Summary of Activities Associated with Current TA WithersRavenel, Inc. (WR) completed the sampling of monitoring wells MW-1, MW-2, MW-4, MW-5, MW-7, and MW-8 at the Hawk’s Rental Property incident located at 1712 S. Main Street in Bannertown, Surry County, North Carolina. These activities were completed in association with Task Authorization TA-01, under North Carolina Department of Environmental Quality (NCDEQ) Contract #N70522-C. 2. Soil, Groundwater and LNAPL Assessment Information Soils Soil samples were not collected during the completion of this TA. Soil samples were collected on November 11, 2008, during the installation of MW-1 at 10 feet below ground surface (ft bgs), 15 ft bgs, 20 ft bgs, and 25 ft bgs. The 10 ft and 25 ft soil samples were laboratory analyzed for volatile organic compounds by Environmental Protection Agency (EPA) Method 8260B, and volatile petroleum hydrocarbons (VPH) by the Massachusetts Department of Environmental Protection (MADEP) method. The 15 ft and 20 ft soil samples were only analyzed for MADEP VPH. Exceedances to the soil-to-water Maximum Soil Contaminant Concentrations (MSCCs) were reported in the 10 ft, 15 ft, and 25 ft soil samples. Though not identified as such, petroleum compounds also exceeded some Industrial / Commercial MSCC standards in the collected soil samples. Groundwater Laboratory results for the groundwater sample obtained from MW-1, MW-2, MW-4, MW-5, MW-7, and MW-8 on July 11, 2023, revealed 1,2-Dibromoethane (EDB) at concentrations that exceeded the Gross Contamination Levels (GCLs) in MW-1 and MW-4. Naphthalene and total xylenes were detected above GCLS in MW-5. Several additional compounds were reported at concentrations that exceeded the NCAC 2L Groundwater Quality Standards (2L Standards) in MW-1, MW-2, MW-4, and MW-5. Several volatile compounds were detected above the laboratory method detection limits (MDLs) in MW-8. No volatile compounds were reported at concentrations above the laboratory MDLs for monitoring well MW-7. Light Non-Aqueous Phase Liquid (LNAPL) No LNAPL was measured in July 2023. WR is not aware of the historical presence of LNAPL in MW-1. LNAPL was reported at a thickness of approximately 1-inch in the onsite water supply well on November 11, 2008. The water supply well was subsequently abandoned. 3. Receptor Information Municipal water is supplied to the local area by the Mount Airy Public Works Department. WR completed a receptor survey in March 2021. Thirteen developed properties within 1,500-feet of the source area property were observed to have water supply wells. Eight of these properties do not receive a water bill from the Mount Airy Public Works Department and presumably rely on a WithersRavenel Our People. Your Success. No active water supply wells were identified within 500-feet of the subject property. Three inactive water supply wells were identified within 500-feet of the subject property. WR determined in May 2022 that 209 Devon Drive is connected to the municipal water system and does not use a well for potable water. Two intermittent blue line tributaries were identified by the Surry County Geographic Information Systems (GIS) Department. Approximately 250-feet north of the subject property an unnamed, intermittent tributary to the Ararat River was identified, and approximately 430-feet south of the subject property an intermittent tributary to Seed Cane Creek was identified. According to the Source Water Assessment Program (SWAP) Mapping Application, no wellhead protection areas are located within 1,500-feet of the subject property. According to the SWAP Mapping Application both the Ararat River and Seed Cane Creek discharge into the Yadkin River which is identified as a “Surface Water Source” for the City of Winston-Salem, Davie County, and the City of King. 4. Nearby Fueling Stations/Incidents In December 1998, three 2,000-gallon gasoline USTs were closed in place at Bannertown Exxon located at 1704 South Main Street. Prior to the closure of the USTs, ten closure soil samples were collected around the perimeter of the tank basin in October 1998. No soil samples were collected near the dispenser island or product lines. The samples were analyzed for total petroleum hydrocarbons (TPH) gasoline range organics (GRO). Analytical results indicated that the soil samples were below the minimum reportable concentration of 10 milligrams per kilogram (mg/kg). Based on the closure sample results, an incident number was not assigned to Bannertown Exxon. In April 1995, a leak was reported following the removal of one 1,000-gallon kerosene UST at Bannertown Texaco located at 1708 South Main Street. Bannertown Texaco (Incident # 14473) was given a low priority ranking in July 1996 and the incident was closed in February 1997. 5. Remediation Activities With the exception of the in-place closure of the onsite underground storage tanks (USTs), WR is not aware of any additional remedial actions on the subject property. 