove, Forsyth County, North Carolina (Exhibit 1).Terracon completed this report to summarize the findings of assessment Subsequent Monitoring Report Parrish Grocery ■Walnut Cove, NC ■Incident No. 11953 March 24, 2020 ■Terracon Project No.75179042 Responsive ■Resourceful ■Reliable 4 activities performed at the site on February 24 and 25,2020.This Subsequent Monitoring report was prepared for the site in response to a request by NCDEQ Incident Manager Mr. Herb Berger with the NCDEQ Division of Waste Management,UST Section,Trust Fund Branch. The Subsequent Monitoring Report was prepared in general accordance with the 15A NCAC 2L.0115 regulations and the NCDEQ,UST Section Guidelines for Assessment and Corrective Action for UST Releases, dated July 15, 2008, revised December 2013,under the supervision of Zachary R. G. Tidmore, a North Carolina registered professional geologist (North Carolina License No.2611). C.1 Site Setting The approximate 1.0-acre site is currently occupied by a single-family residence constructed in 1946 and a former grocery store building constructed in 1952 located in Walnut Cove, Forsyth County, North Carolina. The residence and store occupy the same parcel (Forsyth County Parcel No.6960- 92-1631). The vacant store building, located in the southwestern corner of the property was formerly used as a service station and grocery store from the 1950s until the 1990s. Based on field observations,the former USTs and fuel dispensers were apparently located in the southwestern portion of the site along Pine Hall Road.The site is depicted in Exhibit 2. C.2 Release Summary On March 4, 1992, G&H Oil Equipment Services removed one 1,000-gallon gasoline UST at the site. During the UST closure activities, two additional 550-gallon orphan USTs were discovered and were removed on May 14, 1992. Concurrently with the removal of the 550-gallon USTs, one new 1,000-gallon UST was installed in the former tank basin and the fueling systems for two existing 3,000-gallon USTs were upgraded in response to the UST regulations. According to the UST Closure Reports submitted by Pyramid Environmental, Inc. on April 9 and July 7, 1992, analytical results of the soil samples collected near the 550-gallon USTs indicated gasoline and diesel range petroleum hydrocarbons were present at concentrations exceeding the regulatory action levels. According to a UST Closure Report submitted by Progress Environmental, Inc. on January 19, 2005, the 1,000-gallon UST and two 3,000-gallon USTs were removed on January 4 and 5, 2005. Analytical results of closure soil samples collected beneath the USTs indicated gasoline and diesel range petroleum hydrocarbons were present at concentrations exceeding the regulatory action levels.Additional excavation of petroleum impacted soils (172.84 tons)was conducted during the closure activities and the soils were disposed at an off-site facility. Analytical results of risk-based confirmation soil samples indicated the presence of several petroleum related compounds at concentrations exceeding the corresponding NCDEQ Soil to Water MSCCs in the eastern portion of the excavation. Subsequent Monitoring Report Parrish Grocery ■Walnut Cove, NC ■Incident No. 11953 March 24, 2020 ■Terracon Project No.75179042 Responsive ■Resourceful ■Reliable 5 C.3 Previous Investigations According to a Phase I Limited Site Assessment Report submitted by Schnabel Engineering South (Schnabel) in October 2006, a soil boring was advanced at the approximate location of the former dispenser island.Soil samples were collected at depths of 10, 20, 30 and 40 feet below ground surface (bgs).Analytical results of the soil samples indicated the presence of several petroleum constituents at concentrations below the established Soil to Water MSCCs.Schnabel also measured approximately 0.10 feet of light non-aqueous phase liquid (LNAPL)in on-site monitoring well MW-1. On February 21, 2008, monitoring well MW-2 was installed to the west of the store building by Schnabel Engineering, Inc.Analytical results of groundwater samples collected from monitoring wells MW-1 (no LNAPL)and MW-2 indicated multiple petroleum constituents were detected at concentrations exceeding the 2L Standards and EDB exceeded the GCL in well MW-1. Tetrachloroethene (PCE)was also detected in on-site water supply well WSW-3B at 0.9 µg/L, slightly above the 2L Standard.The source of the PCE was not identified. According to a Monitoring Well Installation and Groundwater Sampling Report submitted by Terraquest on February 2, 2010, four Type II groundwater monitoring wells (MW-3, MW-4, MW-5 and MW-6) were installed in October 2009 to further characterize the extent of petroleum-impacted groundwater at the site.According to groundwater elevation data collected at that time, groundwater was determined to flow to the north-northeast.Well MW-1 was reportedly dry. Terraquest collected groundwater samples from wells MW-2 through MW-6 and off-site water supply wells WSW-2A and WSW-27 on October 29, 2009. The groundwater samples were analyzed for volatile organic compounds (VOCs) by EPA Method 6200B,total lead by EPA Method 3030C and 504.1, and volatile petroleum hydrocarbons (VPH) by the MADEP Method.Analytical results of the groundwater samples identified methyl tert-butyl ether (MTBE), isopropyl ether (IPE), chloromethane, and lead at concentrations exceeding the laboratory detection limits but below the 2L Standards. Chloromethane was also detected at concentrations below the 2L Standard in water supply wells WSW-2A and WSW-27. Terraquest remobilized to the site in November 2010 to collect groundwater samples from monitoring wells MW-1 through MW-6 and the two nearest off-site water supply wells (WSW-2A and WSW-27). The groundwater samples were analyzed for VOCs by EPA Method 6200B. Monitoring well MW-1 was reportedly dry at the time of sampling.Analytical results indicated the presence of MTBE in downgradient well MW-4 at 2.7 micrograms per liter (µg/L)below the 2L Standard. Petroleum constituents were not detected in monitoring wells MW-2, MW-3, MW-5 or MW-6 or water supply wells WSW-2A and WSW-27 above the laboratory reporting limits. Terracon mobilized to the site on July 18, 2018 to locate,gauge and sample the on-site groundwater monitoring wells and collect soil samples from three borings.Terracon located and gauged monitoring wells MW-2 through MW-6. LNAPL was not detected in the monitoring wells.Petroleum constituents were not detected at concentrations exceeding the laboratory limits. Terracon advanced Subsequent Monitoring Report Parrish Grocery ■Walnut Cove, NC ■Incident No. 11953 March 24, 2020 ■Terracon Project No.75179042 Responsive ■Resourceful ■Reliable 6 three soil borings (SB-1, SB-2 and SB-3) at the site in an effort to further delineate petroleum impacted soil beneath the site.Petroleum constituents were not detected in the soils at concentrations exceeding the laboratory detection limits. C.4 Water Supply Wells Terracon previously completed a receptor survey by walking and driving along the nearby streets in the vicinity of the site on February 14, 2018.Based on information from a prior receptor survey and information obtained from our site area reconnaissance and the Forsyth County GIS website, two on-site water supply wells (WSW-3A and WSW3B) were identified at distances approximately 65 feet and 180 feet northeast of the source area of the release. Three additional water supply wells were confirmed within 250 feet of the source area. Six additional wells were confirmed within 500 feet of the site. Fifteen wells were identified between 500 and 1,000 feet of the site, and 27 wells were identified between 1,000 and 1,500 feet of the site.Based on proximity to the source area of the release and groundwater gradient, Terracon considers wells within 500 feet of the source area to be at high risk.Water supply well information is included in Table 1.The locations of the water supply wells are depicted on Exhibit 3.According to the Town of Walnut Cove Water and Sewer Department, municipal water is provided to the site and surrounding properties along Pine Hall Road. C.5 Surface Water Based on our review of the USGS topographic map,the site is located in the drainage basin for Lick Creek which, at its closest point, is located approximately 1.6 miles north-northeast of the site. An unnamed, intermittent tributary of Lick Creek is located approximately 1,790 feet west of the site.A drainage feature is also located to the northeast of the site.Nearby surface water bodies are shown on Exhibit 1. C.6 Land Use Information The site is located in an area developed mostly as single-family residential. D.ASSESSMENT INFORMATION D.1 Site Geology and Hydrogeology The following resources were reviewed to evaluate site-specific geological and hydrogeological characteristics. Subsequent Monitoring Report Parrish Grocery ■Walnut Cove, NC ■Incident No. 11953 March 24, 2020 ■Terracon Project No.75179042 Responsive ■Resourceful ■Reliable 7 Soil Survey of Forsyth County, North Carolina, US Department of Agriculture, Natural Resources Conservation Service, Web Soil Survey via the Internet, http://websoilsurvey.nrcs.usda.gov/app/. The site is located within the Milton Belt of the Piedmont Physiographic Province. The Piedmont Physiographic Province generally consists of low to medium elevation, rounded hills and long rolling northeast to southwest trending ridges with incised creeks and river channels. Rock underlying the site are described as granite. In-place chemical and physical weathering of parent rocks typically form residual soils in the Piedmont region. Weathering is facilitated by the presence of fractures, joints, and less resistant minerals in the rock. In areas not altered by erosion, alluvial deposition or the activities of man, the typical residual soil profile consists of clay-rich unconsolidated materials at the surface, transitioning to sandy silts and silty sands at depth.Typically, the boundary between soil and rock is not sharply defined.This transitional zone is termed “partially weathered rock”.According to information obtained from the Web Soil Survey for Forsyth County,soils underlying the site consist of Toast sandy loam (ToB). In the Piedmont, groundwater generally occurs in pore spaces within soils and in the structural features present in the underlying rock (i.e. joints, fractures, and faults).Recharge to the water table is primarily by precipitation infiltrating the surficial, unsaturated soils (vadose zone) and percolating downward under the influence of gravity to the water table (saturated