6. Recommendations Based on the results of assessment and monitoring activities conducted at the site in association with this TA, WR recommends the following: • Semi-annual monitoring of MW-1, MW-2, MW-4, MW-5, MW-7, and MW-8 • If funds are available, perform a pilot test injection of an in-situ remediation technology in the source area. SITE HISTORY & CHARACTERIZATION WR has completed assessment and sampling activities at the referenced site in association with TA-01. The following information summarizes the site history and characterization. 1. UST Information The site is located at 1712 S. Main Street in Bannertown, Surry County, NC (Figure 1). Two 560-gallon gasoline USTs were closed in-place on the subject property (Figure 2). UST information is provided in Table B-1. 2. Description of Release/General Assessment History The following summary of the assessment history is based on WR’s review of historical files for the subject incident: April 1990 – Tenants at Al Hawk’s rental property located at 1710 S. Main Street reported that their drinking water was contaminated. Subsequently, six water supply wells were determined to have been impacted by petroleum compounds. November 2008 – a geophysical survey was conducted on the subject property to verify the location of the 560-gallon gasoline USTs. December 2008 – Limited Site Assessment (LSA) Report: The LSA documented the installation of MW-1 and soil sampling activities conducted during monitoring well installation. March 2009 – An invoice from Garco, Inc. to North Carolina Department of Environment and Natural Resources (NCDENR) Division of Waste Management (DWM) UST Section indicates that the two USTs at the site were filled with foam as part of the closure in place activities. The invoice also shows that 70-gallons of petroleum contaminated sludge were removed via vacuum truck on the same day (March 26, 2009). April 2009 – The water supply well at 1710 S. Main Street was abandoned. March 2021 - Benzene, EDB, and 1,2-dichloroethane were reported at concentrations that exceeded the GCL in MW-1. Several additional volatile compounds were reported at concentrations that exceeded the 2L Standard. May 2021 – WR oversaw the installation of MW-2 through MW-4. Benzene, EDB, and 1,2- dichloroethane were reported at concentrations that exceeded the GCL in MW-1. EDB was reported at a concentration that exceeded the GCL in MW-4. Several additional volatile compounds were reported at concentrations that exceeded the 2L Standard in MW-1, MW2, and MW-4. WithersRavenel Our People. Your Success. Benzene and EDB were reported at concentrations that exceed the GCL in MW-1 and MW-5. EDB was reported at a concentration that exceeded the GCL in MW-4. Several additional volatile compounds were reported at concentrations that exceeded the 2L Standard in MW-1, MW2, MW- 4, MW- 5, MW-7, and MW-8. May 2022 – WR collected groundwater samples from monitoring wells MW-2, MW-3, MW-4, MW-5, MW-7, and MW-8. Monitoring well MW-1 was obstructed by a parked vehicle and was not sampled. EDB was reported at a concentration that exceeded the GCL in MW-4 and MW-5. Several additional volatile compounds were reported at concentrations that exceeded the 2L Standards in MW-2, MW-7, and MW-8. 3. Brief Description of Site Characteristics The site is located at 1712 S. Main Street in Bannertown, Surry County, NC. The property includes a concrete block vehicle maintenance shop. The structure appeared vacant during the July 2023 assessment. Surface drainage through the area generally follows topography or is channeled through sub- surface stormwater structures or wet-weather ditches running parallel to primary and secondary roadways. Surficial drainage from the subject property flows southerly until it is discharged into an unnamed intermittent tributary of Seed Cane Creek. 4. Information on Receptors/Potential Receptors Municipal water is supplied to the local area by the Mount Airy Public Works Department. WR completed a receptor survey in March 2021. Thirteen developed properties within 1,500-feet of the source area property were observed to have water supply wells. Regional and Site Geology and Hydrology According to the 1985 USGS Geologic Map of the State of North Carolina the site is located in the northwest portion of the Piedmont Physiographic Province of North Carolina and situated in a location characterized by intrusive rocks of the Blue Ridge Belt from the Paleozoic Era. Materials mapped within the area of the site are described as: “Contains biotite, muscovite, and xenocrysts. Includes Whiteside Mountain, Stone Mountain, Mount Airy, Spruce Pine, and other smaller plutons.” Soils described in the LSA Report consisted of orange to brown clayey sands or sandy clays to depth of 15 ft bgs. Saprolitic sandy silt and saprolitic sandy