zone).Typically, the water table is not a level surface, but a subdued reflection of the land surface.The depth to the water table is not consistent and is dependent upon several factors that include the amount of rainfall, permeability of the soil, the extent of fracturing in the underlying rock and the influences of groundwater pumping.Groundwater typically flows in directions perpendicular to lines of equal head and under the influence of gravity, migrates to discharge points such as surface water features, the toes of slopes or natural springs. Please note that fluctuations in the depth of the water table can be expected depending on variations in precipitation, surface water run-off and other factors not evident at the time of our subsurface exploration. Normally, the highest levels of the water table occur in late winter and spring and the lowest levels occur in the late summer and fall.Based on recent groundwater elevation measurements (February 24, 2020),groundwater is calculated to flow to the north. D.2 Sampling Activities D.2.1 LNAPL Evaluation LNAPL was not encountered in the on-site monitoring wells. Subsequent Monitoring Report Parrish Grocery ■Walnut Cove, NC ■Incident No. 11953 March 24, 2020 ■Terracon Project No.75179042 Responsive ■Resourceful ■Reliable 8 D.2.2 Drilling Activities Terracon mobilized to the site on February 24, 2020 to install a replacement well (MW-1R) for well MW-1. Well MW-1R was installed approximately five feet north of well MW-1.The boring was drilled using hollow-stem auger techniques to a depth of approximately 67 feet bgs and was completed by installing a permanent Type II monitoring well.The permanent Type II monitoring well was constructed by the following procedures: n Installation of 10 feet of two-inch diameter, 0.010-inch machine slotted PVC well screen with a threaded bottom cap; n Installation of two-inch diameter, threaded, flush-joint PVC riser pipe to the surface; n Addition of #2 grade sand as an annular sand pack around the well screen from the bottom of the boring to approximately two feet above the top of the well screen; n Addition of approximately two feet of hydrated bentonite chips on top of the sand pack; n Grout remaining annulus above the bentonite seal to the land surface; and n Installation of a flush-mounted, traffic rated protective cover in a square concrete pad. The approximate location of well MW-1R is shown on the Site Plan (Exhibit 2).Well installation and boring log information is attached in Appendix A. D.2.3 Groundwater Sampling Activities Terracon mobilized to the site on February 24 and 25, 2020 to sample monitoring wells MW-1R and MW-2 through MW-6.The approximate locations of the monitoring wells are shown on Exhibit 2. Prior to sample collection, Terracon gauged the groundwater levels in each well using a decontaminated electronic oil-water level indicator.During this sampling event, depth to groundwater in the monitoring wells ranged from approximately 42.17 feet (well MW-4)to 51.35 feet (well MW-1R)below the top of casing.Based on the current data and previous reports, the estimated groundwater flow direction is to the northeast.Copies of Terracon’s field sampling notes are provided in Appendix B.A groundwater elevation map depicting groundwater flow contours and direction is shown on Exhibit 4. After water level measurements were obtained, the wells were purged a minimum of three well volumes using dedicated disposable bailers.Purge water from the monitoring wells was disposed in accordance with NCDEQ guidelines. The wells were allowed to recharge a minimum of 90 percent of the original well volume before samples were collected. A groundwater sample was collected from each well in general accordance with NCDEQ sampling guidelines.The groundwater samples were placed in laboratory-supplied glassware, labeled and stored in an ice-packed cooler.The samples, along with chain-of-custody documentation, were delivered to Con-test Analytical Laboratory East Longmeadow, MA for analysis of VOCs using EPA Method 6200B. E.CONCLUSIONS AND RECOMMENDATIONS Based on information obtained during the completion of these assessment activities, Terracon provides the following conclusions and findings. Several petroleum constituents were detected at concentrations exceeding the 2L standards in well MW-1R. EBD was detected at a concentration exceeding the GCL in well MW-1R. Petroleum constituents were not detected at concentrations exceeding the laboratory reporting limits in wells MW-2 through MW-6. The estimated groundwater flow direction is to the north. LNAPL was not detected in the wells at the site. Based on the presence of EDB (a lead scavenger and identified precluding factor) in groundwater samples, Terracon’s review of Interstate Technology & Regulatory Council (ITRC) Petroleum Vapor Intrusion (PVI) screening guidance suggests the potential exists for vapor intrusion into the on-Site building. Based on the findings of our assessment activities, the following recommendations are provided for the site: Terracon recommends an AFVR event be performed on well MW-1R to reduce petroleum constituent concentrations in the groundwater. Attempt to abandon water supply wells determined to be high risk. Sample on-site monitoring wells MW-1R, MW-3 and MW-5 on a semi-annual basis.