clays were reported from 15 ft bgs to 25 ft bgs. Petroleum odors were reported to a depth of 25 ft bgs in the MW-1 boring. During the installation of MW-2, MW-3, and MW-4 on May 12, 2021, observed soils included sand with clay to approximately 5 ft bgs followed by coarse grained clayey sands to a depth of 25 ft bgs where the boring was terminated. During the installation of MW-5, MW-6, MW-7 and MW- 8 on October 11, 2021, observed soils included sand with clay to approximately 5 ft bgs followed by coarse grained clayey sands to a depth of 25 ft bgs where the borings were terminated. In addition, during the installation of MW-7 and MW-8 buried debris was discovered. Shredded pieces of automobile tires were discovered during the installation of MW-7 and general household refuse and metal debris were discovered during the installation of MW-8. GROUNDWATER SAMPLING – JULY 2023 WR completed assessment activities which included gauging, purging, and sampling monitoring wells MW-1, MW-2, MW-3, MW-4, MW-5, MW-7, and MW-8. Groundwater Elevations Prior to purging of existing site monitoring wells, each monitoring well was gauged for the presence of LNAPL and depth to groundwater elevation. No LNAPL was observed during the July 2023 sampling event. Groundwater elevations ranged from 16.93 ft btoc in MW-2 to 21.80 ft btoc in MW-1 in July 2023. Based on the gauging data, a Hydraulic Gradient Map was created (Figure 3). Groundwater appears to flow in a southerly direction. Monitoring well information and liquid level data is included in Table B-9. Groundwater Sampling Each monitoring well was purged of three well volumes with a dedicated polyethylene bailer. Prior to sample collection natural attenuation parameters were collected using a YSI 556 and recorded in the field book. Samples from the wells were decanted into pre-preserved laboratory provided sample containers. Samples collected from site monitoring wells were submitted to Waypoint for testing by Standard Method (SM) 6200B. Laboratory Analytical Results Analytical results for the groundwater sample revealed the following: • MW-1 and MW-4 – EDB was reported at concentrations that exceeded the GCL. Several additional volatile compounds were reported in MW-1 at concentrations that exceeded the 2L Standards. • MW-5 – Naphthalene and total xylenes were reported at concentrations that exceeded the GCLs. Several additional volatile compounds were reported in MW-5 at concentrations that exceeded the 2L Standards. • MW-2 – Several volatile compounds were reported at concentrations that exceeded the 2L Standard. No compounds were reported at concentrations that exceeded the GCL. • MW-7 and MW-8 – No targeted volatile compounds were reported at concentrations above the laboratory MDLs in MW-7 and MW-8. The following table compares total volatile organic compounds (VOCs) results from MW-5 during the May 2022 groundwater assessment to the total VOC results from MW-5 during the July 2023 groundwater assessment. These results are included in Table B-4 and Figure 4. A benzene isoconcentration map and an EDB isoconcentration map are included as Figures 5 and 6, respectively. CONCLUSIONS & RECOMMENDATIONS This report documents the results of field activities conducted at the site for TA-01, which included the sampling of monitoring wells MW-1, MW-2, MW-4, MW-5, MW-7, and MW-8. Conclusions based on observations, analytical data and research relating to the release at the site combined with historical information include the following: • The property appeared vacant, and the USTs have been closed in-place. • Municipal water is available to the local area. • Eight active water supply wells were identified between 500-feet and 1,500-feet from the subject property. • No active water supply wells were identified within 500-feet of the subject property. • Two intermittent tributaries were identified within 500-feet of the subject property. • No wellhead protection areas are located within 1,500 ft of the site. • No LNAPL was measured during the July 2023 assessment. • EDB was reported at concentrations exceeding the GCL in MW-4 and MW-5. • Additional 2L exceedances were reported in MW-1, MW-2, MW-4, MW-5, MW-7, and MW-8. • The GCL plume is not defined to the north and west of the property boundary. • Total VOC concentrations in MW-5 have increased by 32.85% when comparing July 2023 groundwater data to May 2022 groundwater data. Based on the results of assessment and monitoring activities conducted at the site in association with this TA, WR recommends the following: • Semi-annual monitoring of MW-1, MW-2, MW-4, MW-5, MW-7, and MW-8 • If funds are available, perform a pilot test injection of an in-situ remediation technology in the source area. When appropriate, soil samples may be collected from drill cuttings associated with the boring or well installation. Monitor wells are constructed within select soil borings of one, two, or four-inch inside diameter PVC casing and machine slotted screen in accordance with NCAC Title 15A, Subchapter 02C (Well Construction Standards, Criteria and Standards Applicable to Water Supply and Certain Other Wells). Clean-washed “#2 Torpedo Sand” is typically utilized to fill the boring annulus surrounding the well screen, extending to a minimum of 1-foot above the well screen. A minimum 1-foot thick bentonite clay seal is emplaced above the sand pack and hydrated. Neat cement grout is placed in the remaining annulus space from the upper portion of the bentonite seal to within 1-foot of surface grade. In coastal plain areas where shallow groundwater is encountered, a variance to construction standards may be employed to ensure the upper portion of the well screen intersects the upper aquifer surface. Wellheads may be completed with either concrete-set flush mounted traffic-bearing steel skirted manholes with bolt down covers or steel above-ground “stick-up” well protectors set in a concrete pad. The monitoring wells are secured with locking caps. Temporary wells or piezometers may be completed with the PVC riser cut at or above grade, and are abandoned within 24-hours of their installation. Wells may be developed using a bailer, electric submersible pump, pneumatic pump, or peristaltic pump, dependent upon site conditions. When wells are developed by the driller, they may be developed with any of the methods outlined above, in addition to air or water injection. Soil boring logs and well construction details are typically prepared for presentation in a report appendix. Borings completed in bedrock may be drilled using air rotary, coring, or sonic drilling methods. Groundwater Depth to Water Measurements Groundwater level measurements are typically obtained using an electronic water level indicator which registers a tone and/or illuminated light when the probe contacts groundwater. Depth to groundwater measurements are obtained by lowering the probe attached to a cable usually marked in 0.01 foot intervals into the well until the probe of the meter intersects the water surface. The measurement from the top of the riser pipe or top of casing (TOC) to the water level is recorded to the nearest 0.01 foot. The electronic water level indicator probe and exposed probe cable is decontaminated between measurement locations. Free Product Level Measurements Light Non-Aqueous Phase Liquid (LNAPL) or “free product” present on the water table is measured using an electronic oil/water interface probe. The interface probe has both an audible and illuminating light that indicate when a liquid level is encountered. The oil/water interface probe registers a solid tone and solidly lit light when LNAPL petroleum product is encountered and a beeping tone flashing light when groundwater is encountered. Depth to LNAPL measurements are obtained by lowering the probe attached to a cable marked in 0.01 foot intervals into the well until the probe of the meter intersects the product or groundwater. The measurements from the top of the riser pipe or TOC to the liquid level are recorded to the nearest 0.01 foot. The thickness of LNAPL is calculated by subtracting the depth to LNAPL measurement from the depth to groundwater measurement. The electronic interface probe is decontaminated between measurement locations. Soil Sampling and Field Screening Soil sampling is performed in general accordance with ASTM: 1586-84 where applicable. Sampling procedures are followed to protect the chemical integrity of the soil samples and minimize the potential for cross contamination between sample locations, including discarding of the exterior of the sample which contacts the auger bucket, acetate sleeve, or split barrel sampler, the use of stainless steel sampling equipment, and the thorough decontamination of sampling apparatus prior to use and between sample locations. Soil samples retained as a part of excavation or other intrusive “digging” activities may be collected from the bucket of excavation equipment or using a hand shovel, if appropriate. Care to collect soil samples from excavation equipment includes retrieving native soil from material that has not made contact with the excavating equipment. Samples are collected in sealable bags or by directly filling sample containers as described above. Sampling is performed using new, disposable nitrile gloves for each sample location to ensure sample integrity and minimize the potential for cross contamination or field personnel exposure to contaminants. Sampling equipment such as hand auger extensions and buckets, direct push samplers, split spoons or shovels, are decontaminated prior to their use in the collection of samples. Samples are placed within laboratory grade sample containers provided by the laboratory to which the samples are submitted, labeled, sealed, and placed in an ice filled cooler. An aliquot of the sample is typically placed in a clean sealable bag and allowed to reach ambient air temperature for future screening of volatile vapor content with a photoionization detector (PID), flame-ionization detector (FID) or other portable screening device. Screening of samples with a PID, FID or other field screening device is performed in accordance with device manufacturer specifications for screening, following calibration of the instrument based on manufacturer recommended procedures. Groundwater Sampling Groundwater samples are generally collected using one of three standard methods – through the use of a dedicated, disposable bailer, a peristaltic pump or submersible electric pump. When sampling with a bailer, a minimum of three well volumes of groundwater are extracted from the well as a means of purging, unless the well is purged to a “dry” state prior to the removal of three volumes of water. Prior to collection of the groundwater sample with the bailer, the water level is allowed to recover to within 80% of the original level measured prior to purging. When sampling with a peristaltic or submersible electric pump, polyethylene (PE) tubing (or equivalent) is emplaced into the screened section of the monitoring well, and the pump is used to extract water through the tubing. When using the either of these methods, general water quality measurements including but not limited to pH, temperature, dissolved oxygen (DO), oxygen reductive potential (ORP), specific conductivity and turbidity may be used as methods for determining when an adequate purge of the well has been conducted. When water quality parameters are measured, the resulting data are included in well purge forms or field notes specific to each site and sampling point. Following the purging of three well volumes of groundwater (in the case of the bailed well) or stability of monitored measurements (in the case of the peristaltic or submersible electric pump), the groundwater sample(s) are introduced directly into sample containers provided by the laboratory to which samples are ultimately submitted for analysis. Any samples requiring chemical preservation are contained in pre-preserved container(s) provided by the laboratory. Sampling is performed using new, disposable nitrile gloves for each sample location to ensure sample integrity and minimize the potential for cross contamination of samples or field personnel exposure to contaminants. Water Supply Well Sampling Sampling of water supply wells is accomplished through the collection of a representative sample from an in-line water tap or other point accessible for sampling. Where possible, the sample is collected from a point prior to any filtration or point of entry (POE) system which might be installed on a well. In some cases, samples will be collected from both pre and post-POE locations. Prior to the collection of samples through an in-place sampling point, the tap or sampling port is cleaned with an Alconox® and deionized water solution, rinsed with deionized water, and again with an isopropyl alcohol (IPA) rinse. In the case of active supply wells (e.g., a well with a useable well pump with an active power supply), the well pump is energized and allowed to run for a minimum of ten (10) minutes time, after which a sample is contained in laboratory grade sample containers as described in the previous section. Water quality parameters may be (but are not necessarily) measured in association with the well purging. Supply wells without active power supplies are not generally sampled, as the well seal cannot be compromised by individuals not certified as a well contractor. However, in the case of a well with no power supply or pump installed, samples could be obtained through the use of a bailer, peristaltic pump or electric submersible pump. Decontamination Procedures Decontamination procedures employed in the performance of field activities will vary dependent upon the method(s) of sampling performed at any site, but will generally include the following: • Remove excess soil/sludge from any sampling equipment as needed, • Wash and scrub the equipment with a phosphate-free detergent such as Alconox® or Liquinox® in a contaminant-free tap water solution. Use a brush to remove particulate matter and surface films as necessary, • Rinse thoroughly with tap water, • Rinse thoroughly with analyte-free water (deionized water),  Rinse thoroughly with isopropyl alcohol, • Allow the equipment to air dry, • Prevent inadvertent contamination during storage and transport (accomplished through storage of equipment in foil or plastic bags/sheeting). In the case of disposable bailers and tubing utilized for the purging and sampling of wells, these items may be decontaminated prior to their disposal. In general, no decontamination of these items is necessary prior to their use in sampling.
